Description détaillée
L'étude de suivi du NICHD Neonatal Research Network est une étude de cohorte multicentrique dans laquelle les nourrissons survivants de poids extrêmement faible à la naissance subissent des évaluations neurodéveloppementales, neurosensorielles et fonctionnelles à l'âge corrigé de 22 à 26 mois (les nourrissons nés avant le 1er juillet 2012 ont été vus à 18-22 mois d'âge corrigé). L'objectif de l'étude est d'identifier les facteurs de risque maternels et néonatals potentiels susceptibles d'affecter le développement neurologique du nourrisson, notamment :
- Évaluer le développement des habiletés motrices, des habiletés cognitives, du langage et du comportement
- Détermination de la mortalité et de la prévalence de conditions médicales spécifiques
- Évaluer la relation entre la croissance et les résultats neurodéveloppementaux
- Évaluer la relation entre le statut socioéconomique et les résultats développementaux
- Évaluer l'utilisation des services de soutien spéciaux et des programmes d'intervention précoce par cette population
- Évaluer le besoin de suivi à l'âge scolaire.
Les évaluations programmées collectent : des informations démographiques ; Statut socioéconomique; antécédents médicaux; médicaments; équipement médical requis; données de croissance ; un examen neurologique détaillé; Échelles de Bayley du développement du nourrisson (mental, moteur, comportement du nourrisson); Liste de contrôle du comportement de l'enfant.
Une sous-étude évaluera un groupe de référence composé d'un nombre limité de nourrissons nés à terme et en bonne santé dans les centres du réseau pour atteindre les trois objectifs suivants : 1) éviter les biais de détermination potentiels dus aux attentes de l'examinateur lorsqu'il s'agit uniquement de nourrissons extrêmement prématurés ou d'autres nourrissons à haut risque. sont évalués 2) en l'absence de normes bien développées pour les échelles de Bayley, pour définir des seuils de déficience basés sur les données d'un échantillon représentatif d'enfants en bonne santé nés à terme dans nos centres et évalués simultanément par les mêmes examinateurs que pour notre haut- nourrissons à risque; et 3) pour aider à identifier et à traiter les cas où une "dérive" se produit au fil du temps dans la conduite et la notation des évaluations Bayley.

- Nourrissons innés dans les centres NRN
- <27 semaines d'âge gestationnel
- Nourrissons inscrits à une ou plusieurs études de suivi supplémentaires du NICHD NRN. Pour les nourrissons qui ne répondent pas aux critères d'inclusion ci-dessus, les critères d'inclusion et d'exclusion sont déterminés par les critères des essais supplémentaires. Dans ces cas, les nourrissons pesant plus de 1 000 grammes et/ou âgés de plus de 27 semaines peuvent être inclus dans l'étude FU.
Remarque : Ces critères d'inclusion ont été modifiés au 01/01/2008. Avant cette date, les nourrissons dont le poids à la naissance se situait entre 401 et 1 500 grammes et qui étaient admis dans les USIN du NRN dans les 14 jours suivant la naissance étaient inclus dans la base de données.
