Therasphere® para tratamento de câncer de fígado primário irressecável e câncer de fígado metastático

Um protocolo humanitário de isenção de uso de dispositivo de Therasphere® para tratamento de tumores hepáticos metastáticos e hepatocelulares irressecáveis - HDE #980006
O objetivo deste protocolo é fornecer acesso supervisionado e limitado ao tratamento Therasphere® para pacientes com câncer hepático primário e metástase hepática refratária à quimioterapia que não podem ser tratados por remoção cirúrgica da parte afetada do fígado.
A resposta do paciente ao tratamento e quaisquer efeitos colaterais do tratamento Therasphere® serão examinados.

Descrição
Critério de inclusão:
- Diagnóstico confirmado de carcinoma intra-hepático primário ou metastático
- O câncer deve ser irressecável com opções de tratamento estabelecidas limitadas
- Pontuação de status de desempenho ECOG 0-2
- Idade 19 anos ou mais
- Capaz de compreender e fornecer consentimento informado por escrito
Critério de exclusão:
- Quaisquer achados laboratoriais pré-tratamento dentro de 15 dias após o tratamento demonstrando:
- Contagem absoluta de granulócitos ≤ 1.500/ul
- Contagem de plaquetas ≤ 75.000/ul
- Creatinina sérica > 2,0 mg/dl
- Bilirrubina sérica ≥ 2,0 mg/dl
- Qualquer uma das seguintes contraindicações para angiografia e cateterismo visceral seletivo:
- História de alergia grave ou intolerância a qualquer meio de contraste, narcóticos, sedativos ou atropina
- Sangramento, diátese, não corrigível por formas usuais de terapia
- Doença vascular periférica grave que impediria o cateterismo
- Shunt venoso substancial para longe do fígado
- Evidência de distribuição potencial de mais de 11 mCi (dose absorvida de 20 Gy) de radiação para os pulmões em 1) primeira administração de Therasphere®; ou 2) administração cumulativa de radiação aos pulmões em tratamentos múltiplos
- Evidência de qualquer fluxo detectável de Tc-99 MAA para o estômago ou duodeno, aplicação de técnicas angiográficas estabelecidas para interromper esse fluxo
- Doença extra-hepática significativa representando um desfecho de situação de risco de vida iminente
- Disfunção hepática grave ou insuficiência pulmonar
- Infecção ativa descontrolada
- Doença médica ou psiquiátrica subjacente significativa
- Grávidas não podem participar
