O estudo de acompanhamento da Rede de Pesquisa Neonatal do NICHD é um estudo de coorte multicêntrico no qual bebês sobreviventes com peso extremamente baixo ao nascer passam por avaliações neurodesenvolvimentais, neurossensoriais e funcionais aos 22-26 meses de idade corrigida (bebês nascidos antes de 1º de julho de 2012 foram vistos aos 18-22 meses de idade corrigida). O objetivo do estudo é identificar potenciais fatores de risco maternos e neonatais que podem afetar o neurodesenvolvimento infantil, incluindo:
- Avaliando o desenvolvimento de habilidades motoras, habilidades cognitivas, linguagem e comportamento
- Determinar a mortalidade e a prevalência de condições médicas específicas
- Avaliação da relação entre o crescimento e o resultado do neurodesenvolvimento
- Avaliação da relação entre o status socioeconômico e o resultado do desenvolvimento
- Avaliar a utilização de serviços especiais de apoio e programas de intervenção precoce por esta população
- Avaliar a necessidade de acompanhamento na idade escolar.
As avaliações agendadas coletam: informações demográficas; status socioeconômico; histórico médico; medicamentos; equipamento médico necessário; dados de crescimento; um exame neurológico detalhado; Escalas Bayley de Desenvolvimento Infantil (mental, motor, comportamento infantil); Lista de Verificação do Comportamento Infantil.
Um subestudo avaliará um grupo de referência composto por um número limitado de bebês a termo saudáveis nascidos em centros da Rede para atender aos três objetivos a seguir: 1) evitar possíveis vieses de apuração devido às expectativas do examinador quando apenas bebês extremamente prematuros ou outros de alto risco são avaliadas 2) na ausência de normas bem desenvolvidas para as Escalas de Bayley, para definir limiares de comprometimento com base em dados de uma amostra representativa de crianças saudáveis nascidas a termo em nossos centros e avaliadas simultaneamente pelos mesmos examinadores de nossos centros de alta lactentes de risco; e 3) para ajudar a identificar e abordar quando ocorre "desvio" ao longo do tempo na condução e pontuação das avaliações de Bayley.

- Recém-nascidos em centros NRN
- <27 semanas de idade gestacional
- Bebês inscritos em um ou mais estudos adicionais de acompanhamento do NICHD NRN. Para bebês que não atendem aos critérios de inclusão acima, os critérios de inclusão e exclusão são determinados pelos critérios do(s) estudo(s) adicional(is). Nesses casos, bebês com mais de 1.000 gramas e/ou com mais de 27 semanas podem ser incluídos no Estudo FU.
Nota: Esses critérios de inclusão foram alterados a partir de 01/01/2008. Antes desta data, crianças com peso ao nascer entre 401 e 1.500 gramas que foram internadas em NRN NICUs dentro de 14 dias após o nascimento foram incluídas no banco de dados.
