Utforske helsefordelene forbundet med daglig pulsforbruk hos personer med perifer arteriell sykdom

Dette er et enkelt nettsted, åpen registrering, diettbevis på konsept, matstoffstudie designet for å utforske helsefordelene forbundet med daglig pulsforbruk hos personer med perifer arteriell sykdom.
Etterforskerne antar at en diett som inneholder minst én porsjon pulsvekster (tørkede bønner, erter, linser, kikerter) per dag gir flavonoidforbindelser som forbedrer kardiovaskulær helse ved å øke nivåene av serumadiponectin er basert på bevis fra litteraturen som indikerer flavonoider som finnes i disse matvarene er i stand til å forbedre arteriell stivhet og redusere hyperkolesterolemi.

Mål:
- Korreler serumadiponektinnivåer med daglig inntak av pulser hos personer med perifer arteriell sykdom
- Overvåke endringer i arteriell stivhet og endoteldysfunksjon hos individer med tidlig hjerte- og karsykdom
- Bestem tolerabiliteten av daglig forbruk av pulser i den målrettede befolkningen
- Identifiser endringer i ekspresjonsprofilen til hvite blodceller.
Studietid: 12 måneder
Studere design:
- Dette er et enkelt nettsted, åpen registrering, kostholdsbevis, matstoffstudie designet for å utforske helsefordelene forbundet med daglig pulsforbruk hos individer med arteriell sykdom
- Hvert forsøksperson vil gjennomgå en 7-dagers tilpasningsperiode som består av et inntak på ¼ kopp pulser per porsjon daglig, etterfulgt av minimum ½ kopp pulser per porsjon daglig
- Total varighet av fagdeltakelse er 8 uker;
- Forsøkspersonene vil bli bedt om å delta på 3 personlige klinikkbesøk i løpet av varigheten av deres deltakelse i studien for screening, registrering/baseline og sluttvurderinger
- Telefonoppfølging av fag vil skje i uke 1, 2, 4 og 6
- Både klinisk vurdering og fagbaserte data vil bli samlet inn på ulike punkter i studieplanen
- Forsøkspersonene vil bli bedt om å fylle ut et spørreskjema for matfrekvens i begynnelsen, opprettholde en 3-dagers matjournal ved to separate anledninger, og gjennomgå et kort semistrukturert intervju under telefonoppfølgingsvurderingene av studien
Vurderinger:
- Screeningbesøk: Informert samtykke; vurdering av inkluderings-/eksklusjonskriterier; medisinsk historie; Fysisk eksamen; mat frekvens spørreskjema; 3-dagers matrekord
- Registrering/Baseline: Registrering; vurdere for endringer i sykehistorie (inkludert medisinprofil) og fysisk tilstand; urinprøve; fastende blodprøve; vurdering av arteriell stivhet og ankel-brachial indeks; start tilpasningsperioden (7 dager) med matvarer som inneholder ¼ c belgfrukter per porsjon
- Besøk 1,2,4 & 6: Telefonoppfølging for å vurdere uønskede hendelser og toleranse med semistrukturert emneintervju; distribusjon (ukentlig) av matvarer som inneholder minimum ½ kopp pulser per porsjon; 3-dagers matrekord gjentatt i uke 6
- Besøk 8 (endelig): Vurder for uønskede hendelser og endringer i medisinsk historie og fysisk undersøkelse; urinprøve; innsamling av fastende blodprøver; vurdering av arteriell stivhet og ankel-brachial indeks
Utfall:
- Beskrivende analyse av kliniske data: Demografi, medisinsk historie, fysiske funn, samtidige medisiner og bivirkninger
- Identifikasjon av endringer i endotelfunksjon (bestemt ved å måle pulsbølgehastighet (PWV), ankel-brachial indeks, nivåer av løselige adhesjonsmolekyler og koagulasjonsstatus) som svar på kosttilpasninger
- Korrelasjon av serumadiponektinnivåer, inkludert adiponektinmultimerer og avkortet form (globulært adiponektin) med endotelfunksjon
- Korrelasjon av endringer i endotelcellefunksjon med serumisoflavonnivåer
- Kvalitativ analyse av data samlet inn fra semistrukturerte emneintervjuer for å vurdere parametere assosiert med tolerabilitet av kosthold og for å identifisere foretrukne oppskrifter
- Profilkohort som bruker 55 000 genmikroarray for å identifisere potensielle biomarkører og endringer i genuttrykk (fenotypekartlegging) indusert av diett
- Bruk mikromatriser for å undersøke genmetylering og enkeltnukleotidpolymorfismer (SNP) i DNA-prøver for å bestemme om endringer i ekspresjonsprofil skyldes epigenetisk modifikasjon (global) eller allel (individuell) variasjon i studiekohorten som svar på en pulsanriket diett
- Serum og urinære eikosanoider vil bli analysert ved hjelp av en flertrinnsprosedyre ved bruk av væskekromatografi, derivatiseringstrinn, tynnsjiktskromatografi og gasskromatografi-massespektrometri
- Serum vil også bli analysert for fettsyresammensetning ved bruk av tynnsjiktskromatografi og gasskromatografi

Inklusjonskriterier:
- Tilstedeværelse av perifer arteriell sykdom inkludert de med claudicatio som definert av en ankelbrachial indeks på ≤ 0,90 og asymptomatisk karotisstenoselesjon på > 50 %
- Mann eller kvinne (> 40 år)
- Villig til å overholde protokollkravene
- Villig til å gi informert samtykke
- Stabil medikamentprofil uten forventet endringer i løpet av den foreslåtte studieplanen (8 uker)
Ekskluderingskriterier:
- Nyresvikt som krever dialyse
- Røyker for tiden
- Hormonerstatningsterapi
- Manglende evne til å følge et vanlig kosthold
- Ekstra inntak av pulser utenfor de planlagte daglige kravene skissert i studien
- Anamnese med gastrointestinale reaksjoner eller allergi mot pulser
