Projeto: Os investigadores estão empregando um projeto de estudo de incentivo quase experimental no qual os indivíduos são capazes de escolher suas próprias terapias de CIH, mas alguns recebem incentivos extras (empurrões) para escolher terapias específicas de CIH. Em vez de serem randomizados para uma terapia de CIH, os indivíduos entrarão em um dos dois grupos de estudo com base nas terapias de CIH que usaram seis meses após iniciarem qualquer terapia de CIH. Embora o recrutamento para o grupo de estudo não seja aleatório, alguns locais têm fortes práticas/empurrões de negócios que levam a uma variação considerável no uso de terapias de HIC pelos pacientes. A variação induzida por essas práticas/nudges de negócios fornece um experimento natural quase experimental que pode ser avaliado por meio de uma abordagem analítica de variáveis instrumentais. O propósito de usar as práticas de negócios como um instrumento para agregar pacientes ao grupo de tratamento, semelhante ao uso da randomização, é reduzir a seleção potencial e o viés de confusão. A janela de tratamento de 6 meses permitirá que os pacientes iniciem um tipo de terapia de HIC e adicionem terapias de HIC adicionais com base em práticas/incentivos de negócios em seu local durante o período de avaliação. As terapias de CIH que os pacientes recebem serão fornecidas pelos 18 principais locais de estudo da VHA Whole Health e práticas de cuidados comunitários.

(Bill) Hefner VAMC, Atlanta VAMC, Tampa VAMC, Tennessee Valley Health Care System, Aleda E. Lutz VAMC, Tomah VAMC, St. Louis VA Health Care System, Central Arkansas Veterans Healthcare System, South Texas Veterans Health Care System, Salt Lake City VAMC, VA Portland Health Care System, Palo Alto VAMC, Tucson VAMC, VA Nebraska-Western Iowa Health Care System).
Dados Secundários da Pesquisa de Experiência do CIH/Pesquisa do VA Office of Patient Centered Care e da Transformação Cultural: O estudo APPROACH está conduzindo predominantemente uma análise secundária de dados relatados pelo paciente sendo coletados pela pesquisa VA OPCC&CT. Essa é uma pesquisa de 4 pontos no tempo (linha de base e meses 1, 3, 6). Quase todos os resultados listados abaixo são dessa pesquisa.

Critério de inclusão:
- Veterano
- Histórico de condições de dor musculoesquelética (crônica) registradas no EHR no ano anterior à visita índice e relato de dor presente todos os dias ou quase todos os dias durante 3 meses a partir da visita índice do CIH usando um rastreador de elegibilidade
- 18-89 na data da visita do índice CIH
Critério de exclusão:
- diagnósticos de doença mental grave no ano anterior ao início do CIH
- história de lesão medular
- internação 30 dias antes do início da HIC
- registrou o uso de CIH no EHR nos 6 meses anteriores à visita de índice de CIH e relatou o uso de CIH nas 8 semanas anteriores à visita de índice de CIH

Scores are reported on a T-score metric wherein the mean score of the general population is standardized to 50 with a standard deviation of 10. A score of 50 indicates average physical health, as compared to the general population. Higher scores indicate better physical health. |
