Multicenter klinisch onderzoek ter evaluatie van de immunologische respons van vaccinatie tegen infectie door humaan papillomavirus (HPV) 6, 11, 16, 18 bij meisjes die immunosuppressieve therapie krijgen.

De humaan papillomavirussen (HPV) zijn de oorzaak van de meest voorkomende seksueel overdraagbare infectie. Van de oncogene HPV worden HPV 16 en 18 aangetroffen in 70% van de invasieve kankers. Van de niet-oncogene HPV worden HPV 6 en 11 aangetroffen in 90% van de anogenitale wratten. Er zijn momenteel twee profylactische vaccins beschikbaar: Gardasil ® beschermt tegen HPV 6, 11, 16 en 18 en Cervarix ® beschermt tegen HPV 16 en 18. Gardasil ® is geïndiceerd voor de preventie van hooggradige cervicale dysplasie (CIN2-3), kanker van de baarmoederhals, hoogwaardige dysplasie van de vulva (VIN2-3) en genitale wratten.
De keuze voor Gardasil ® hangt samen met het theoretische risico op transplantaatafstoting met het bivalente vaccin en het feit dat de frequentie van anogenitale wratten gerelateerd aan HPV 6 en 11 verhoogd is bij de immuungecompromitteerde populatie.
De immunosuppressieve vrouwen hebben meer kans op abnormale baarmoederhalsuitstrijkjes dan de algemene bevolking.
Een bericht over de leeftijd waarop meisjes gevaccineerd kunnen worden tegen HPV voor een transplantatie werd opgesteld door de Hoge Raad voor Volksgezondheid, waarin werd aanbevolen dat vaccinatie tegen HPV zou kunnen worden aangeboden aan meisjes voor een transplantatie vóór de leeftijd van 14 jaar en volgens gegevens van de MA. De Hoge Raad voor Volksgezondheid herhaalde ook zijn verzoek om specifiek onderzoek te doen naar de vaccinatie van meisjes en jonge vrouwen met een verzwakt immuunsysteem, inclusief degenen die immunosuppressieve therapie krijgen.
Het primaire doel is het evalueren van de persistentie van de immunologische respons op het tetravalente HPV-vaccin 18 maanden na de eerste dosis vaccin.

Inclusiecriteria:
- Vrouwelijk geslacht
- Leeftijd ≥ 9 jaar en < 18 jaar
- Gewicht ≥ 25 kg
- Vaste orgaantransplantatie: nier-, lever-, hart-, long-, darm- of gecombineerde transplantatie; of systemische lupus erythematosus of een andere systemische immuunziekte
- Transplantatie of diagnose van lupus of diagnose van systemische immuunziekte sinds meer dan 6 maanden
- Immunosuppressieve behandeling door antimetabolieten of calcineurineremmers, met of zonder geassocieerde corticosteroïden
- Minimaal vereiste periode van 3 maanden beschouwd als stabiel na transplantatie of zonder terugval van lupus volgens beoordeling door arts
- In geval van seksuele activiteit (beoordeeld door automatische verklaring): aanvang minder dan een jaar voor opname
- Schriftelijke geïnformeerde toestemming ondertekend door de onderzoeker en de wettelijke vertegenwoordigers van de patiënt, en instemming van de patiënt

- Eerdere HPV-vaccinatie
- Immunosuppressieve behandeling door anti-TNF (adalimumab, etanercept, infliximab) of monoklonale antilichamen (rituximab, anakinra, abatacept) gedurende de laatste 3 maanden
- Actieve maligniteit
- Actieve opportunistische infectie
- HIV-infectie
