The presence of damage to the central and/or peripheral nervous system resulting from various pathologies, such as Multiple Sclerosis (MS), Amyotrophic Lateral Sclerosis (ALS), Parkinson's disease (PD), Alzheimer's disease (AD), dementia, traumatic brain injury (TBI), stroke, or other neurological syndromes, is commonly a cause of both physical and mental disability. Dit leidt tot symptomen bij de patiënt, waaronder: pijn, migraine, hoofdpijn, neuropathische pijn, trigeminale neuralgie, depressie, angst, apathie, vermoeidheid, cognitieve achteruitgang, afasie, functionele motorische stoornissen (FMD), neuromusculaire toonveranderingen, en hyposthenia, in advertentie, geheugen, geheugen, geheugen, geheugen, geheugen. Executive functies, sociale cognitie en emotionele cognitie. De aanwezigheid van deze neurologische symptomen is vaak zichtbaar in een eerste klinisch onderzoek en is een van de belangrijkste redenen voor verdere overleg met de gezondheidszorg. Deze moeilijkheden hebben een grote invloed op de kwaliteit van leven, die werk, sociaal en gezinsfunctioneren beïnvloeden.
In de afgelopen jaren zijn verschillende niet-invasieve hersenstimulatie (NIB's) technieken ontstaan, gericht op het opwekken van neurale netwerken van hersenen, zoals transcraniële statische magnetische veldstimulatie (TSMS) en transcraniële directe stroomstimulatie (TDC's).
TSMS is een NIBS -techniek die de toepassing van een neodymiummagneet op de hoofdhuid omvat. Sinds de eerste studie die deze methode voorstelt, hebben verschillende anderen bevestigd dat TSM's corticospinale prikkelbaarheid kunnen verminderen. TDC's omvatten de toepassing van zwakke elektrische stromen die in staat zijn om een elektrisch veld te genereren dat neurale activiteit op een exciterende of remmende manier kan moduleren. Nibs -technieken kunnen experimenteel worden gebruikt om corticale activiteit te moduleren.
Het primaire doel van dit voorstel is om de impact van neurologische symptomen aan te pakken door de combinatie van TSM's met TDC's en revalidatietechnieken. In het bijzonder wil het begrijpen of de combinatie van deze neuromodulerende therapeutische nibs -methoden de symptoomverbetering bij patiënten met neurologische aandoeningen kan verbeteren.
Om de impact van deze interventie te beoordelen, zal een reeks tests en vragenlijsten, hieronder beschreven in detail, worden gebruikt om de ernst van de gerapporteerde symptomen en secundaire resultaten te evalueren.
Bovendien zal de bijdrage van specifieke hersengebieden aan het symptoom worden geëvalueerd door de directe modulatie van hersenactiviteit. Deze modulatie zal worden bereikt met behulp van een extra NIBS -techniek, zoals transcraniële magnetische stimulatie (TMS). Vooral TMS is een niet-invasieve methode voor het stimuleren van neuronen in de oppervlakkige gebieden van de hersenen, die sinds de introductie vaak in neurologie worden gebruikt als diagnostisch en onderzoekstool. TMS gebruikt magnetische velden om elektrische stromen te induceren die in staat zijn om corticale activiteit te vergemakkelijken of te remmen.

Beschrijving
Inclusiecriteria:
- Mannetjes of vrouwen tussen 18 en 80 jaar;
- Aanwezigheid van een neurologische aandoening, met name de volgende aandoeningen zullen worden overwogen: MS, ALS, PD, AD, Dementias, TBI, Neuroschirurgische interventies, beroerte, fibromyalgie, Epilepsie, hoofdpijn, migraine, met ten minste een van de volgende symptomen: Pijn, Neuropathische pijn, Neuropathische pijn, Neuropathische pijn, Neuropathische pijn, Neuropathische pijn, neuropathische pijn, neuropathische pijn, neuropathische pijn, neuropathische pijn, neuropathische pijn, neuropathische pijn, neuropathische pijn, neuropathische pijn, neuropathische pijn, neuropathische paar aandoeningen (FMD), neuromusculaire toonveranderingen, hyposthenie, betrokkenheid van meerdere cognitieve functies (inclusief besluitvorming, probleemoplossing, leren, geheugen, uitvoerende functies, sociale en emotionele cognitie);
- Patiënten moeten in staat zijn om de protocolinstructies voor de duur van het onderzoek te volgen;
- In staat zijn om de doelen en risico's van de studie te begrijpen;
- In staat zijn om schriftelijke geïnformeerde toestemming voor de studie te begrijpen en te verstrekken.
Uitsluitingscriteria:
- Gedeeltelijk of totaal onvermogen om beslissingen te begrijpen of te nemen, onvermogen om schriftelijke geïnformeerde toestemming voor de studie te geven;
- Patiënten met een geschiedenis of aanwezigheid van een onstabiele medische aandoening, zoals neoplasmata of infecties;
- Vrouwen met een positieve zwangerschapstest bij aanvang of van plan zijn zwanger te worden. Vrouwen die borstvoeding geven of de afgelopen drie maanden voorafgaand aan het begin van het onderzoek zijn bevallen;
- Gebruik van medicijnen die het risico op aanvallen verhogen (bijv. Fampridine, 4-aminopyridine);
- Gelijktijdig gebruik van medicijnen die synaptische transmissie en plasticiteit kunnen veranderen (L-DOPA, anti-epileptica);
- In het geval van het gebruik van NIBS -technieken, mogen proefpersonen geen contra -indicaties hebben die specifiek zijn voor deze methode (zie voor meer informatie de "methoden" en de "stimulatiebeoordelingsvragenlijst" die aan dit voorstel zijn gekoppeld).

De Expanded Disability Status Scale (EDSS) is een door clinicus geregistreerde beoordelingsschaal die de functionele systemen van het centrale zenuwstelsel evalueert.
De EDSS wordt gebruikt om ziekteprogressie te beschrijven bij patiënten met multiple sclerose MS en om de effectiviteit van therapeutische interventies in klinische onderzoeken te beoordelen.
Het bestaat uit een ordinaal beoordelingssysteem variërend van 0 (normale neurologische status) tot 10 (overlijden vanwege MS) in 0,5 stappen interval (bij het bereiken van EDSS 1).

Meting van motorische cortexplasticiteit (ITBS) door transcraniële magnetische stimulatie (TMS).
De gemiddelde amplitudes van motor opgeroepen potentialen (MEP) worden opgenomen na ITBS, op elk tijdstip na ITBS (5, 15 en 30 minuten).
Het LTP-achtige effect zal worden uitgedrukt als procentuele veranderingen ten opzichte van de baseline-EP's.

De bloedmonsters, direct na de verzameling, zullen de nodige procedures ondergaan voor het isoleren van de verschillende componenten.
Deze zullen vervolgens worden gebruikt om de niveaus van microRNA, cytokines, chemokines, cellulaire groeifactoren, markers van neuronale schade (TAU, gefosforyleerde en afgeknotte tau, neurofilamenten), mitochondriale markers (lactaat) en vrije D-amino-autjes te analyseren.
Genotyperingstudies zullen worden uitgevoerd om enkele nucleotide polymorfismen (SNP's) te identificeren in coderende en/of regulerende regio's van genen (microRNA of eiwit) die correleren met specifieke klinische parameters en de niveaus van de geïdentificeerde potentiële biomarkers.

De korte pijninventaris (BPI) is een zelf toegediende maat voor de sensorische en reactieve dimensies van pijn die de ernst of intensiteit van de pijn is en het niveau van interferentie die het heeft op verschillende aspecten van het leven.
Interferentie is verdeeld in activiteit en affectieve subdimensies.
De BPI begint met een screeningvraag, die vraagt naar de aanwezigheid van pijn en een lichaamskaart wordt gebruikt om pijnlijke gebieden en het slechtste gebied aan te geven.
Deze items zijn niet geëvalueerd.
Dit wordt gevolgd door de kern BPI -items: de beoordelingsschalen voor ernst en interferentie van pijn.
Numerieke beoordelingsschalen van 0 tot 10 worden gebruikt voor alle items.
De ankers voor pijnschalen van pijn zijn 0 = 'geen pijn' en 10 = 'pijn zo slecht als je je kunt voorstellen', terwijl de interferentie -ankers 0 = 'geen interferentie' zijn en 10 = 'interfereert volledig'.

De Zung Self-Rating Depression Scale (SDS) is een kort zelf toegediende enquête om de depressieve status van een patiënt te kwantificeren. Er zijn 20 items op de schaal die de vier gemeenschappelijke kenmerken van depressie beoordelen: het doordringende effect, de fysiologische equivalenten, andere storingen en psychomotorische activiteiten. Er zijn tien positief geformuleerde en tien negatief geformuleerde vragen. Elke vraag wordt gescoord op een schaal van 1-4 (een beetje van de tijd, soms, een groot deel van de tijd, meestal). De scores variëren van 25-100.

De Fibromyalgia Impact Questionnaire (FIQ) is een kort, zelfbeheerd instrument met 10 items dat fysiek functioneren, werkstatus, depressie, angst, slaap, pijn, stijfheid, vermoeidheid en welzijn meet.
De FIQ bestaat uit 10 items.
Het eerste item bevat 11 vragen met betrekking tot fysiek functioneren - elke vraag wordt beoordeeld op een 4 -punts Likert -type schaal.
Items 2 en 3 vragen de patiënt om het aantal dagen dat ze zich goed voelden te markeren en het aantal dagen dat ze niet konden werken (inclusief huishoudelijk werk) vanwege de symptomen van fibromyalgie.
Items 4 tot en met 10 zijn horizontale lineaire schalen gemarkeerd in 10 stappen waarop de patiëntsnelheden werken, pijn, vermoeidheid, ochtendmoeheid, stijfheid, angst en depressie.

De multiple sclerose-impactschaal (MSIS-29) is een zelfrapportage met 29 items bestaande uit 20 items geassocieerd met een fysieke schaal en 9 items geassocieerd met een psychologische schaal.1 Items vragen patiënten (of hun proxy's) over de impact van multiple sclerose (MS) in de afgelopen 2 weken.
Alle items hebben 5 antwoordopties van 1 (helemaal niet) tot 5 (extreem).
Elk van de 2 schalen wordt gescoord door de reacties tussen items op te tellen en vervolgens om te zetten in een schaal van 0 tot 100, waarbij 100 een grotere impact van de ziekte op de dagelijkse functie (slechtere gezondheid) aangeeft.

De multiple sclerosekwaliteit van het leven-54 (MSQOL-54) is een multidimensionale maatregelgerelateerde kwaliteit van leven die zowel generieke als multiple sclerose MS-specifieke items combineert in een enkel instrument.
Dit instrument van 54 items genereert 12 subschalen samen met twee samenvattende scores en twee extra maatregelen met één item.
De subschalen zijn: fysieke functie, rolbeperkingen-fysiek, rolbeperkingen-emotioneel, pijn, emotioneel welzijn, energie, gezondheidspercepties, sociale functie, cognitieve functie, gezondheidsleiders, algehele kwaliteit van leven en seksuele functie.
De samenvattende scores zijn de samengestelde samenstelling van de lichamelijke gezondheid en de samengestelde samenstelling van de geestelijke gezondheid.
De enkele itemmaatregelen zijn tevredenheid met seksuele functie en verandering in gezondheid.
De 54 items zijn verdeeld in 12 schalen met meerdere items en 2 single-item.
De MSQOL-54-items worden lineair getransformeerd naar 0-100 scores en eindscores worden verkregen door het gemiddelde van items binnen de schalen.

De vermoeidheidsschaal (FSS) is een schaal met 9 items die de ernst van vermoeidheid en het effect ervan op de activiteiten en levensstijl van een persoon meet bij patiënten met een verscheidenheid aan aandoeningen.
Antwoorden worden gescoord op een schaal van zeven punten waar 1 = zeer niet mee eens is en 7 = helemaal mee eens.
Dit betekent dat de minimale score negen is en de hoogste is 63.
Hoe hoger de score, hoe ernstiger de vermoeidheid is en hoe meer het de activiteiten van de persoon beïnvloedt.
Het is eenvoudig te begrijpen en duurt gemiddeld acht minuten om te antwoorden
