参与者将被要求:
- 与研究调查人员会面以确定是否符合资格
- 签署书面知情同意书

据估计,美国有180万人患有1型糖尿病(T1D),其中至少20%患有慢性肾脏病(CKD)。钠-葡萄糖协同转运蛋白抑制剂(SGLT2i)及类似药物可改善血糖控制,并具有心脏和肾脏保护作用。然而,在SGLT2i作为胰岛素辅助疗法用于T1D患者的试验中,由于糖尿病酮症酸中毒(DKA)发生率增加,其潜在益处未能完全实现。SGLT2i未来在T1D中的批准将取决于对酮症风险的更全面理解以及可行的DKA预防策略。连续酮体监测(CKM)的使用可能使SGLT2i在T1D患者中安全使用,但患者实时使用酮体数据以及患者和医疗提供者使用回顾性CGM/CKM综合报告,都将是CKM数据如何帮助促进SGLT2i治疗安全使用以改善健康结果的关键组成部分。

本研究中使用的药物是索格列净,一种由Lexicon Pharmaceuticals(德克萨斯州伍德兰兹)生产的SGLT 1和2组合抑制剂。该药物已获美国FDA批准用于治疗心力衰竭,包括伴有T2D的患者。索格列净在门诊通过口服给药。欧洲药品管理局也已批准其作为胰岛素辅助疗法,用于改善BMI >27 kg/m²且胰岛素优化治疗后血糖控制不佳的成人T1D患者的血糖控制。该药物在美国以Zynquista品牌销售,目前可通过常规药房渠道处方使用。增加索格列净剂量的决定将由研究受试者和研究者共同商定。
我们计划使用的研究设备是雅培糖尿病(伊利诺伊州芝加哥)生产的连续葡萄糖和酮体监测仪(CGKM)。该设备使用皮下放置的传感器每分钟测量组织间液的葡萄糖和BHB水平,并通过皮肤表面的附接发射器将数据传输到接收器(或智能手机应用程序)。该设备目前正在等待FDA批准;我们将在开始任何研究相关活动前确保设备获得FDA批准。
患者将在设备制造商设定的酮体阈值(根据市售CKM设备确定)处收到实时警报。患者还将接受关于正确识别和管理急性酮血症的培训,包括如何实时使用CKM数据以在酮体水平出现时立即识别和处理。这将基于我们内部制定的“成人糖尿病管理:高血糖和酮症酸中毒”标准医嘱,该医嘱依据ADA高血糖管理指南,并参考STOP协议指导碳水化合物摄入和适当的胰岛素剂量。该标准医嘱将根据葡萄糖和酮体水平提供液体、胰岛素和碳水化合物摄入指导,并考虑症状的存在和严重程度,以建议何时前往急诊科就医。CKM将提供关于酮症的新型趋势箭头,这些箭头也将纳入实时酮体管理指导中(例如,如果趋势箭头向上,则增加胰岛素推注10%)。患者将获得详细说明以及一张纸质钱包卡随身携带。

纳入标准:
- 提供签署并注明日期的知情同意书。
- 表示愿意遵守所有研究程序,并在研究期间能够参与。
- 男性和女性;年龄18-75岁。
- 诊断为1型糖尿病,基于临床诊断且发病时间至少为筛选前3个月。
- 使用自动化胰岛素输送系统(AID)或每日多次注射(MDI)(定义为进餐时使用速效类似物和已批准的长效类似物,例如地特胰岛素或甘精胰岛素)。
- 最近一次估算肾小球滤过率(eGFR)≥30(且在最近12个月内)。
- 糖化血红蛋白(HbA1c)<10%。
- 过去一年内在HealthPartners医疗系统中有过≥1次初级或专科门诊就诊。
- 从未处方过SGLT2抑制剂药物。
- 必须愿意并能够佩戴连续葡萄糖监测(CGM)/连续酮体监测(CKM)设备,并愿意遵循研究方案。
- 必须能够阅读和说英语。

排除标准:
- 妊娠、哺乳、计划怀孕或不愿在试验期间采取避孕措施。
- 除1型糖尿病外的任何其他形式的糖尿病。
- 有任何使用钠-葡萄糖协同转运蛋白抑制剂的历史,或在过去6个月内使用过其他非胰岛素降糖药物。
- 筛选前6个月内长期使用全身性皮质类固醇(>连续4周)或计划在研究期间使用。
- 筛选前3个月内有糖尿病酮症酸中毒史,或过去一年内发生过2次或更多次糖尿病酮症酸中毒。
- 筛选前6个月内有多次(≥3次感染)生殖器真菌感染史。
- 筛选时低血压,定义为收缩压<90且舒张压<60,并伴有低血压症状(意识模糊、头晕、头昏、昏厥、心悸)。
- 筛选前3个月内有3级低血糖事件史(根据美国糖尿病协会标准定义)。
- 筛选前3个月内近期心肌梗死、中风、因不稳定型心绞痛或心力衰竭住院。
- 纽约心脏协会IV级心力衰竭。
- 慢性肾脏病流行病学协作组估算肾小球滤过率(eGFR)<30 mL/min/1.73m²。
- 可能增加参与干预研究风险或影响研究结果的系统或器官损伤(例如:终末期肾病、活动性肝功能障碍、胃轻瘫、贫血、器官移植)。

- 筛选前一年内接受过癌症治疗(不包括通过切除治疗的非黑色素瘤皮肤癌、宫颈或子宫原位癌、乳腺导管原位癌、已切除的非转移性乳腺癌或前列腺癌)。
- 有肾脏移植史。
- 由已知非1型糖尿病原因引起的慢性肾脏病。
- 诊断为进食障碍。
- 体重指数(BMI)<22。

实验性的:索格列净
在这项单臂试验中,所有患者将开始接受剂量为200mg/d的索格列净治疗。
在服用200 mg/d索格列净3个月后,未达到良好血糖控制(TIR >60%)且患有中度或无慢性肾病(eGFR >60)的患者将可选择将索格列净剂量增加至400mg/d。
所有其他参与者将继续每日服用200 mg索格列净。
