Beschrijving
Inclusiecriteria:
- Leeftijd ≤21 jaar oud
- Chiari misvorming type I ≥5 mm tonsillaire ectopie
- Syrinx tussen 3 mm en 6 mm
- Chiari Severity Index (CSI) classificatie 1
- MRI van de hersenen en cervicale en thoracale wervelkolom zijn vereist voorafgaand aan de operatie en moeten beschikbaar zijn om te worden gedeeld met het Data Coordinating Centre
Uitsluitingscriteria:
- CSI-2 of CSI-3 classificatie
- Syrinx
- Neuro-imaging die basilaire invaginatie aantoont
- Hoek van het Clival-kanaal
- Chiari Malformatie I + syringomyelie secundair aan andere pathologie (bijv. een tumor)
- Kan pre-decompressie MRI van de hersenen en de cervicale en thoracale wervelkolom niet delen
- Patiënten die niet willen deelnemen

Experimenteel: Posterior fossa decompressieoperatie
Het bot wordt operatief verwijderd uit het suboccipitale gebied van de schedel en Cervical 1 lamina zodat de vernauwende epidurale band kan worden gereseceerd

Geplande gebieden voor het verwijderen van bot uit het suboccipitale gebied van de schedel en de cervicale l1 lamina.
Vervolgens wordt de vernauwende epidurale band ter hoogte van het foramen magnum gereseceerd.

Experimenteel: Durale augmentatie-operatie
Het bot wordt verwijderd uit het suboccipitale gebied van de schedel en Cervical 1 lamina zodat de vernauwende epidurale band kan worden gereseceerd.
Vervolgens wordt de dura geopend.
Microchirurgische dissectie wordt uitgevoerd en de dura wordt dichtgenaaid.

De dura wordt scherp geopend, waardoor de cerebellaire amandelen, de hersenstam en het bovenste ruggenmerg zichtbaar worden.
Na microchirurgische dissectie wordt de dura dichtgenaaid met een duratransplantaat.

Doel 3: Gemiddelde verandering in meting van de kwaliteit van leven na een operatie ten opzichte van de uitgangswaarde.

De kwaliteit van leven (QOL) werd beoordeeld met behulp van de Chiari Health Index Pediatrics (CHIP) en de Health Utilities-3 (HUI-3). De CHIP is een 5-punts antwoordschaal. Binnen het domein van de lichamelijke symptomen worden vijf vragen met betrekking tot de ernst van de pijn afwisselend geschaald. De Health Utilities Index Mark 3 (HUI3) is een generiek statusclassificatiesysteem dat generaliseerbaar is bij pediatrische patiënten met een ziekte. De HUI3 beoordeelt domeinen zoals visie, gehoor, spraak, lopen, behendigheid, emotie, cognitie en pijn. In de analyse hebben we gebruik gemaakt van een non-inferioriteitsbenadering. De score wordt gecorrigeerd voor ontbrekende waarden en geschaald naar een bereik van scores tussen 0 en 1. Een stijgende score vertegenwoordigt een stijgende HRQOL. Toename/afname van de levenskwaliteitsmetriek (op een schaal van 0-1, waarbij 0 staat voor geen levenskwaliteit en 1 voor 100% levenskwaliteit). |
