Opzet: De onderzoekers maken gebruik van een quasi-experimenteel aanmoedigingsonderzoek waarbij proefpersonen hun eigen CIH-therapieën kunnen kiezen, maar sommigen krijgen extra aanmoedigingen (duwtjes) aangeboden om specifieke CIH-therapieën te kiezen. In plaats van gerandomiseerd te worden naar een CIH-therapie, zullen individuen een van de twee studiegroepen binnengaan op basis van welke CIH-therapieën ze zes maanden nadat ze met een CIH-therapie zijn begonnen, hebben gebruikt. Hoewel toerekening aan de onderzoeksgroep niet willekeurig is, hebben sommige sites sterke zakelijke praktijken/duwtjes die leiden tot aanzienlijke variatie in de CIH-therapieën die patiënten gebruiken. De variatie die door deze zakelijke praktijken/nudges wordt veroorzaakt, biedt een quasi-experimenteel natuurlijk experiment dat kan worden beoordeeld door middel van een analytische benadering van instrumentele variabelen. Het doel van het gebruik van bedrijfspraktijken als instrument om patiënten toe te voegen aan de behandelingsgroep, vergelijkbaar met het gebruik van randomisatie, is om mogelijke selectie en confounding bias te verminderen. Het behandelingsvenster van 6 maanden stelt patiënten in staat om één type CIH-therapie te starten en aanvullende CIH-therapieën toe te voegen op basis van zakelijke praktijken/duwtjes op hun locatie binnen de evaluatieperiode. De CIH-therapieën die patiënten ontvangen, zullen zijn zoals ze worden geleverd door de 18 VHA Whole Health vlaggenschipstudiecentra en gemeenschapszorgpraktijken.
Locaties: De onderzoekslocaties zijn de 18 Whole Health Flagship-locaties van de VA: VA Boston Health Care System, VA New Jersey Health Care System, Erie VAMC, Beckley VAMC, WG (Bill) Hefner VAMC, Atlanta VAMC, Tampa VAMC, Tennessee Valley Health Care System, Aleda E. Lutz VAMC, Tomah VAMC, St. Louis VA Health Care System, Central Arkansas Veterans Healthcare System, South Texas Veterans Health Care System, Salt Lake City VAMC, VA Portland Health Care System, Palo Alto VAMC, Tucson VAMC, VA Nebraska-Western Iowa gezondheidszorgsysteem).
Secundaire gegevens van de CIH Experience Survey/ VA Office of Patient Centered Care and Cultural Transformation's Survey: De APPROACH-studie voert voornamelijk een secundaire analyse uit van door de patiënt gerapporteerde gegevens die worden verzameld door de VA OPCC&CT-enquête. Dat is een 4-puntsonderzoek (baseline en maanden 1, 3, 6). Vrijwel alle onderstaande uitkomsten komen uit dat onderzoek.

Inclusiecriteria:
- Veteraan
- Geschiedenis van (chronische) musculoskeletale pijnaandoeningen geregistreerd in het EPD in het jaar voorafgaand aan het indexbezoek, en zelfgerapporteerde pijn die elke dag of bijna elke dag aanwezig was gedurende 3 maanden vanaf het CIH-indexbezoek met behulp van een geschiktheidsscreener
- 18-89 op index CIH-bezoekdatum
Uitsluitingscriteria:
- diagnose van een ernstige psychische aandoening in het jaar voorafgaand aan de start van CIH
- voorgeschiedenis van dwarslaesie
- ziekenhuisopname 30 dagen voorafgaand aan het starten van CIH
- geregistreerd CIH-gebruik in het EPD in de 6 maanden voorafgaand aan het index-CIH-bezoek en gerapporteerd CIH-gebruik in de 8 weken voorafgaand aan het index-CIH-bezoek
