Yksityiskohtainen kuvaus
Tutkimus on pilotti satunnaistettu kontrolloitu koe, jonka tavoitteena on arvioida viikoittaisen MIT:n toteutettavuutta ja tehokkuutta CBT-F-riippuvuudessa ryhmässä aikuisia, joilla on diagnosoitu ED. Erityisesti tutkijat tutkivat otoksesta ei-alipainoisia aikuisia, joilla on ED:tä, onko CBT-E:hen verrattuna MIT:n ja CBT-F:n yhdistävä hoito a) mahdollinen, b) hyvin siedetty ja c) mahdollisesti tehokas syömisessä. häiriöiden oireet ja muut seuraukset.
Tutkijat odottavat, että koeryhmä CBT-F+MIT kykenisi saavuttamaan korkean hoitoon sitoutumisen ja retention aiempien persoonallisuushäiriöiden MIT-tutkimusten perusteella. He odottavat myös hyviä tuloksia ensisijaisessa tuloksessa. Pienen otoksen vuoksi tämä on vain pilottitutkimus, joten muita tuloksia koskevia päätelmiä pidetään alustavina. Tulokset tarjoavat uusia todisteita, jotka voivat johtaa siihen, että CBT-F+MIT:tä voidaan pitää hoitovaihtoehtona ED-tutkimuksille suuremmissa tutkimuksissa.

- ED diagnosoitu viimeisen 6 kuukauden aikana
- Syömishäiriön hoitoon hakeutuminen
- Pystyy antamaan kirjallinen, tietoon perustuva suostumus
- BMI > 18,5
Poissulkemiskriteerit:
- Akuutti psykoottinen jakso, psykoottiset oireet, kaksisuuntainen mielialahäiriö I; antisosiaalinen persoonallisuushäiriö
- Itsemurha-ajatukset
- Päihteiden väärinkäyttö
- Aiempi psykologinen interventio muiden syömishäiriöiden vuoksi
- Tällä hetkellä mukana muussa meneillään olevassa hoidossa

Active Comparator: CBT-E
CBT-E-tilan osallistujat saavat yhteensä 20 CBT-E-istuntoa 20 viikon aikana.
CBT-E-hoito koostuu neljästä vaiheesta.
Erityisesti CBT pyrkii tiedottamaan potilaille itsehillinnän tärkeydestä ja joidenkin rajoittavien käytösten, kuten itsensä aiheuttaman oksentelun, vaaroista.
Lisäksi CBT tarjoaa potilaille strategioita, joiden avulla he voivat seurata heidän tavallisesti huonosti toimivaa käyttäytymistään ja lisätä siten heidän tietoisuuttaan (esim.
ruokapäiväkirjamenetelmä) vähentäen samalla ruoan saatavuutta ja rohkaisemaan toimintaan, joka ei sovi ylensyömiseen.
Potilaita koulutetaan ongelmanratkaisuun näiden tunteiden muuttamiseksi sekä itsetietoisuuden lisäämiseen, jotta voidaan tunnistaa irrationaalisia ajatuksia ruumiinpainostaan ja -muodostaan.
Lisäksi he altistuvat vähitellen elintarvikkeille, joita he olivat välttäneet.

Ensimmäisessä vaiheessa hoidossa keskitytään yhteisymmärrykseen potilaan syömishäiriöstä ja siihen liittyvistä ylläpitotekijöistä.
Tässä vaiheessa potilasta autetaan säätelemään ja vakauttamaan ruokailutottumuksiaan ja siten käsittelemään painoongelmiaan.
Toisessa vaiheessa edistymistä tarkastellaan yksityiskohtaisesti.
Kolmannessa vaiheessa istunnot keskittyvät keskeisiin prosesseihin, jotka ylläpitävät potilaan syömishäiriön psykopatologiaa.
Tämä koskee erityisesti painoa ja kehon muotoa, kognitiivisia ja kaloripitoisia ruokavaliorajoituksia sekä ravitsemukseen vaikuttavia tapahtumia ja tunteita koskevia huolenaiheita.
Kolmannen ja neljännen kliinisen täydellisyyden vaiheissa käsitellään myös alhaista globaalia itsetuntoa, tunteiden suvaitsemattomuutta ja ihmisten välisiä vaikeuksia.
Kolmannen vaiheen loppupuolella ja neljännen vaiheen aikana toteutetaan myös toimenpiteitä lyhyen ja pitkän aikavälin uusiutumisen riskin minimoimiseksi.

Kokeellinen: CBT-F+MIT
CBT-F+MIT-tilan osallistujat saavat yhteensä 20 istuntoa 20 viikon aikana. Tarkemmin sanottuna 2 istuntoa perustuu tavalliseen tapaan vain CBT-F:ään. Näiden istuntojen aikana osallistujat saavat psykoedukoivaa koulutusta syömiskäyttäytymisestä ja johdannon protokollatyökaluihin, nimittäin seurantalomakkeeseen, painokaavioon, transdiagnostiseen formulaatioon ja syömisongelmien tarkistuslistaan (EPCL). Näitä elementtejä käytetään jäljellä olevien 18 istunnon alussa syömiskäyttäytymisen säännösten seuraamiseksi sekä kerrontajaksojen herättämiseksi. Nämä materiaalit muodostavat perustan istunnon MIT-osalle, jossa terapeutit pyrkivät muodostamaan potilaan kanssa yhteisymmärryksen ED-oireiden taustalla olevista psykologisista syistä ja heidän sopeutumattomasta ihmissuhteistaan. MIT-istunnot integroidaan CBT-F-protokollaan, joka tarjoaa psykokasvatusta, ravitsemuksellista uudelleenkoulutusta ja ED:n hallintaa. |
Tavoitteena on kehittää terveellisempiä strategioita ED:tä edeltäneiden negatiivisten ajatusten ja tunteiden hallintaan ja potilaiden saamiseen mukaan sosiaaliseen vuorovaikutukseen, joka täyttää heidän suhteellisia perustoiveitaan.
Siten he ymmärtävät, että perfektionismi ja hallinnan tarve, joita he kokevat ED:n kautta, ovat selviytymisstrategioita, jotka on kehitetty ihmisten välisissä vuorovaikutusmalleissa merkittävien muiden kanssa, joissa alhainen itsetunto ja tunteiden häiriöt ovat keskeisessä asemassa.
MIT:n tavoitteena on parantaa yksilöiden kykyä ymmärtää omia vaikutteitaan ja kognitioitaan ja tulla tietoisiksi siitä, että heitä ohjaavat huonosti mukautuvat, jäykät ja puolueelliset skeemat itsestä ja muista, muodostaa rikkaampi ymmärrys toisten mielestä ja käyttää tätä tietoa reagoida sopeutuvammin sosiaalisiin vaikeuksiin tai evolutionaarisesti valittuihin toiveisiin.
MIT antaa yksilöille myös mahdollisuuden pohtia, kuinka nämä skeemat voivat toimia ED-käyttäytymisen laukaisimina, ja kehittää tehokkaampia selviytymisstrategioita ihmisten välisten stressitekijöiden edessä.
