这项前瞻性、多中心、随机对照试验的目的是比较术中双重腹腔镜联合新直肠镜(IDLnR)与传统腹腔镜(CL)在肠子宫内膜异位症(BE)患者手术中的临床疗效和安全性。 主要目标是评估两组患者在术后6个月时肠道功能改善的差异。
本研究旨在回答的主要问题包括:
IDLnR组与CL组在术后6个月的肠道功能(主要终点)上是否存在显著差异? 两组在次要结局指标上是否存在差异,包括疗效指标(LARS评分/治愈率/改善率、CRADI-8、Wexner评分、GIQLI、EHP-30、VAS疼痛减轻)和安全性指标(30天内严重并发症、抗生素使用强度、住院时间>7天)? 研究者将BE手术患者随机分配至IDLnR组或CL组,随后比较上述主要和次要终点,以评估IDLnR的临床价值。

描述
纳入标准:
- 年龄18至55岁的绝经前女性。
- 术前盆腔增强磁共振成像和/或经直肠超声内镜检查证实肠道子宫内膜异位症(BE)病灶累及肠壁肌层,位于直肠和/或乙状结肠。术后病理诊断为金标准。
- 治疗前LARS评分>20。
- 适合腹腔镜手术。
- 自愿参加本研究并签署书面知情同意书。
排除标准:
- 妊娠或哺乳期。
- 存在活动性炎症性肠病、肠易激综合征或过去1年内有类似疾病急性发作的其他情况。
- 术前4周内患有急性或严重感染性疾病。
- 有恶性肿瘤病史,或基于术前影像学或肿瘤标志物检查高度怀疑妇科/胃肠道恶性肿瘤。
- 美国麻醉医师协会(ASA)身体状况分级≥Ⅲ级,或存在严重心、肺、肝、肾功能或凝血功能障碍,无法耐受全身麻醉和择期腹腔镜手术。
- 接受机器人辅助腹腔镜手术或开腹手术。
- 目前正在参加其他可能影响本研究结果评估的临床试验。
- 既往参加过其他可能影响本研究结果评估的干预性临床试验。
- 有结直肠切除吻合或造口手术史。
- 有盆腔放疗史。
