Spierinsulineresistentie bij veroudering
Het doel van deze studie is om informatie te verschaffen over mogelijke factoren die ten grondslag liggen aan metabole disfunctie, insulineresistentie en verlies van spiermassa bij verouderende spieren.

Studie Doelstellingen:
- Om de effecten te bepalen van door voeding geïnduceerd gewichtsverlies met en zonder toevoeging van lichaamsbeweging op mitochondriale biogenese en energetische capaciteit, cellulaire redoxtoestand en insulineresistentie.
- Om de effecten te bepalen van door voeding geïnduceerd gewichtsverlies met en zonder toevoeging van lichaamsbeweging op intramyocellulaire lipidenprofielen.
- Om de effecten te bepalen van door voeding veroorzaakt gewichtsverlies met en zonder lichaamsbeweging op skeletspiereiwitten die een programma van autofagie en ofwel verlies of behoud van spiermassa mediëren.

Deelname Criteria
Onderzoekers zoeken naar mensen die aan een bepaalde beschrijving voldoen, de zogenaamde geschiktheidscriteria. Enkele voorbeelden van deze criteria zijn iemands algemene gezondheidstoestand of eerdere behandelingen.

Inclusiecriteria:
- 65-80 jaar oud
- Stabiel gewicht (geen winst/verlies van >10 lbs in 6 maanden)
- Sedentair (≤ 1 continue oefening/week)
- Niet-roker
- BMI ≥ 30 kg/m2
- Rustbloeddruk ≤ 150 millimeter systolisch kwik en ≤ 95 millimeter diastolisch kwik
- Opmerking van huisarts/cardioloog voor het vrijgeven van inspanning als positieve stresstestsymptomen werden waargenomen bij inspanningstest
- Moet bereid zijn om gedurende 14 dagen alle diabetesmedicatie te gebruiken en onafhankelijk te zijn in zelfcontrole van de bloedglucose tijdens de wash-outperiodes (alleen mensen met diabetes)
Uitsluitingscriteria:
- Klinisch significante hart- en vaatziekten, waaronder een voorgeschiedenis van een hartinfarct, in het afgelopen jaar
- Perifere vaatziekte
- Lever-, nier-, spier-/neuromusculaire of actieve hematologische/oncologische ziekte
- Klinisch verminderde pols
- Aanwezigheid van bruits in de onderste ledematen
- Voorgeschiedenis van longembolie
- Perifere neuropathie

Experimenteel: Oefentrainingsprotocol
|
Deelnemers volgen een progressief trainingsprogramma van 6 maanden, 4-5 dagen per week, 45 minuten per sessie (180 minuten per week), voornamelijk bestaande uit wandelen (zowel buiten als op een loopband binnenshuis), maar met de mogelijkheid om stationair te lopen. fietsen, elliptische trainers en roeimachines, vergelijkbaar met wat we eerder hebben gebruikt om significante verbeteringen in insulinegevoeligheid op te wekken bij zowel middelbare als oudere volwassenen (52-55).
Vanaf week 8 zullen deze proefpersonen ook 2 niet-opeenvolgende weerstandsoefeningen per week uitvoeren, 30 minuten per sessie, gericht op de belangrijkste spiergroepen met behulp van weerstandsmachines (het totale aantal trainingsdagen zal nog steeds 4 tot 5 zijn).

Meten van intramyocellulaire lipiden

Er zal een percutane spierbiopsie van de vastus lateralis worden uitgevoerd en de intramyocellulaire lipiden zullen worden gemeten door middel van hogedrukvloeistofchromatografie-tandem massaspectrometrie.

De insulinegevoeligheid wordt gemeten met behulp van een intraveneuze katheter (glucoseklemmen) die in de antecubitale ader wordt geplaatst voor daaropvolgende insuline- en glucose-infusies en voor stabiele isotopeninfusies.
Dit zal door insuline gestimuleerde veranderingen in insulinesignaleringseiwitten en metabolisme meten.

Spierkracht en kracht worden gemeten bij deelnemers die alle screeningsprocedures met succes hebben doorlopen. Meting wordt verkregen met behulp van een korte fysieke prestatiebatterij, knie-extensie en spierkrachttesten met behulp van een pneumatisch aangedreven dynamometer. |
