Virkning af kostfibre som præbiotika på tarmmikrobiota hos overvægtige thailandske børn

Forekomsten af fedme blandt børn er stigende på verdensplan. Forekomsten af overvægt og fedme hos børn og unge er steget dramatisk fra 4 % til 18 % på 40 år.
Årsagen til fedme er gen-miljø interaktioner. Nylige beviser tyder på, at tarmmikrobiotaen er involveret i energiregulering såvel som inflammation. Definition af fedme for børn og teenagere er defineret som et BMI på eller over median +2 standardafvigelse (SD) af samme alder og køn fra Verdenssundhedsorganisationen ( WHO) reference Håndtering af fedme hos børn er terapeutisk livsstilsændring ved at ændre kostvaner og det fysiske aktivitetsniveau. Indtagelse af præbiotika, som er ikke-fordøjelige polysaccharider, der anvendes af tarmmikroorganismer, derefter mikrobielle skift som reaktion på præbiotisk indtagelsesændring i Bifidobacterium og fører til nedsat kropsvægt og fedt. Den mikrobielle metabolit kortkædede fedtsyrer (SCFA'er) har sandsynligvis indvirkning på forskellige aspekter af værtens fysiologi og kan derefter falde i kropsvægt og fedt.
Inflammationsmekanismen ved fedme, Lipopolysaccharider (LPS), som stammer fra den ydre cellemembran af Gram-negative bakterier, er den udløsende faktor for inflammation. LPS krydser mave-tarmslimhinden, så når de det systemiske kredsløb og udløser medfødt immunrespons aktiverer modningen af IL-1β. Cirkulerende LPS-niveauer var forbundet med forhøjede TNF-a- og IL-6-koncentrationer i adipocytter.
Fruktaner af inulintypen er ufordøjelige, fuldt opløselige og fermenterbare fødevareingredienser med kendte præbiotiske egenskaber, som findes naturligt i cikorierod og jordskok, en plante dyrket i Thailand, som fermenteres i tyktarmen for at producere SCFA. Bifidobakterier stimuleres fortrinsvis til at vokse ved at øge antallet af sundhedsfremmende bakterier og reducere antallet af potentielt skadelige arter.
Der var kun én undersøgelse om virkningen af præbiotika på sammensætningen af tarmmikrobiotaen hos børn med overvægt eller fedme. Undersøgelsen udførte et randomiseret kontrolleret forsøg for at studere børn, 7-12 år gamle, med overvægt eller fedme. Deltagerne blev tilfældigt fordelt til grupper, der fik enten oligofructoseberiget inulin (OI; 8 g/dag; n = 22) eller maltodextrin placebo (isokalorisk dosis, kontroller; n = 20) én gang dagligt i 16 uger. Fækale prøver blev indsamlet ved baseline og 16 uger, og sammensætningen af mikrobiotaen blev analyseret ved 16S ribosomal ribonukleinsyre (rRNA) sekventering og qPCR. Det primære resultat var ændring i procent kropsfedt fra baseline til 16 uger. Efter 16 uger viste kvantitativ polymerasekædereaktion (qPCR) en signifikant stigning i Bifidobacterium spp. i OI-gruppen sammenlignet med kontroller. 16S rRNA-sekventering afslørede signifikante stigninger i arter af slægten Bifidobacterium og fald i Bacteroides vulgatus inden for gruppen, der indtog OI. Børn, der indtog OI havde signifikante fald i kropsvægt z-score (fald på 3,1%), procent kropsfedt (fald på 2,4 %), procent kropsfedt (fald på 3,8 %), interleukin 6 fra baseline (fald på 15 %) sammenlignet med børn, der fik placebo.

- Body mass index (BMI) ≥ median + 2 standardafvigelse (SD)
Ekskluderingskriterier:
- Underliggende sygdom af syndromisk fedme og monogen fedme
- Endokrine årsager til fedme (f. hypothyroidisme, væksthormonmangel)
- Brug af lægemidler, der påvirker appetitten eller kropsvægten (f. kortikosteroider)
- Deltagelse i andre samtidige vægtreduktionsprogrammer

The relative abundance of Bifidobacterium was determined using 16S rRNA gene sequencing from stool samples.
The abundance values were then transformed into z-scores based on the distribution of the entire study population at baseline.
A z-score of 0 represents the mean abundance of the population, while positive or negative values indicate the number of standard deviations above or below the mean, respectively.

FMI and FFMI were calculated as the weight of fat mass or fat-free mass in kilograms divided by the square of height in meters (kg/m^2).

A positive change value indicates an increase in the respective mass index from baseline, while a negative value indicates a decrease.
FFMI measures the amount of fat-free mass (including muscle) relative to height.

Variable Importance in Projection (VIP) scores were derived from Orthogonal Projections to Latent Structures Discriminant Analysis (OPLS-DA) to assess the contribution of each metabolite to group separation.
VIP scores are continuous, non-negative values with a theoretical minimum of 0. There is no theoretical maximum or fixed upper limit for a VIP score.
A value greater than 1.0 is a universally accepted threshold indicating that a metabolite contributes significantly to the model's projection.
For this analysis, VIP scores are reported as single, group-level cumulative metrics derived directly from the overall multivariate OPLS-DA model.
Because these are model-derived descriptive values reflecting whole-group patterns rather than participant-level summary statistics, individual-level variance does not exist, and standard deviations are not applicable.
