Szczegółowy opis
Badanie jest pilotażowym, randomizowanym, kontrolowanym badaniem, którego celem jest ocena wykonalności i skuteczności cotygodniowego MIT w uzależnieniu od CBT-F w grupie dorosłych, u których zdiagnozowano zaburzenia erekcji. W szczególności badacze zbadają na próbie osób dorosłych bez niedowagi z zaburzeniami erekcji, czy w porównaniu z CBT-E leczenie łączące MIT i CBT-F jest a) wykonalne, b) dobrze tolerowane i c) potencjalnie skuteczne w jedzeniu objawy zaburzeń i inne skutki.
Badacze spodziewają się, że grupa eksperymentalna CBT-F+MIT będzie w stanie osiągnąć wysokie przestrzeganie i retencję leczenia na podstawie wcześniejszych badań MIT dotyczących zaburzeń osobowości. Będą również oczekiwać dobrych wyników w wyniku pierwotnym. Biorąc pod uwagę małą próbę, jest to tylko badanie pilotażowe, więc wszelkie wnioski dotyczące innych wyników zostaną uznane za wstępne. Wyniki dostarczą nowych dowodów, które mogą prowadzić do rozważenia CBT-F+MIT jako opcji leczenia zaburzeń erekcji, które zasługują na badania w większych badaniach.

Kryteria przyjęcia:
- ED zdiagnozowane w ciągu ostatnich 6 miesięcy
- Poszukuję leczenia zaburzeń odżywiania
- Zdolność do wyrażenia pisemnej, świadomej zgody
- BMI > 18,5
Kryteria wyłączenia:
- Ostry epizod psychotyczny, objawy psychotyczne, zaburzenie afektywne dwubiegunowe typu I; aspołeczne zaburzenie osobowości
- Myśli samobójcze
- Nadużywanie substancji
- Wcześniejsza interwencja psychologiczna w przypadku innych zaburzeń odżywiania
- Obecnie zaangażowany w inne trwające leczenie

Aktywny komparator: CBT-E
Uczestnicy w stanie CBT-E otrzymają łącznie 20 sesji CBT-E w ciągu 20 tygodni.
Terapia CBT-E będzie składać się z czterech etapów.
W szczególności CBT ma na celu poinformowanie pacjentów o znaczeniu samokontroli, niebezpieczeństwach związanych z niektórymi zachowaniami ograniczającymi, takimi jak prowokowanie wymiotów.
Co więcej, CBT zapewnia pacjentom strategie monitorowania ich zwykle dysfunkcyjnych zachowań, a tym samym zwiększania ich świadomości (tj.
metoda dzienniczka żywieniowego), jednocześnie zmniejszając dostępność pożywienia i zachęca do działań nie dających się pogodzić z przejadaniem się.
Pacjenci zostaną przeszkoleni w rozwiązywaniu problemów w celu zmiany tych odczuć, a także w zwiększeniu samoświadomości w celu rozpoznawania irracjonalnych myśli na temat masy ciała i sylwetki.
Dodatkowo będą stopniowo narażeni na pokarmy, których wcześniej unikali.

W pierwszym etapie leczenie będzie koncentrować się na osiągnięciu wspólnego zrozumienia zaburzeń odżywiania się pacjenta i związanych z nimi czynników podtrzymujących.
W tej fazie pacjentowi pomoże się uregulować i ustabilizować jego nawyki żywieniowe, a tym samym rozwiązać problemy związane z wagą.
W drugim etapie dokonuje się szczegółowego przeglądu poczynionych postępów.
W trzecim etapie sesje skupią się na centralnych procesach podtrzymujących psychopatologię zaburzeń odżywiania się pacjenta.
W szczególności obejmuje to rozwiązywanie problemów związanych z wagą i kształtem ciała, poznawczymi i kalorycznymi ograniczeniami dietetycznymi, wydarzeniami i emocjami, które wpływają na odżywianie.
W fazie trzeciej i czwartej zajmuje się również doskonałością kliniczną, niską samooceną globalną, nietolerancją emocji i trudnościami interpersonalnymi.
Pod koniec trzeciego etapu iw trakcie czwartego etapu zostaną również wdrożone procedury minimalizujące ryzyko krótko- i długoterminowego nawrotu choroby.

Eksperymentalny: CBT-F+MIT
Uczestnicy w stanie CBT-F+MIT otrzymają łącznie 20 sesji w ciągu 20 tygodni. Konkretnie, 2 sesje będą jak zwykle oparte wyłącznie na CBT-F. Podczas tych sesji uczestnicy przejdą szkolenie psychoedukacyjne dotyczące zachowań żywieniowych oraz zapoznają się z narzędziami protokołu, tj. formularzem monitorowania, wykresem wagi, sformułowaniem transdiagnostycznym oraz listą kontrolną problemów żywieniowych (EPCL). Elementy te zostaną wykorzystane na początku pozostałych 18 sesji, w celu monitorowania regulacji zachowań żywieniowych oraz wywoływania epizodów narracyjnych. Materiały te będą stanowić podstawę dla części sesji MIT, podczas której terapeuci będą starali się wypracować z pacjentem wspólne zrozumienie psychologicznych przyczyn leżących u podstaw objawów zaburzeń erekcji i nieprzystosowawczego funkcjonowania interpersonalnego. Sesje MIT zostaną zintegrowane z protokołem CBT-F, który zapewni psychoedukację, reedukację żywieniową i zarządzanie zaburzeniami erekcji. |
Celem jest opracowanie zdrowszych strategii radzenia sobie z negatywnymi myślami i uczuciami poprzedzającymi ED oraz angażowanie pacjentów w interakcje społeczne, które spełniają ich podstawowe życzenia relacyjne.
W ten sposób zrozumieją, że perfekcjonizm i potrzeba kontroli, których doświadczają poprzez zaburzenia erekcji, to strategie radzenia sobie opracowane w ramach interpersonalnych wzorców interakcji z innymi znaczącymi osobami, w których główną rolę odgrywają niska samoocena i rozregulowanie emocjonalne.
MIT ma na celu poprawę zdolności jednostek do zrozumienia własnych afektów i procesów poznawczych oraz uświadomienia sobie, że kierują nimi nieprzystosowawcze, sztywne i tendencyjne schematy dotyczące siebie i innych, aby uzyskać bogatsze zrozumienie umysłu innych i wykorzystać tę wiedzę do reagują w bardziej adaptacyjny sposób na trudności społeczne lub ewolucyjnie wybrane życzenia.
MIT umożliwia również jednostkom zastanowienie się, w jaki sposób te schematy mogą działać jako wyzwalacze zachowań ED i opracowanie skuteczniejszych strategii radzenia sobie w obliczu stresorów interpersonalnych.
