En randomiseret kontrolleret fase II, to-arm undersøgelse af navlestrengsblodcelletransplantation (MC001) i skadet rygmarv efterfulgt af bevægelsestræning for patienter med kroniske komplette rygmarvsskader (SCI).

Dette studie er et randomiseret kontrolleret, fase II, to-arm studie af navlestrengsblod mononukleær celle (MC001) transplantation i den skadede rygmarv efterfulgt af bevægelsestræning i op til 5±1 time om dagen, 5±1 dage om ugen , i 3-6 måneder.
I alt 18 forsøgspersoner med kronisk komplet rygmarvsskade (SCI) vil blive randomiseret til en af de to grupper. Forsøgspersonerne, der er tildelt gruppe A, vil modtage 6,4 millioner UCBMNC (MC001) transplanteret ind i de dorsale rodindgangszoner over og under skadestedet, der er udsat ved en laminektomi. Forsøgspersoner i gruppe B vil ikke have MC001-transplantation eller operation. Alle forsøgspersoner vil modtage 3-6 måneders intensiv bevægelsestræning.

Beskrivelse
Inklusionskriterier:
- Mandlige og kvindelige forsøgspersoners ≥18 til ≤60 år.
- Traumatisk SCI på et neurologisk niveau (det laveste sammenhængende segmentale rygmarvsniveau, der har intakt motorisk og sensorisk score) mellem C5 og T11 ved MR.
Bemærk: For de første tre emner på hvert studiecenter vil det neurologiske niveau af SCI være begrænset til thoraxregionen (mellem T1 og T11).
- Forsøgspersoner med kronisk SCI (defineret som ≥ 12 måneder efter indledende SCI-operation) med stabile neurologiske fund i mindst seks måneder og være i stand til at stå mindst 1 time/dag ved hjælp af et ståstel, vippebord eller tilsvarende anordning.
- Forsøgspersoner med en aktuel neurologisk status på ASIA svækkelse grad A (fuldstændig).
- Det skadede sted i rygmarven er inden for tre hvirvelniveauer som bekræftet ved MR-scanning.
- Forsøgspersonen skal være i godt nok fysisk helbred til at tolerere operationen og deltage i det intensive gåprogram.
- Klinisk normalt hvile 12-aflednings-EKG ved screeningbesøg eller, hvis unormalt, vurderet som ikke klinisk signifikant af hovedforskeren.
- Både mandlige og kvindelige forsøgspersoner og deres partnere i den fødedygtige alder skal acceptere at bruge medicinsk accepterede præventionsmetoder.
- Villig og i stand til at deltage i alle aspekter af undersøgelsen, herunder færdiggørelse af subjektive evalueringer, deltagelse ved planlagte klinikbesøg og overholdelse af alle protokolkrav som dokumenteret ved at give skriftligt informeret samtykke.
- Mindst én frossen HLA-matchet (≥4:6 til en specifik patient) CBU identificeret for hvert individ.
Ekskluderingskriterier:
- Klinisk signifikante nyre-, kardiovaskulære, lever- og psykiatriske sygdomme eller andre tilstande, der kan øge risikoen for komplikationer under eller efter operationen eller kan reducere patientens evne til at deltage i intens bevægelsestræning baseret på investigatorens medicinske vurdering.
- Tilstedeværelse af enhver klinisk signifikant medicinsk tilstand eller infektion (herunder, men ikke begrænset til, bæreren af hepatitis B-virus eller HIV), som efter investigatorens mening kunne forstyrre behandlingen eller deltagelsen i undersøgelsen.
- Personer med slap lammelse med fravær af dybe senereflekser i benene, alvorlig atrofi af underekstremiteterne eller andre tegn på lumbosakral skade, perifer nerveskade og motoneuronalt tab.
- Brud på vægtbærende knogler og led. Disse omfatter brud på lårbenet, skinnebenet og fibula samt ankel-, knæ- eller hofteled. Hvis sådanne brud er helet, kan patienten inddrages i forsøget.
- Skade på hjerne, perifer nerve eller muskel, der kan forstyrre neurologisk vurdering eller gang.
- Gravid eller ammende kvinde.
- Utilgængelighed af HLA-matchede navlestrengsblodceller.
Enhver kontraindikation for laminektomioperation eller lokomotorisk træning omfatter:
- Patient med aktive infektionssygdomme.
- Patient med sårinfektion på eller nær implantationsstedet.
- Patient med alvorlig deformitet af rygsøjlen på eller nær implantationsstedet.
- Patienten har immunkompromitteret tilstand eller har kendte autoimmune tilstande eller er seropositiv med human immundefektvirus (HIV).
- Patienten har igangværende moderat til svær organsvækkelse ud over undersøgelsen.
- Personer med unormal nyrefunktion, kardiovaskulær sygdom, depression ved screening vil blive udelukket, hvis den vurderes klinisk signifikant og ustabil af hovedforskeren.
- Forsøgsperson, der i øjeblikket deltager i et andet forsøgsstudie eller har taget et forsøgslægemiddel inden for de sidste 4 uger før screening for denne undersøgelse.
- Eventuelle andre kriterier, som efter investigators mening tyder på, at forsøgspersonen ikke ville være i overensstemmelse med undersøgelsesprotokollen og/eller ikke ville være egnet til at deltage i denne undersøgelse.
- Person med lav knogletæthed (DEXA-scanning resulterede i T-score < -4).

(2) Motorisk score: Der er en maksimal score på 25 for hver ekstremitet, i alt 50 for de øvre lemmer og 50 for de nedre lemmer.
