皮层下脑梗塞后的语言功能重组:一项纵向功能磁共振成像研究
脑卒中后失语症(PSA)是脑卒中最常见的缺陷之一,影响语言的说、理解、写和读。 一般PSA多见于皮层损伤,但近年来发现皮层下损伤也是PSA的重要病因,称为皮层下失语。 研究者利用fMRI技术,旨在调查皮层下脑梗死患者不同恢复阶段的语言功能,探索大脑损伤后语言重组的机制。
研究者招募了60名单侧皮层下白质病变的首发急性脑梗死患者(左侧40例,右侧20例)和20名健康志愿者。 所有参与者都是右撇子,并使用 MMSE、HAMD 和 HAMA 进行筛查,以排除精神病、中风后痴呆和抑郁症病例。 每个参与者被安排在梗死后的不同阶段进行三个测试阶段(T1:中风发作后 3 天内;T2:发作后 28 ± 3 天;T3:发作后 90 ± 3 天),使用 fMRI 和西方失语电池(WAB) 在每个会话中。

详细说明
受试者来自广州军区广州总医院脑血管病科的急性缺血性脑卒中住院患者。 缺血性脑卒中的诊断采用国际神经病学会和脑卒中协会1982年制定的诊断标准。 缺血性脑卒中的分类标准是基于目前国际上的TOAST病因学分类方法。
本研究通过广州军区总医院伦理委员会批准,所有受试者或其监护人均签署知情同意书。 根据发生部位和诊断标准将参与者分为三组:左半球梗死患者组、右半球梗死患者组和正常健康对照组。 受试者在脑梗死发作后3天、28±3天、90±3天进行了3次语言功能行为测试和功能磁共振(fMRI)测试。 健康对照组仅进行1次上述检查。 语言功能行为评估包括中文版西方失语成套测试(WAB)、自发语言频率测试(SLFT)和图片命名测试(PNT)。 fMRI 检查包括任务状态 fMRI、静息状态 fMRI 和弥散张量成像 (DTI)。
任务状态函数磁共振设计:采用块设计,每个序列采用“基线-刺激-基线-刺激-基线-刺激-基线-刺激-基线-刺激-基线-刺激-基线-刺激-基线”模型。 第一基线持续时间为 24 秒。 下面的基线持续时间和刺激持续时间都是18s,每个序列都是240s,一共三个序列。 扫描 12 分钟。 从Snodgrass图片库中,54张动物图片和54张工具图片可以被所有被试准确识别。 重复 6 个动物组块和 6 个工具命名组块,每个组块六张图片,持续 18 秒。 选择所有美洲臭鼬的抽象图作为动物名称的基线图,选择简单的箭头图片作为工具的基线图。 为了避免练习后的重复效应,MRI设计中使用的图片在言语行为评估中不重复。 在基线时,受试者被要求通过默念“直立”或“倒立”来识别箭头图像的方向和臭鼬的尾巴方向。 被试在测试前接受任务熟悉训练,确保基线任务中没有无意义的图片命名,而是位置判断。 语言任务的设计是基于Damasio的设计。 在磁共振扫描任务状态下,要求患者对视觉图像的无声图像进行朗读和命名。 视觉信息由DMDX软件写入,画面通过脑功能视听刺激系统(SA-9900)投射到屏幕上,将被试放在头线上。 观察圆上的反射体。
静息态fMRI:静息态fMRI扫描过程中,没有给参与者任何任务指令,参与者完全放松,闭上眼睛,平静呼吸,头部保持不动,但无法入睡,尽量避免任何系统的思考活动,扫描 8 分钟。
功能磁共振数据采集:使用HDX3.0Tesla进行头颅扫描 美国GE公司的超导磁共振扫描仪。 8通道相控阵头线圈为接收线圈,扫描顺序及参数如下:
- 使用 FSPGR BRAVO 序列的 T1 结构成像。 参数包括:重复时间,8.86 ms;回声时间,3.52 ms;视野,24×24 cm2;面内分辨率,256×256;切片厚度, 1 mm;interslice gap, 1 mm;和切片数,176。

描述
纳入标准:
- 72 小时内左侧单个皮层下区域的第一次中风。
- 小学及以上学历,18-75岁,母语中文
- 根据常用的视力表检查,矫正视力在1.0以上。
- 根据爱丁堡惯用手习惯问卷 (EHQ) 为右撇子。
- 发病前语言功能正常。 发病后,语言功能轻度至中度受损,西式失语成套测试(WAB)(失语商数(AQ)在60至88之间)。
- 患者配合检查,本人及监护人签署知情同意书
排除标准:
- 神经系统器质性疾病史和颅脑外伤史。
- 癫痫和精神病史。
- 物质依赖史。
- 重要器官功能代偿失调。
- 汉密尔顿抑郁量表(HAMD)>8分。
- 汉密尔顿焦虑量表(HAMA)>7分。
- 简易精神状态检查(MMSE)得分<20分。
- 孕妇和哺乳期妇女。
- MRI 扫描的禁忌症。
