Een vroege en correcte behandeling met antibiotica heeft een fundamenteel effect op de overleving bij ernstige infecties en op het voorkomen van resistentieontwikkeling. Het is echter onduidelijk of dit wordt bereikt bij ernstig zieke patiënten met ernstige infecties. Onderdosering van antibiotica leidt tot gebrek aan effect tegen bacteriën en selectie van multiresistente stammen. Overdosering van antibiotica verhoogt het risico op toxiciteit en vormt een bedreiging voor het milieu. Momenteel is er geen methode voor snelle of bedside bepaling van de antibioticumconcentratie in de reguliere gezondheidszorg.
Pharmacolog AB, een Med-tech-bedrijf uit Uppsala, heeft een technologie en een product DrugLog® ontwikkeld op basis van absorptiespectroscopie in ultraviolet - zichtbare (UV-VIS) frequenties die de concentratie van antibiotica kunnen meten.
Het doel van dit project is om te onderzoeken of antibioticaconcentraties bij patiënten kunnen worden gevolgd door UV-VIS-spectroscopie.
In een prospectieve observationele studie zullen bloedmonsters van 100 patiënten met ernstige infecties die worden behandeld met cefotaxim, piperacilline/tazobactam of meropenem worden gemeten met UV-VIS-spectroscopie en met de gouden standaard, hogedrukvloeistofchromatografie-massaspectrometrie (HPLC- MS) bij Klinisk farmakologi, Huddinge ziekenhuis, Stockholm.
Na geïnformeerde toestemming worden monsters genomen voorafgaand aan de antibioticabehandeling en driemaal daarna. Demografische en klinische gegevens worden geregistreerd.

Inclusiecriteria:
- Meer dan 18 jaar oud
- Vermoedelijke infectie waarbij behandeling met cefotaxim, piperacilline/tazobactam of meropenem is voorgeschreven
Uitsluitingscriteria:
- Zwangerschap
- Intermitterende hemodialyse
- Patiënt met beperkte behandelbeslissing
