TORS De-Intensification Protocol Versie 2.0: Dosis- en volumevermindering in de nek

Een fase II-onderzoek naar de-intensificatie van volume en dosis na transorale robotchirurgie (TORS) en nekdissectie voor p16+ orofaryngeaal plaveiselcelcarcinoom
Dit is een eenarmige fase II-studie van adjuvante bestraling voor lokaal gevorderd p16+ orofaryngeaal plaveiselcelcarcinoom.
Het belangrijkste doel van dit onderzoek is het bepalen van de waarschijnlijkheid dat kanker twee jaar na voltooiing van de bestraling teruggroeit in de keel of in de nek als lagere doses straling worden gebruikt voor een kleiner gebied van het hoofd-halsgebied dan momenteel wordt gebruikt in zorgstandaard.

Deelname Criteria

Inclusiecriteria:
- Patiënten >= 18 jaar oud
- Histologisch bevestigde diagnose van plaveiselcelcarcinoom van de orofarynx, p16-positief op immunohistochemie
- Pathologische T0 (onbekende primaire), T1-, T2- of T3-ziekte (volgens AJCC 7th Ed)
- Pathologische ziekte N0, N1, N2a of N2b (volgens AJCC 7th Ed), met < 5 positieve lymfeklieren
- ECOG-prestatiestatus 0-1
Uitsluitingscriteria:
- Voorafgaande radiotherapie aan het hoofd en de nek
- Aanwezigheid van T4-ziekte
- Aanwezigheid van nekziekte N2c of N3 (volgens AJCC 7th Ed)
- >= 5 lymfeklieren
- Aanwezigheid van metastatische ziekte op afstand

Experimenteel: Arm 1
Bij alle patiënten zal het behandelde volume en de stralingsdosis die wordt toegediend aan de regionale lymfevaten worden verlaagd volgens de kenmerken van de primaire locatie en de betrokken lymfeklieren.
De nek met hoog risico krijgt 50 Gy in plaats van 60 Gy, en het behandelde volume van de contralaterale nek met laag risico wordt verkleind en krijgt slechts 45 Gy.

Patiënt rapporteerde kwaliteit van leven (QOL)-resultaten met behulp van MD Anderson Symptom Inventory (MDASI) Head and Neck-enquête tussen patiënten behandeld met IMRT en IMPT.
De MDASI-HN beoordeelt de ernst van de symptomen in de afgelopen 24 uur met behulp van een schaal van 0 tot 10, waarbij 0 staat voor "niet aanwezig" en 10 voor "zo erg als u zich kunt voorstellen".
De interferentie van symptomen in de afgelopen 24 uur wordt beoordeeld met behulp van een schaal van 0-10, waarbij 0 staat voor "niet gestoord" en 10 voor "volledig gestoord".
Het gemiddelde van de ernst van de symptomen en interferentie kan worden gebruikt om de algemene symptomen van lijden weer te geven en kan worden geanalyseerd op veranderingen tijdens de behandeling en tijdens de follow-up.
