POZADÍ:
Epidemiologické studie v posledních čtyřiceti letech zásadně přispěly k poznání etiologie a přirozené historie chronických onemocnění - kardiovaskulárních onemocnění, cukrovky a rakoviny. Poznatky z epidemiologických studií poskytly praktické základy pro rozvoj preventivních a kontrolních programů ke zlepšení životního stylu a ke kontrole vysokého krevního tlaku léky, pokud je to indikováno. Tento vývoj a jeho dopad na životní styl a rizikové faktory pravděpodobně přispěly k výraznému poklesu úmrtnosti na ischemickou chorobu srdeční, mrtvici, všechna kardiovaskulární onemocnění a všechny příčiny v průběhu 70. a 80. let 20. století.
Čtyři epidemiologické studie mají za svůj základ hlavní přínos dřívějších epidemiologických studií „první generace“, včetně studie Albany Civil Servant Study, Charlestonské studie, Framinghamské studie srdce, Tecumseh Study a dalších. Význam těchto studií spočívá v jejich potenciálu objasnit klíčové problémy „druhé generace“ v epidemiologii hlavních kardiovaskulárních a nekardiovaskulárních chronických onemocnění.
DESIGNOVÝ PŘÍBĚH:
Projekt detekce Chicago Heart Association v průmyslu: Zaměstnanci 85 firem v širší oblasti Chicaga byli prověřováni od podzimu 1967 do začátku roku 1973. Celková pracovní síla mezi těmito společnostmi byla více než 75 000 osob. Podíl dobrovolníků na screeningu byl 55 procent. Všichni zaměstnanci byli povzbuzováni k účasti bez ohledu na typ práce nebo odpracované směny. Na každém pracovišti byly shromážděny údaje o kuřáckých návycích, předchozí lékařské diagnóze, současné léčbě hypertenze, srdečních chorob a cukrovky, výšce a hmotnosti, věku, pohlaví, rase, srdeční frekvenci.
Peoples Gas Company: Všechny údaje o dvou kohortách byly shromážděny v rámci programu pravidelných lékařských prohlídek pro všechny zaměstnance lékařského oddělení společnosti jejími lékaři na plný úvazek. Byly shromážděny údaje o věku, rase, výšce, váze, krevním tlaku, sérovém cholesterolu, kouření, srdeční frekvenci, klidovém EKG, lékařské a rodinné anamnéze, konzumaci alkoholu a kávy a čaje, vzdělání, vojenské službě, povolání, fyzické aktivitě, stravě a hladiny glukózy a kyseliny močové v séru. Opakované zkoušky byly naplánovány ročně pro muže 50-59, každé dva roky pro muže 40-49 a každé čtyři roky pro muže 25-39.
Western Electric Company: Tato longitudinální studie ischemické choroby srdeční byla zahájena v roce 1957 v Hawthorne Works společnosti Western Electric Company v Chicagu. Muži byli bez koronární srdeční choroby na začátku. Lékařská anamnéza a fyzikální vyšetření byly modelovány podle Framinghamské srdeční studie. Byly shromážděny údaje o stravě, fyzické aktivitě, elektrokardiogramu, hladinách hemoglobinu a cholesterolu v krvi, tělesném tuku, krevním tlaku, výšce, hmotnosti, lipoproteinové lipáze, hladině glukózy a kouření.
Studie byla obnovena ve fiskálním roce 1992 a znovu ve fiskálním roce 1998. Při obnově v roce 1998 byly použity údaje a analýzy ze čtyř kohort. Zaměřila se na šest specifických cílů: faktory ovlivňující dlouhodobé riziko koronárních onemocnění srdce a kardiovaskulárních onemocnění u mladých dospělých žen a mužů (ve věku 18 až 39 let na začátku); faktory ovlivňující ICHS-kardiovaskulární onemocnění dlouhodobé riziko u afroamerických žen a mužů; faktory ovlivňující ICHS-riziko kardiovaskulárních onemocnění během dřívějšího i pozdějšího sledování (prvních 15 let a déle než 15 let sledování); dopad výchozího stavu s nízkým rizikem na dlouhodobé riziko úmrtnosti na ischemickou chorobu srdeční, kardiovaskulární onemocnění, nekardiovaskulární onemocnění a všechny příčiny; vztah obvyklého příjmu více skupin potravin k 10leté změně sérového cholesterolu, krevního tlaku, hmotnosti a ke 30letému riziku úmrtí na ischemickou chorobu srdeční, kardiovaskulární onemocnění, rakovinu a všechny příčiny; rozdíly v dopadu rizikových faktorů mezi skupinami na základě pohlaví, rasy, věku nebo stavu rizikového faktoru.
Studie byla prodloužena do července 2005 a využívá data a analýzy z kohorty Chicago Heart Association Detection Project. Studie se zaměřuje na tři specifické cíle: faktory ovlivňující dlouhodobé riziko koronárních onemocnění srdce a kardiovaskulárních onemocnění u mladých dospělých žen (ve věku 18–39 let na začátku studie); faktory ovlivňující ICHS a kardiovaskulární onemocnění dlouhodobé riziko u mladých dospělých a afroamerických mužů a žen středního věku; a dopad výchozího stavu „nízkého rizika“ na dlouhodobou úmrtnost na ischemickou chorobu srdeční, kardiovaskulární onemocnění, nekardiovaskulární onemocnění a všechny příčiny u mladých dospělých žen (ve věku 18–39 let), včetně afroamerických a bílých žen.
