Detailní popis
Studie je pilotní randomizovanou kontrolovanou studií, jejímž cílem je vyhodnotit proveditelnost a účinnost týdenní MIT při závislosti na CBT-F ve skupině dospělých s diagnózou ED. Konkrétně budou vyšetřovatelé zkoumat na vzorku dospělých bez podváhy s ED, zda ve srovnání s CBT-E je léčba kombinující MIT a CBT-F a) proveditelná, b) dobře tolerovaná a c) potenciálně účinná při přijímání potravy. symptomy poruch a další výsledky.
Vyšetřovatelé očekávají, že experimentální skupina CBT-F+MIT bude schopna dosáhnout vysoké adherence a udržení léčby na základě předchozích studií MIT pro poruchy osobnosti. Budou také očekávat dobré výsledky v primárním výsledku. Vzhledem k malému vzorku se jedná pouze o pilotní studii, takže jakékoli závěry o dalších výsledcích budou považovány za předběžné. Výsledky poskytnou nové důkazy, které mohou vést ke zvážení CBT-F+MIT jako léčebné možnosti pro ED, která si zaslouží vyšetření ve větších studiích.

- ED diagnostikovaná v posledních 6 měsících
- Hledání léčby poruchy příjmu potravy
- Schopnost poskytnout písemný informovaný souhlas
- BMI > 18,5
Kritéria vyloučení:
- Akutní psychotická epizoda, psychotické symptomy, bipolární porucha I; antisociální porucha osobnosti
- Sebevražedné myšlenky
- Zneužívání návykových látek
- Předchozí psychologická intervence u jiných poruch příjmu potravy
- V současné době zapojen do další probíhající léčby

Aktivní komparátor: CBT-E
Účastníci ve stavu CBT-E absolvují celkem 20 sezení CBT-E během 20 týdnů.
Léčba CBT-E se bude skládat ze čtyř fází.
Cílem CBT je zejména informovat pacienty o důležitosti sebekontroly, nebezpečí některých restriktivních chování, jako je samovolně vyvolané zvracení.
Kromě toho KBT poskytuje pacientům strategie ke sledování jejich obvykle dysfunkčního chování a tím ke zvýšení jejich povědomí (tj.
metoda potravinového deníku) a zároveň snižuje dostupnost jídla a podporuje činnosti, které jsou neslučitelné s přejídáním.
Pacienti budou trénováni v řešení problémů, aby tyto pocity změnili, a také ve zvýšení jejich sebeuvědomění, aby rozpoznali iracionální myšlenky o své tělesné hmotnosti a tvaru.
Navíc budou postupně vystaveni potravinám, kterým se vyhýbali.

V první fázi bude léčba zaměřena na dosažení společného porozumění pacientově poruše příjmu potravy a souvisejícím udržovacím faktorům.
V této fázi se pacientovi pomůže regulovat a stabilizovat jeho stravovací návyky, a tak řešit jeho problémy s nadváhou.
Ve druhé fázi je podrobně přezkoumán dosažený pokrok.
Ve třetí etapě budou sezení zaměřena na centrální procesy, které udržují psychopatologii pacientovy poruchy příjmu potravy.
Zejména se jedná o řešení obav o váhu a tvar těla, kognitivní a kalorické dietní omezení, události a emoce, které ovlivňují výživu.
Ve třetí a čtvrté fázi klinické dokonalosti se také řeší nízké globální sebevědomí, nesnášenlivost emocí a mezilidské potíže.
Ke konci třetí etapy a během čtvrté etapy budou rovněž implementovány postupy k minimalizaci rizika krátkodobého a dlouhodobého relapsu.

Experimentální: CBT-F+MIT
Účastníci ve stavu CBT-F+MIT absolvují celkem 20 sezení během 20 týdnů. Konkrétně 2 sezení budou založeny pouze na CBT-F jako obvykle. Během těchto sezení účastníci absolvují psychoedukační školení o stravovacím chování a seznámení s nástroji protokolu, jmenovitě s monitorovacím formulářem, hmotnostním grafem, transdiagnostickou formulací a kontrolním seznamem problémů s jídlem (EPCL). Tyto prvky budou použity na začátku zbývajících 18 sezení, aby bylo možné monitorovat pravidla stravovacího chování a také vyvolávat narativní epizody. Tyto materiály budou tvořit základ pro MIT-část sezení, ve kterém se terapeuti budou snažit vytvořit s pacientem společné porozumění psychologickým důvodům, které jsou základem jeho symptomů ED a jejich maladaptivního interpersonálního fungování. Sezení MIT budou integrována do protokolu CBT-F, který bude poskytovat psychoedukační, nutriční reedukaci a management pro ED. |
Cílem je vyvinout zdravější strategie pro zvládání negativních myšlenek a pocitů předcházejících ED a pro zapojení pacientů do sociálních interakcí, které splňují jejich základní vztahová přání.
Pochopí tak, že perfekcionismus a potřeba kontroly, kterou zažívají prostřednictvím ED, jsou strategie zvládání vyvinuté v rámci interpersonálních vzorců interakce s významnými druhými, kde hraje ústřední roli nízké sebevědomí a emotivní dysregulace.
MIT si klade za cíl zlepšit schopnost jednotlivců porozumět vlastnímu vlivu a kognitivním schopnostem a uvědomit si, že jsou poháněni maladaptivními, rigidními a zaujatými schématy o sobě a druhých, vytvořit bohatší porozumění mysli druhých a využít tyto znalosti k reagovat adaptivnějšími způsoby na sociální potíže nebo evolučně vybraná přání.
MIT také umožňuje jednotlivcům přemýšlet o tom, jak mohou tato schémata působit jako spouštěče chování ED, a vyvinout účinnější strategie zvládání interpersonálních stresorů.
