描述

入选标准:

  • 18岁≤患者≤90岁;

    • 患者愿意遵守研究方案要求和数据收集程序、了解研究目标并签署知情同意书;② 心脏团队确定患者有冠状动脉血运重建指征,但CABG风险高,或患者拒绝接受CABG,经评估后心脏团队认为PCI可使患者受益;③ 左心室射血分数(LVEF)≤35%,或LVEF≤40%合并严重二尖瓣反流;④ 复杂PCI符合以下条件之一:a) 无保护左主干病变 b) 最后一支通畅冠状动脉PCI c) 三支血管病变(冠状动脉造影提示前降支、回旋支和右冠状动脉同时存在狭窄程度>70%的狭窄) d) 钙化病变需行冠状动脉旋磨术 e) CTO病变需行双侧造影或逆向技术

排除标准:

  • 急性ST段抬高型心肌梗死一周内;

    • 合并心源性休克(心源性休克定义为同时满足以下三个条件:① 动脉收缩压<90mmHg持续≥30分钟,或需升压药/正性肌力药/机械循环支持维持动脉收缩压≥90mmHg;② 有证据表明心输出量减少,如心脏指数<2.2L/min/m2;③ 存在器官灌注不足现象,包括肢端皮肤温度降低、尿量减少<30ml/h、意识障碍或乳酸>2mmol/L);

CHIP-PCI Registry Cohort
Patients with complex high-risk indicated coronary artery disease undergoing percutaneous coronary intervention as part of routine clinical practice. Eligible patients include adults with complex coronary anatomy and reduced left ventricular function who are considered by the heart team to have an indication for coronary revascularization but are at high risk for coronary artery bypass grafting or decline surgical revascularization. PCI strategy, use of intravascular imaging or physiological assessment, revascularization approach, antithrombotic therapy, and use of mechanical circulatory support are determined by the treating physicians according to clinical judgment and local practice. No study-mandated intervention is assigned.
Percutaneous coronary intervention performed as part of routine clinical care in patients with complex high-risk indicated coronary artery disease. The procedural strategy, use of intravascular imaging or physiological assessment, lesion preparation, revascularization extent, staged PCI, antithrombotic therapy, and use of mechanical circulatory support are determined by the treating physicians according to clinical judgment and local practice. No study-mandated intervention is assigned.

Mechanical circulatory support, including intra-aortic balloon pump, extracorporeal membrane oxygenation, Impella-like devices, or other support devices, may be used during high-risk PCI at the discretion of the treating physicians. Use of mechanical circulatory support is not mandated by the study protocol and will be analyzed as an observational exposure.

The proportion of participants who experience a major adverse cardiovascular event within 1 year after PCI. Major adverse cardiovascular events are defined as a composite of all-cause death, myocardial infarction, target vessel revascularization, or hospitalization for heart failure.
