Szczegółowy opis
Projekt STAR*D obejmie 4000 pacjentów ambulatoryjnych (w wieku od 18 do 75 lat), u których zdiagnozowano niepsychotyczne duże zaburzenie depresyjne. Uczestnicy będą początkowo leczeni (otwarta próba) citalopramem, leczenie Poziomu 1, przez co najmniej 8 tygodni. Pacjenci, u których wystąpią minimalne korzyści, będą zdecydowanie zachęcani do ukończenia 12-tygodniowego leczenia w celu zmaksymalizowania szans na remisję objawów (chyba że po 8 tygodniach nie będzie żadnej poprawy). Wszyscy uczestnicy otrzymają również krótki program edukacyjny dotyczący depresji.
Przy każdej zmianie poziomu uczestnicy zostaną poproszeni o wskazanie nieakceptowalności potencjalnych strategii leczenia (np. zwiększenie lub zmiana leków). Uczestnicy będą następnie kwalifikować się do losowego przydziału do jednej z akceptowalnych i medycznie bezpiecznych opcji leczenia.
Poziom 2: Uczestnicy, którzy albo nie wykazali odpowiedniej odpowiedzi na citalopram, albo nie mogli go tolerować, kwalifikują się do Poziomu 2. Strategie leczenia Poziomu 2 to:
i) Zmiana leków i psychoterapii: zmiana na sertralinę, wenlafaksynę XR, bupropionSR lub terapię poznawczą (CT).
ii) Leki i psychoterapia Wzmocnienie: dodać do citalopramu a) buspiron, b) bupropionSR lub c) CT.
iii) Dla uczestników, dla których tomografia komputerowa jest nieakceptowalna, dostępne są opcje zamiany tylko na leki lub tylko na leki.
iv) Tylko psychoterapia Zamiana lub Tylko psychoterapia Opcje augmentacji są dostępne dla uczestników, dla których dodatkowe leki są niedopuszczalne na tym etapie badania (uczestnicy muszą być chętni do kontynuowania citalopramu)
Poziom 2A: Uczestnicy, którzy nie uzyskali zadowalającej odpowiedzi na leczenie na poziomie 2, kwalifikują się do losowego przydziału do dodatkowego leczenia na poziomie 2A (jeśli jest to medycznie bezpieczne i akceptowalne). Uwzględniono poziom 2A, aby wszyscy uczestnicy przystępujący do poziomu 3 mieli możliwość zareagowania na co najmniej 2 leki. Poziom 2A polega na zmianie leku na jeden z dwóch leków przeciwdepresyjnych (wenlafaksyna XR lub bupropion SR).
Poziom 3: Uczestnicy, którzy nie uzyskali zadowalającej odpowiedzi na Poziom 2 i, w stosownych przypadkach, Poziom 2A, kwalifikują się do losowego przydzielenia do jednego z następujących zabiegów (jeśli jest to dopuszczalne i bezpieczne z medycznego punktu widzenia):
i) Zmień lek na: a) mirtazapinę lub b) nortryptylinę, trójpierścieniowy lek przeciwdepresyjny.
ii) Ulepszenie leku: Dodaj (do obecnego leku poziomu 2 lub poziomu 2A) albo: a) lit albo b) hormon tarczycy (T3).
Poziom 4: Uczestnicy bez odpowiedniej odpowiedzi na poziomie 3 kwalifikują się do losowego przydziału do leczenia na poziomie 4 (jeśli jest to dopuszczalne i bezpieczne z medycznego punktu widzenia). Poziom 4 obejmuje dwie opcje zmiany leku: na tranylcyprominę [inhibitor monoaminooksydazy (MAOI)] lub na mirtazapinę plus wenlafaksynę XR.
Po poziomie 4 uczestnicy bez odpowiedniej odpowiedzi omówią ze swoim lekarzem inne akceptowalne opcje leczenia.
Po uzyskaniu odpowiedniej odpowiedzi na poziomach 2, 2A, 3 lub 4 uczestnicy kwalifikują się do udziału w 12-miesięcznej obserwacji, podczas której pozostaną na obecnym leczeniu i zostaną zapytani o objawy i inne istotne informacje co miesiąc telefonicznie.
