De Brain and Heart Task Force van de World Stroke Organization (WSO) ontwikkelde The Brain & hEart globAl iniTiative (BEAT), een pilot-haalbaarheidsprogramma gericht op het tot stand brengen van klinische samenwerking tussen cardiologen en artsen die een beroerte hebben gehad.1 Het WSO BEAT-pilotproject richtte zich op atriale fibrillatie (AF ) en patent foramen ovale (PFO) detectie en behandeling, zeer voorkomende cardio-embolische oorzaken van ischemische beroerte en transiënte ischemische aanval (TIA).
Het WSO BEAT-pilotprogramma voor haalbaarheid omvatte 10 locaties uit 8 landen, waaronder Brazilië, China, Egypte, Duitsland, Japan, Mexico, Roemenië en de VS. Van de tien locaties behoorden er twee uitsluitend tot de publieke sector en één was publiek en privaat. Het primaire doel van het pilot-WSO BEAT-programma was het beoordelen van de haalbaarheid ervan door het bereiken van een samengesteld haalbaarheidsresultaat, waaronder: (1) het ontwikkelen van locatiespecifieke klinische trajecten voor de diagnose en behandeling van AF, PFO en het beroerte-hartsyndroom; (2) het opzetten van regelmatige neurocardiologische rondes (bijvoorbeeld maandelijks, tweemaandelijks, enz.); en (3) het opnemen van een cardioloog in het beroerteteam.
Over het geheel genomen heeft het WSO BEAT-programma op de meeste locaties met succes de vooraf gespecificeerde doelen bereikt. Met betrekking tot de drie primaire programmadoelstellingen was 9/10 (90%) van de locaties in staat om respectievelijk de voorgestelde klinische zorgtrajecten te ontwerpen of bij te werken, neurocardiologierondes op te zetten en een toegewijde cardioloog voor het beroerteteam aan te wijzen.
Tot op heden is het onbekend of de implementatie van educatieve hulpmiddelen op het gebied van neurocardiologie en de samenwerking tussen cardiologen en neurologen de naleving van de aanbevelingen voor beste praktijken gericht op de behandeling van cardio-embolische beroertes kunnen verbeteren.

Placebo-vergelijker: No intervention arm - Standard of Care for the institution
This group will not receive any of the educational and/or implementation Neurocardiology until the data collection is completed.

Participants in the non-intervention arm will be followed for up to 12 months.
Sites randomized to this arm will not receive any educational or implementation tools during the follow-up period.
After the 12-month follow-up has been completed, sites may be provided with access to the Neurocardiology tool.

Actieve vergelijker: Intervention arm - Educational and implementation Neurocardiology tool
Sites randomized in this arm will receive the educational and implementation Neurocardiology tool with the aim of improving adherence to best practice recommendations for secondary stroke prevention of cardioembolic strokes, as defined by the WSO Systematic Review and Synthesis of Global Stroke Guidelines or more recent high-quality evidence published after the last stroke guidelines.

Sites randomized to the intervention arm will receive an educational and implementation-focused Neurocardiology tool designed to enhance adherence to best practice recommendations for secondary prevention of cardioembolic stroke.
These recommendations are based on the World Stroke Organization (WSO) Systematic Review and Synthesis of Global Stroke Guidelines, supplemented by any high-quality evidence published after the most recent stroke guidelines.
In addition, participating sites will be provided with 14-day Holter monitors for all patients enrolled in the study.

It is composed of 8 categories (initial workup, stroke-heart syndrome, ESUS workup, patent foramen ovale, atrial fibrillation management, etc.) with a weighted scoring system (see Table 1). Scale range: 0 (no adherence) to 10 (full adherence). Interpretation: Higher scores indicate greater adherence to guideline-recommended care. Calculation: Each patient's score is calculated by dividing the total points achieved by the total possible points for which the patient is eligible, as not all patients qualify for ESUS, PFO, or AF items. |
