この研究は、臍帯血単核細胞 (MC001) を損傷した脊髄に移植し、その後、1 日 5±1 時間、週に 5±1 日までの自発運動訓練を行う無作為化対照第 II 相 2 群研究です。 、3〜6ヶ月間。
慢性完全脊髄損傷(SCI)の合計18人の被験者が、2つのグループのいずれかに無作為に割り付けられます。 グループ A に割り当てられた被験者は、椎弓切除術によって露出した損傷部位の上下の後根エントリ ゾーンに移植された 640 万個の UCBMNC (MC001) を受け取ります。 グループBの被験者は、MC001移植または手術を受けません。 すべての被験者は、3〜6か月の集中的な運動訓練を受けます。

説明
包含基準:
- -18歳以上60歳以下の男性および女性被験者。
- -MRIによるC5とT11の間の神経学的レベル(無傷の運動および感覚スコアを持つ最も低い隣接脊髄分節レベル)の外傷性SCI。
注: 各スタディ センターの最初の 3 人の被験者については、SCI の神経学的レベルは胸部領域 (T1 と T11 の間) に限定されます。
- -慢性SCI(最初のSCI手術後12か月以上と定義)の被験者で、少なくとも6か月間安定した神経学的所見があり、スタンディングフレーム、チルトテーブル、または同等のデバイスを使用して少なくとも1日1時間立つことができます。
- -ASIA障害グレードA(完全)の現在の神経学的状態の被験者。
- 脊髄の損傷部位は、MRI スキャンによって確認されるように、3 つの脊椎レベル内にあります。
- 被験者は、手術に耐え、集中的な歩行プログラムに参加するのに十分な健康状態にある必要があります。
- -スクリーニング訪問時の臨床的に正常な安静時12誘導心電図、または異常な場合、主任研究者によって臨床的に重要ではないと見なされます。
- 男性と女性の両方の被験者と、出産の可能性のあるパートナーは、医学的に認められた避妊方法を使用することに同意する必要があります。
- -主観的評価の完了、予定されたクリニック訪問への出席、および書面によるインフォームドコンセントを提供することによって証明されるすべてのプロトコル要件の順守を含む、研究のすべての側面に参加する意思と能力。
- -少なくとも1つの凍結HLA一致(特定の患者に対して4:6以上)のCBUが各被験者について特定されました。
除外基準:
- -臨床的に重要な腎臓、心血管、肝臓、精神疾患、または手術中または手術後の合併症のリスクを高める可能性のあるその他の状態、または治験責任医師の医学的判断に基づく激しい運動訓練に参加する患者の能力を低下させる可能性があります。
- -臨床的に重要な病状または感染症(B型肝炎ウイルスまたはHIVの保因者を含むがこれらに限定されない)の存在。治験責任医師の意見では、研究への治療または参加を妨げる可能性があります。
- -脚の深部腱反射がない弛緩性麻痺、下肢の重度の萎縮、または腰仙骨損傷、末梢神経損傷、および運動ニューロン喪失の他の証拠を伴う被験者。
- 体重を支える骨や関節の骨折。 これらには、大腿骨、脛骨、および腓骨の骨折、ならびに足首、膝、または股関節の骨折が含まれます。 そのような骨折が治癒した場合、患者は試験に参加できます。
- -神経学的または歩行評価を妨げる可能性のある脳、末梢神経、または筋肉の損傷。
- 妊娠中または授乳中の女性。
- HLAが一致した臍帯血細胞が利用できない。
椎弓切除術または運動訓練の禁忌には、次のものがあります。
- -アクティブな感染症の患者。

- -現在別の治験に参加している被験者、またはこの治験のスクリーニング前の過去4週間以内に治験薬を服用している被験者。

- 骨密度が低い被験者 (DEXA スキャンの結果、T スコア < -4)。
