FUNDO:
Nos últimos quarenta anos, estudos epidemiológicos deram contribuições básicas para o conhecimento da etiologia e história natural das doenças crônicas - doenças cardiovasculares, diabetes e cânceres. Os achados dos estudos epidemiológicos fornecem fundamentos práticos para o desenvolvimento de programas de prevenção e controle para melhorar os estilos de vida e controlar a pressão arterial elevada por medicamentos, quando indicado. Esses desenvolvimentos e seu impacto nos estilos de vida e fatores de risco provavelmente contribuíram para o declínio acentuado nas taxas de mortalidade por doença coronariana, acidente vascular cerebral, todas as doenças cardiovasculares e todas as causas ao longo dos anos 1970 e 1980.
Os quatro estudos epidemiológicos têm como base as principais contribuições dos estudos epidemiológicos anteriores de 'primeira geração', incluindo o Albany Civil Servant Study, o Charleston Study, o Framingham Heart Study, o Tecumseh Study e outros. A importância desses estudos reside em seu potencial para elucidar questões-chave de 'segunda geração' na epidemiologia das principais doenças crônicas cardiovasculares e não cardiovasculares.
NARRATIVA DO DESENHO:
Projeto de Detecção da Associação Cardíaca de Chicago na Indústria: Funcionários de 85 empresas na área metropolitana de Chicago foram rastreados entre o outono de 1967 e o início de 1973. A força de trabalho total entre essas empresas era de mais de 75.000 pessoas. A taxa de voluntários para a triagem foi de 55 por cento. Todos os funcionários foram incentivados a participar, independentemente do tipo de trabalho ou turno trabalhado. Em cada local de trabalho foram coletados dados sobre tabagismo, diagnóstico médico anterior, tratamentos atuais para hipertensão, doenças cardíacas e diabetes, altura e peso, idade, sexo, raça, frequência cardíaca.
Peoples Gas Company: Todos os dados sobre as duas coortes foram coletados como parte do programa de exames médicos periódicos para todos os funcionários do Departamento Médico da Empresa por sua equipe de médicos em tempo integral. Foram coletados dados sobre idade, raça, altura, peso, pressão arterial, colesterol sérico, histórico de tabagismo, frequência cardíaca, ECG em repouso, histórico médico e familiar, consumo de álcool e café e chá, educação, serviço militar, ocupação, atividade física, dieta e níveis séricos de glicose e ácido úrico. Exames repetidos foram agendados anualmente para homens de 50 a 59 anos, a cada dois anos para homens de 40 a 49 anos e a cada quatro anos para homens de 25 a 39 anos.
Western Electric Company: Este estudo longitudinal da doença cardíaca coronária foi iniciado em 1957 na Hawthorne Works da Western Electric Company em Chicago. Os indivíduos do sexo masculino estavam livres de doença cardíaca coronária no início do estudo. A história médica e o exame físico foram modelados após o Framingham Heart Study. Foram coletados dados sobre dieta, atividade física, eletrocardiograma, níveis de hemoglobina e colesterol no sangue, gordura corporal, pressão arterial, altura, peso, lipoproteína lipase, nível de glicose e tabagismo.
O estudo foi renovado no ano fiscal de 1992 e novamente no ano fiscal de 1998. A renovação de 1998 usou dados e análises das quatro coortes. Ele se concentrou em seis objetivos específicos: fatores que influenciam o risco de doença cardíaca coronária-doença cardiovascular a longo prazo em mulheres e homens adultos jovens (idades de 18 a 39 no início do estudo); fatores que influenciam o risco de doença cardíaca coronariana a longo prazo em mulheres e homens afro-americanos; fatores que influenciam o risco de doença coronariana-cardiovascular durante o acompanhamento inicial e posterior (primeiros 15 anos e além de 15 anos de acompanhamento); impacto do status de baixo risco basal no risco de longo prazo de mortalidade por doença coronariana, doença cardiovascular, doença não cardiovascular e todas as causas; relação da ingestão habitual de vários grupos de alimentos com a mudança de 10 anos no colesterol sérico, pressão arterial, peso e riscos de morte em 30 anos por doença coronariana, doença cardiovascular, câncer e todas as causas; as diferenças no impacto dos fatores de risco entre os grupos com base no sexo, raça, idade ou status do fator de risco.
O estudo foi estendido até julho de 2005 e usa dados e análises do Chicago Heart Association Detection Project in Industry coorte. O estudo concentra-se em três objetivos específicos: fatores que influenciam a doença cardíaca coronária e o risco de doença cardiovascular a longo prazo em mulheres adultas jovens (idades 18-39 no início do estudo); fatores que influenciam a doença cardíaca coronária e o risco de doença cardiovascular a longo prazo em homens e mulheres afro-americanos adultos jovens e de meia-idade; e o impacto do status inicial de "baixo risco" na mortalidade a longo prazo por doença coronariana, doença cardiovascular, doença não cardiovascular e todas as causas em mulheres adultas jovens (de 18 a 39 anos), incluindo mulheres afro-americanas e brancas.
