Veiligheid en potentiële werkzaamheid van menselijke mesenchymale stamcellen bij niet-cystische fibrose bronchiëctasie (CELEB)
Een fase I-onderzoek om de veiligheid, verdraagbaarheid en potentiële werkzaamheid van allogene infusie van menselijke mesenchymale stamcellen (hMSC) bij patiënten met niet-cystische fibrose bronchiëctasie te evalueren
Studie Overzicht
Gedetailleerde beschrijving
Er zal een fase 1-onderzoek worden uitgevoerd om de veiligheid te testen van twee doses beenmerg-afgeleide hMSC's (20.000.000 en 100.000.000) toegediend via perifere intraveneuze infusie.
Groep 1: 3 proefpersonen krijgen een enkele toediening van allogene hMSC's: 20 x106 (20 miljoen) cellen toegediend via perifere intraveneuze infusie Groep 2: 3 proefpersonen krijgen een enkele toediening van allogene hMSC's: 1 x108 (100 miljoen) cellen toegediend via perifere intraveneuze infusie Tussentijdse veiligheidsanalyse zal worden uitgevoerd vier weken nadat de eerste proefpersoon in elk cohort is opgenomen. Voortdurende veiligheid en verdraagbaarheid met beoordeling van ongewenste voorvallen (AE's) zal bij elk bezoek worden beoordeeld. Werkzaamheidsparameters (longfunctietesten, longdiffusiecapaciteit, longvolumes, 6-Minute Walk Test (6MWT) en vragenlijsten voor kortademigheid/kwaliteit van leven (QOL)) zullen elke 12 weken worden beoordeeld totdat de studie is voltooid. Bij elk bezoek worden klinische laboratoriumtests uitgevoerd om de veiligheid te beoordelen.
High Resolution Computed Tomography (HRCT)-scan zal worden uitgevoerd tijdens het basisbezoek (indien niet gedaan binnen drie maanden voorafgaand aan inschrijving) en vervolgens in week 24.

Beschrijving
Inclusiecriteria:
- Geef schriftelijke geïnformeerde toestemming,
- tussen 30 en 87 jaar oud zijn op het moment van ondertekening van de geïnformeerde toestemming,
- gewicht meer dan 45 en minder dan 150 kg,
- een klinische diagnose van niet-CF-bronchiëctasie hebben voorafgaand aan de screening,
- Minstens 2 exacerbaties hebben gehad in het afgelopen jaar, zoals gedocumenteerd door dokterspraktijken of ziekenhuisbezoeken (gebruik van antibiotica van minstens één keer in het afgelopen jaar),
- Toon een baseline FEV1 tussen 25% en 85% voorspeld en meer dan of gelijk aan 1 L en een baseline diffusiecapaciteit van de long voor koolmonoxide (DLCO) van meer dan of gelijk aan 30% (gecorrigeerd voor hemoglobine maar niet alveolair volume),
- Een normale rechterventrikelfunctie hebben, zoals gedocumenteerd door Doppler-echo of rechterhartkatheterisatie,
- als het een vrouw is die zwanger kan worden, ga ermee akkoord zich te houden aan de hieronder beschreven anticonceptieregels.
- Onderwerpen kunnen niet-medicamenteuze therapieën krijgen, waaronder zuurstofsuppletie van niet meer dan 4 liter per minuut en longrevalidatie.
- Proefpersonen kunnen chronische bronchiëctasie ondergaan met macrolide of geïnhaleerde antibiotica
Uitsluitingscriteria:
- HRCT- en/of chirurgische longbiopsieresultaten hebben die niet overeenkomen met de diagnose van niet-CF-bronchiëctasie. (Uitsluiting van emfyseem en/of diffuse parenchymziekte)
- niet in staat zijn om een van de beoordelingen uit te voeren die vereist zijn voor eindpuntanalyse (veiligheids- of verdraagbaarheidsproblemen melden, longfunctietests (PFT) of HRCT uitvoeren, bloedafnames ondergaan, vragenlijst lezen en beantwoorden
- Als een vrouw in de vruchtbare leeftijd een follikelstimulerend hormoon (FSH) heeft van minder dan 25,8 IE/L
- actief worden behandeld voor een acute infectieuze exacerbatie van bronchiëctasie
- Heb een actieve infectie die niet wordt behandeld
- Actieve infecties hebben gehad binnen minimaal 4 weken van de studiebehandeling
- Momenteel onder behandeling zijn voor NTM-infecties
- In de afgelopen 6 maanden positieve sputumkweken gehad voor niet-tuberculeuze mycobacteriën (NTM)
- Een voorgeschiedenis hebben van drugs- of alcoholmisbruik in de afgelopen 24 maanden.
- Momenteel experimentele middelen krijgen (of binnen vier weken na screening hebben gekregen) voor de behandeling van bronchiëctasie of in de afgelopen 30 dagen zijn opgenomen in klinische onderzoeken
- Wees actief vermeld (of verwacht toekomstige lijst) voor transplantatie van een orgaan.
- Klinisch belangrijke afwijkende screeningslaboratoriumwaarden hebben.
- Een ernstige comorbide ziekte hebben die, naar de mening van de onderzoeker, de veiligheid of therapietrouw van de patiënt in gevaar kan brengen of een succesvolle afronding van het onderzoek in de weg kan staan.
- Een andere aandoening hebben die, naar de mening van de onderzoeker, de veiligheid of therapietrouw van de patiënt in gevaar kan brengen of een succesvolle afronding van het onderzoek in de weg kan staan.
- Allergieën hebben gekend voor penicilline of streptomycine.
- Wees een ontvanger van een orgaantransplantatie.
- Een klinische voorgeschiedenis van maligniteit hebben binnen 5 jaar (d.w.z. patiënten met een eerdere maligniteit moeten gedurende 5 jaar ziektevrij zijn), behalve curatief behandeld basaalcelcarcinoom van de huid, plaveiselcelcarcinoom van de huid of cervicaal carcinoom.
- Een niet-pulmonale aandoening hebben die de levensduur beperkt tot minder dan 1 jaar.
- Wees serumpositief voor HIV, hepatitis BsAg (surface agent reactive) of viremische hepatitis C.
- Overgevoelig zijn voor dimethylsulfoxide (DMSO)
- Niet in staat zijn om een verzadigde zuurstof (SpO2) van meer dan 93% in de kamerlucht op zeeniveau in rust te handhaven) of een SpO2 van meer dan 88% in de kamerlucht boven 5000 voet (1524 meter) boven zeeniveau in rust.
