Estudo de resultados do programa de prevenção de diabetes (DPPOS)
O Programa de Prevenção de Diabetes (DPP) foi um estudo multicêntrico que examinou a capacidade de um estilo de vida intensivo ou metformina para prevenir ou retardar o desenvolvimento de diabetes em uma população de alto risco devido à presença de intolerância à glicose (IGT, 2 horas de glicose de 140-199 mg/dl). O DPP terminou precocemente demonstrando que o estilo de vida reduziu o aparecimento de diabetes em 58% e a metformina reduziu o aparecimento de diabetes em 31%.
O DPPOS (2002-2013) foi projetado para aproveitar os participantes do DPP com valor científico e clínico. Este grupo de participantes é quase 50% minoritário e representa a maior população de risco já estudada. Permanecem questões de pesquisa clinicamente importantes que se concentram em 1) durabilidade da intervenção DPP anterior, 2) determinação do curso clínico de diabetes de início precisamente conhecido, em particular em relação à doença microvascular, fatores de risco CVD e aterosclerose, 3) exame minucioso desses tópicos em homens versus mulheres e em populações minoritárias.
Os principais objetivos do DPPOS-3 (2014-2025) aproveitam a exposição aleatória de longo prazo da coorte do estudo à metformina e o envelhecimento da coorte DPPOS. A exposição à metformina e o alto grau de retenção e adesão ao estudo (~85% da coorte DPPOS continua a comparecer às consultas anuais e de meio de ano) permitem que o DPPOS-3 examine os efeitos de longo prazo da metformina em doenças cardiovasculares (DCV) e câncer resultados, resultados de grande interesse clínico e importância.

Prevalência de Complicação Microvascular Agregada

A doença microvascular agregada é definida como a prevalência média de 3 componentes: (1) retinopatia medida por fotografia (ETDRS de 20 ou mais); (2) neuropatia detectada pelo monofilamento de 10 gramas de Semmes Weinstein e (3) nefropatia com base na taxa de filtração glomerular estimada (eGFR pela equação da doença renal crônica (CKD-Epi)) (<45 ml/min, confirmado) e albumina-para- taxa de creatinina na urina spot (> 30mg/gm, confirmado).

Cognitive Function

Cognitive function was defined by tests of memory SEVLT (Spanish English Verbal Learning Test) and executive function, DSST (Digit Symbol Substitution Test).
The measure of memory was the Spanish English Verbal Learning Test (SEVLT).
The SEVLT consists of recalling a list of 15 words in three trials of immediate recall and one trial after a distractor list.
The total number of correct words recalled after four trials is the outcome reported.The test of frontal-executive abilities was the total score in the Digit Symbol Substitution Test (DSST).
The DSST is a test in which participants try to match numbers to symbols in 90 s.
The total number of correct answers is reported.

Short Physical Performance Battery

Physical function is measured using the short physical performance battery (SPPB), which ranges from 0-12 and is comprised of measures of 1) time to walk 3-4 meters, 2) balance, i.e., side-by-side stand, semi-tandem stand, and tandem stand, and 3) repeated chair stands and derived as SPPB= (balance score + gait score + chair score).
Each sub scale is 0, 1, 2, 3 or 4 depending on the gender specific quintiles with 0 being the lowest and 4 being the highest functional status as described in Guralnik JM, Simonsick EM, Ferrucci L, et al.
