Un protocolo de uso de exención de dispositivo humanitario de Therasphere® para el tratamiento de tumores hepáticos metastásicos y hepatocelulares irresecables - HDE n.º 980006
El propósito de este protocolo es brindar acceso supervisado y limitado al tratamiento con Therasphere® para pacientes con cáncer primario de hígado y metástasis hepáticas refractarias a la quimioterapia que no pueden tratarse mediante la extirpación quirúrgica de la parte afectada del hígado.
Se examinará la respuesta del paciente al tratamiento y cualquier efecto secundario del tratamiento con Therasphere®.

Descripción
Criterios de inclusión:
- Diagnóstico confirmado de carcinoma intrahepático primario o metastásico
- El cáncer debe ser irresecable con opciones de tratamiento establecidas limitadas
- Puntuación del estado de desempeño de ECOG 0-2
- 19 años o más
- Capaz de comprender y proporcionar consentimiento informado por escrito.
Criterio de exclusión:
- Cualquier hallazgo de laboratorio previo al tratamiento dentro de los 15 días posteriores al tratamiento que demuestre:
- Recuento absoluto de granulocitos ≤ 1.500/ul
- Recuento de plaquetas ≤ 75.000/ul
- Creatinina sérica > 2,0 mg/dl
- Bilirrubina sérica ≥ 2,0 mg/dl
- Cualquiera de las siguientes contraindicaciones para la angiografía y el cateterismo visceral selectivo:
- Antecedentes de alergia grave o intolerancia a cualquier medio de contraste, narcóticos, sedantes o atropina
- Sangrado, diátesis, no corregible por las formas habituales de terapia
- Enfermedad vascular periférica grave que impediría el cateterismo
- Derivación venosa sustancial lejos del hígado
- Evidencia de la entrega potencial de más de 11 mCi (dosis absorbida de 20 Gy) de radiación a los pulmones en 1) la primera administración de Therasphere®; o 2) suministro acumulativo de radiación a los pulmones durante múltiples tratamientos
- Evidencia de cualquier flujo detectable de Tc-99 MAA hacia el estómago o el duodeno, aplicación de técnicas angiográficas establecidas para detener dicho flujo
- Enfermedad extrahepática significativa que representa un resultado inminente de una situación que pone en peligro la vida
- Disfunción hepática grave o insuficiencia pulmonar
- Infección activa no controlada
- Enfermedad médica o psiquiátrica subyacente significativa
