Impact van voedingsvezels als prebiotica op de darmmicrobiota bij zwaarlijvige Thaise kinderen

De prevalentie van obesitas bij kinderen neemt wereldwijd toe. De prevalentie van overgewicht en obesitas bij kinderen en adolescenten is in 40 jaar tijd dramatisch gestegen van 4% naar 18%.
Oorzaak van obesitas zijn gen-omgevingsinteracties. Recent bewijs suggereert dat de darmmicrobiota betrokken is bij energieregulatie en ontstekingen. De definitie van obesitas voor kinderen en tieners wordt gedefinieerd als een BMI op of boven mediaan +2 standaarddeviatie (SD) van dezelfde leeftijd en hetzelfde geslacht van de Wereldgezondheidsorganisatie ( WHO) referentie De behandeling van obesitas bij kinderen is een therapeutische verandering van levensstijl door de voedingsgewoonten en het niveau van fysieke activiteit te veranderen. Consumptie van prebiotica, dit zijn niet-verteerbare polysacchariden die worden gebruikt door darmmicro-organismen en vervolgens microbiële verschuivingen als reactie op veranderingen in de prebiotische inname in Bifidobacterium en leiden tot een verminderd lichaamsgewicht en adipositas. De microbiële metaboliet korteketenvetzuren (SCFA's) hebben waarschijnlijk invloed op verschillende aspecten van de fysiologie van de gastheer en kunnen vervolgens het lichaamsgewicht en de adipositas verminderen.
Het mechanisme van ontsteking bij obesitas, Lipopolysacchariden (LPS) die zijn afgeleid van het buitenste celmembraan van Gram-negatieve bacteriën, zijn de triggerfactor van ontsteking. LPS passeert het maagdarmslijmvlies, bereikt vervolgens de systemische circulatie en activeert de aangeboren immuunrespons activeert de rijping van IL-1β. Circulerende LPS-niveaus waren geassocieerd met verhoogde TNF-α- en IL-6-concentraties in adipocyten.
Inuline-type fructanen zijn niet-verteerbare, volledig oplosbare en fermenteerbare voedselingrediënten met bekende prebiotische eigenschappen, die van nature voorkomen in cichoreiwortel en artisjok, een plant die in Thailand wordt gekweekt, die in de dikke darm worden gefermenteerd om SCFA te produceren. Bifidobacteriën worden bij voorkeur gestimuleerd om te groeien, door het aantal gezondheidsbevorderende bacteriën te vergroten en het aantal potentieel schadelijke soorten te verminderen.
Er was slechts één studie over het effect van prebiotica op de samenstelling van de darmmicrobiota bij kinderen met overgewicht of obesitas. De studie voerde een gerandomiseerde gecontroleerde studie uit om kinderen van 7-12 jaar oud met overgewicht of obesitas te bestuderen. Deelnemers werden willekeurig toegewezen aan groepen die ofwel met oligofructose verrijkte inuline (OI; 8 g/dag; n = 22) of maltodextrine-placebo (isocalorische dosis, controles; n = 20) eenmaal daags gedurende 16 weken kregen. Fecale monsters werden verzameld bij aanvang en 16 weken en de samenstelling van de microbiota werd geanalyseerd door 16S ribosomaal ribonucleïnezuur (rRNA) sequencing en qPCR. De primaire uitkomstmaat was verandering in percentage lichaamsvet vanaf baseline tot 16 weken. Na 16 weken toonde kwantitatieve polymerasekettingreactie (qPCR) een significante toename van Bifidobacterium spp. in de OI-groep vergeleken met controles. 16S rRNA-sequencing onthulde significante toenames in soorten van het geslacht Bifidobacterium en afnames in Bacteroides vulgatus binnen de groep die OI consumeerde. Kinderen die OI consumeerden hadden significante afnames in lichaamsgewicht z-score (afname van 3,1%), percentage lichaamsvet (afname van 2,4%), percentage rompvet (afname van 3,8%), interleukine 6 ten opzichte van baseline (afname van 15%) in vergelijking met kinderen die placebo kregen.

- Kinderen van 7 tot 15 jaar
- Body mass index (BMI) ≥ mediaan + 2 Standaarddeviatie (SD)
Uitsluitingscriteria:
- Onderliggende ziekte van syndromale obesitas en monogene obesitas
- Endocriene oorzaken van obesitas (bijv. hypothyreoïdie, groeihormoondeficiëntie)
- Gebruik van geneesmiddelen die de eetlust of het lichaamsgewicht beïnvloeden (bijv. corticosteroïden)
- Bijwonen van andere gelijktijdige programma's voor gewichtsvermindering

Variable Importance in Projection (VIP) scores were derived from Orthogonal Projections to Latent Structures Discriminant Analysis (OPLS-DA) to assess the contribution of each metabolite to group separation.
VIP scores are continuous, non-negative values with a theoretical minimum of 0. There is no theoretical maximum or fixed upper limit for a VIP score.
A value greater than 1.0 is a universally accepted threshold indicating that a metabolite contributes significantly to the model's projection.
For this analysis, VIP scores are reported as single, group-level cumulative metrics derived directly from the overall multivariate OPLS-DA model.
Because these are model-derived descriptive values reflecting whole-group patterns rather than participant-level summary statistics, individual-level variance does not exist, and standard deviations are not applicable.
