Therasphere® pour le traitement du cancer primitif du foie non résécable et du cancer métastatique du foie

Un protocole d'utilisation humanitaire d'exemption de dispositif de Therasphere® pour le traitement des tumeurs hépatiques hépatocellulaires et métastatiques non résécables - HDE #980006
Le but de ce protocole est de fournir un accès supervisé et limité au traitement Therasphere® aux patients atteints d'un cancer primitif du foie et de métastases hépatiques réfractaires à la chimiothérapie qui ne peuvent pas être traités par l'ablation chirurgicale de la partie du foie affectée.
La réponse du patient au traitement et tout effet secondaire du traitement Therasphere® seront examinés.

Critère d'intégration:
- Diagnostic confirmé de carcinome intrahépatique primaire ou métastatique
- Le cancer doit être non résécable avec des options de traitement établies limitées
- Score de statut de performance ECOG 0-2
- 19 ans ou plus
- Capable de comprendre et de fournir un consentement éclairé écrit
Critère d'exclusion:
- Tout résultat de laboratoire avant le traitement dans les 15 jours suivant le traitement démontrant :
- Numération absolue des granulocytes ≤ 1 500/ul
- Numération plaquettaire ≤ 75 000/ul
- Créatinine sérique > 2,0 mg/dl
- Bilirubine sérique ≥ 2,0 mg/dl
- L'une des contre-indications suivantes à l'angiographie et au cathétérisme viscéral sélectif :
- Antécédents d'allergie grave ou d'intolérance à tout produit de contraste, narcotiques, sédatifs ou atropine
- Saignement, diathèse, non corrigeable par les formes habituelles de traitement
- Maladie vasculaire périphérique sévère qui empêcherait le cathétérisme
- Shunt veineux important loin du foie
- Preuve de la délivrance potentielle de plus de 11 mCi (dose absorbée de 20 Gy) de rayonnement dans les poumons lors de 1) la première administration de Therasphere® ; ou 2) administration cumulative de rayonnement aux poumons au cours de plusieurs traitements
- Preuve de tout flux détectable de Tc-99 MAA vers l'estomac ou le duodénum, application de techniques angiographiques établies pour arrêter un tel flux
- Maladie extrahépatique significative représentant une issue imminente à une situation menaçant le pronostic vital
- Dysfonctionnement hépatique sévère ou insuffisance pulmonaire
- Infection active non contrôlée
- Maladie médicale ou psychiatrique sous-jacente importante
- Les femmes enceintes ne peuvent pas participer
