Szczegółowy opis
Badanie follow-up NICHD Neonatal Research Network to wieloośrodkowe badanie kohortowe, w którym przeżywające niemowlęta z bardzo niską masą urodzeniową są poddawane ocenie neurorozwojowej, neurosensorycznej i funkcjonalnej w wieku 22-26 miesięcy (niemowlęta urodzone przed 1 lipca 2012 r. w wieku korygowanym 18-22 miesięcy). Celem badania jest identyfikacja potencjalnych czynników ryzyka dla matki i noworodka, które mogą wpływać na rozwój neurologiczny niemowlęcia, w tym:
- Ocena rozwoju zdolności motorycznych, zdolności poznawczych, języka i zachowania
- Określanie śmiertelności i częstości występowania określonych schorzeń
- Ocena związku między wzrostem a wynikami neurorozwojowymi
- Ocena zależności między statusem społeczno-ekonomicznym a wynikami rozwojowymi
- Ocena korzystania przez tę populację ze specjalnych usług wsparcia i programów wczesnej interwencji
- Ocena potrzeby obserwacji w wieku szkolnym.
Zaplanowane oceny gromadzą: informacje demograficzne; status społeczno ekonomiczny; Historia medyczna; leki; wymagany sprzęt medyczny; dane dotyczące wzrostu; szczegółowe badanie neurologiczne; Bayley Scales of Infant Development (umysłowe, motoryczne, zachowania niemowląt); Lista kontrolna zachowania dziecka.
W badaniu cząstkowym oceniona zostanie grupa referencyjna złożona z ograniczonej liczby zdrowych noworodków urodzonych w terminie w ośrodkach Network, aby spełnić następujące trzy cele: 1) uniknięcie potencjalnych błędów w ustaleniach wynikających z oczekiwań badacza, gdy tylko skrajne wcześniaki lub inne niemowlęta wysokiego ryzyka są oceniane 2) w przypadku braku dobrze opracowanych norm dla skali Bayleya, określenie progów upośledzenia na podstawie danych dla reprezentatywnej próby zdrowych dzieci urodzonych o czasie w naszych ośrodkach i jednocześnie ocenianych przez tych samych egzaminatorów, co w naszych ośrodkach niemowlęta ryzyka; oraz 3) pomoc w identyfikowaniu i rozwiązywaniu przypadków „dryfowania” pojawiających się w czasie w przeprowadzaniu i ocenianiu ocen Bayley.

Opis
- Niemowlęta urodzone w ośrodkach KSOW
- <27 tydzień ciąży
- Niemowlęta włączone do jednego lub więcej dodatkowych badań uzupełniających KSOW NICHD. W przypadku niemowląt, które nie spełniają powyższych kryteriów włączenia, kryteria włączenia i wyłączenia są określane na podstawie kryteriów dodatkowych badań. W takich przypadkach niemowlęta ważące ponad 1000 gramów i/lub starsze niż 27 tygodni mogą zostać włączone do badania FU.
Uwaga: te kryteria włączenia zostały zmienione z dniem 1 stycznia 2008 r. Przed tą datą w bazie danych uwzględniono niemowlęta z masą urodzeniową między 401 a 1500 gramów, które zostały przyjęte na OIOM KSOW w ciągu 14 dni od urodzenia.
