NICHD Neonatal Research Networks opfølgningsstudie er et multicenter kohortestudie, hvor overlevende spædbørn med ekstrem lav fødselsvægt gennemgår neuroudviklingsmæssige, neurosensoriske og funktionelle vurderinger i 22-26 måneders korrigeret alder (spædbørn født før 1. juli 2012 blev set ved 18-22 måneders korrigeret alder). Målet med undersøgelsen er at identificere potentielle maternelle og neonatale risikofaktorer, der kan påvirke spædbarns neuroudvikling, herunder:
- Evaluering af udvikling af motoriske færdigheder, kognitive færdigheder, sprog og adfærd
- Bestemmelse af dødelighed og forekomsten af specifikke medicinske tilstande
- Vurdering af forholdet mellem vækst og neuroudviklingsresultat
- Vurdering af sammenhængen mellem den socioøkonomiske status og udviklingsresultater
- Vurdering af denne befolknings brug af særlige støttetjenester og programmer for tidlig indsats
- Evaluering af behovet for opfølgning i skolealderen.
De planlagte evalueringer indsamler: demografiske oplysninger; socioøkonomisk status; medicinsk historie; medicin; påkrævet medicinsk udstyr; vækstdata; en detaljeret neurologisk undersøgelse; Bayley Scales of Infant Development (mental, motorisk, spædbarnsadfærd); Tjekliste for børns adfærd.
Et delstudie vil vurdere en referencegruppe bestående af et begrænset antal raske terminsbørn født i netværkscentre for at opfylde følgende tre mål: 1) at undgå potentielle konstateringsforstyrrelser på grund af eksaminatorforventninger, når det kun er ekstremt for tidligt fødte eller andre højrisikobørn. vurderes 2) i mangel af veludviklede normer for Bayley-skalaen for at definere tærskler for svækkelse baseret på data for et repræsentativt udsnit af raske børn født til termin i vores centre og samtidigt vurderet af de samme eksaminatorer som for vores høj- risiko spædbørn; og 3) at hjælpe med at identificere og adressere, når "afdrift" opstår over tid ved udførelse og scoring af Bayley-vurderinger.

- Spædbørn født på NRN-centre
- <27 ugers svangerskabsalder
- Spædbørn, der er tilmeldt en eller flere yderligere NICHD NRN-opfølgningsundersøgelser. For spædbørn, der ikke opfylder inklusionskriterierne ovenfor, bestemmes inklusions- og eksklusionskriterierne af kriterierne for det eller de yderligere forsøg. I disse tilfælde kan spædbørn, der er større end 1.000 gram og/eller ældre end 27 uger, indgå i FU-undersøgelsen.
Bemærk: Disse inklusionskriterier blev ændret pr. 1/1/2008. Før denne dato var spædbørn med fødselsvægte mellem 401 og 1500 gram, som blev indlagt på NRN NICU'er inden for 14 dage efter fødslen, inkluderet i databasen.
