Onderzoek naar de gezondheidsvoordelen die samenhangen met de dagelijkse polsconsumptie bij personen met perifere arteriële aandoeningen

Dit is een single-site, open registratie, dieet proof of concept, voedselsubstantie-onderzoek ontworpen om de gezondheidsvoordelen te onderzoeken die samenhangen met dagelijkse polsconsumptie bij personen met perifere arteriële ziekte.
De hypothese van de onderzoekers dat een dieet met ten minste één portie peulvruchten (gedroogde bonen, erwten, linzen, kikkererwten) per dag flavonoïdeverbindingen levert die de cardiovasculaire gezondheid verbeteren door de niveaus van serumadiponectine te verhogen, is gebaseerd op bewijs uit de literatuur dat flavonoïden aangeeft die in deze voedingsmiddelen aanwezig zijn, kunnen de arteriële stijfheid verbeteren en hypercholesterolemie verminderen.

Doelstellingen:
- Correleer de serum-adiponectinespiegels met de dagelijkse inname van pulsen bij personen met perifere arteriële ziekte
- Monitor veranderingen in arteriële stijfheid en endotheliale disfunctie bij personen met hart- en vaatziekten in een vroeg stadium
- Bepaal de verdraagbaarheid van de dagelijkse consumptie van peulvruchten in de beoogde populatie
- Identificeer veranderingen in het expressieprofiel van witte bloedcellen.
Studieduur: 12 maanden
Studie ontwerp:
- Dit is een enkele site, open registratie, proof-of-concept via de voeding, onderzoek naar de substantie van voedingsmiddelen, ontworpen om de gezondheidsvoordelen te onderzoeken die gepaard gaan met dagelijkse pulsconsumptie bij personen met arteriële aandoeningen
- Elke proefpersoon ondergaat een aanpassingsperiode van 7 dagen, bestaande uit een consumptie van ¼ kopje peulvruchten per portie per dag, gevolgd door minimaal ½ kopje peulvruchten per portie per dag
- Totale duur van de proefpersoonparticipatie is 8 weken;
- Proefpersonen zullen worden gevraagd om 3 persoonlijke kliniekbezoeken bij te wonen gedurende hun deelname aan het onderzoek voor screening, registratie/baseline en beoordelingen aan het einde van het onderzoek
- Telefonische follow-up van proefpersonen vindt plaats in week 1, 2, 4 en 6
- Op verschillende punten van het studieschema zullen zowel klinische beoordelingen als op het onderwerp gebaseerde gegevens worden verzameld
- De proefpersonen wordt gevraagd om aan het begin een vragenlijst over de voedselfrequentie in te vullen, bij twee verschillende gelegenheden een driedaags voedingsregister bij te houden en een kort semi-gestructureerd interview te ondergaan tijdens de telefonische follow-upbeoordelingen van het onderzoek
beoordelingen:
- Screeningsbezoek: geïnformeerde toestemming; beoordeling van in- en uitsluitingscriteria; medische geschiedenis; fysiek examen; vragenlijst over voedselfrequentie; 3-daags voedselrecord
- Registratie/basislijn: Registratie; beoordelen op veranderingen in de medische geschiedenis (inclusief medicatieprofiel) en fysieke conditie; urine monster; nuchter bloedmonster; beoordeling van arteriële stijfheid en enkel-armindex; begin de aanpassingsperiode (7 dagen) met voedselproducten die ¼ c peulvruchten per portie bevatten
- Bezoeken 1,2,4 & 6: telefonische follow-up om ongewenste voorvallen en verdraagbaarheid te beoordelen met een semi-gestructureerd interview met de patiënt; distributie (wekelijks) van voedingsmiddelen met een minimum van ½ kopje peulvruchten per portie; 3-daags voedselrecord herhaald in week 6
- Bezoek 8 (definitief): beoordelen op bijwerkingen en veranderingen in de medische geschiedenis en lichamelijk onderzoek; urine monster; nuchtere bloedafname; beoordeling van arteriële stijfheid en en enkel-armindex
Uitkomsten:
- Beschrijvende analyse van klinische gegevens: demografische gegevens, medische geschiedenis, fysieke bevindingen, gelijktijdige medicatie en bijwerkingen
- Identificatie van veranderingen in de endotheliale functie (bepaald door meting van de pulsgolfsnelheid (PWV), enkel-armindex, niveaus van oplosbare adhesiemoleculen en stollingsstatus) als reactie op dieetaanpassing
- Correlatie van serum-adiponectinespiegels, inclusief adiponectine-multimeren en afgeknotte vorm (globulair adiponectine) met endotheliale functie
- Correlatie van veranderingen in endotheelcelfunctie met serumisoflavonspiegels
- Kwalitatieve analyse van gegevens verzameld uit semi-gestructureerde interviews met proefpersonen om parameters te beoordelen die verband houden met de verdraagbaarheid van voeding en om favoriete recepten te identificeren
- Profiel cohort met behulp van 55.000 genen microarray om potentiële biomarkers en veranderingen in genexpressie (fenotype mapping) geïnduceerd door voeding te identificeren
- Gebruik microarrays om genmethylering en single nucleotide polymorphisms (SNP) in DNA-monsters te onderzoeken om te bepalen of veranderingen in expressieprofiel het gevolg zijn van epigenetische modificatie (globaal) of allelische (individuele) variatie in het studiecohort als reactie op een met puls verrijkt dieet
- Eicosanoïden in serum en urine zullen worden geanalyseerd door middel van een meerstapsprocedure waarbij gebruik wordt gemaakt van vloeistofchromatografie, derivatisatiestappen, dunnelaagchromatografie en gaschromatografie-massaspectrometrie
- Serum zal ook worden geanalyseerd op vetzuursamenstelling met behulp van dunnelaagchromatografie en gaschromatografie

Inclusiecriteria:
- Aanwezigheid van perifere arteriële ziekte, waaronder die met claudicatio zoals gedefinieerd door een enkel-armindex van ≤ 0,90 en asymptomatische carotisstenose-laesie van> 50%
- Man of vrouw (> 40 jaar)
- Bereid om te voldoen aan de protocolvereisten
- Bereid om geïnformeerde toestemming te geven
- Stabiel medicatieprofiel zonder verwachte veranderingen gedurende het voorgestelde studieschema (8 weken)
Uitsluitingscriteria:
- Nierfalen waarvoor dialyse nodig is
- Momenteel rokend
- Hormoonvervangingstherapie
- Onvermogen om zich aan een normaal dieet te houden
- Extra inname van peulvruchten buiten de geplande dagelijkse behoeften zoals beschreven in de studie
- Geschiedenis van gastro-intestinale reacties of allergieën voor pulsen
