Beschreibung
Einschlusskriterien:
- Klinische Diagnose einer motorischen inkompletten Rückenmarksverletzung (SCI).
- SCI liegt oberhalb des 12. Brustwirbels.
- Fehlende permanente Knöchelgelenkskontrakturen.
- SCI trat 6 Monate vor Aufnahme in die Studie auf.
Ausschlusskriterien:
- Supraspinale Läsionen
- Neuropathien des peripheren Nervensystems
- Degenerative neurologische Erkrankungen der Wirbelsäule oder des Rückenmarks
- Motor komplett SCI
- Vorhandensein von Druckstellen
- Harnwegsinfekt
- Neoplastische oder vaskuläre Erkrankungen der Wirbelsäule oder des Rückenmarks
- Schwangere Frauen oder Frauen, die vermuten, schwanger zu sein oder schwanger zu werden.
- Menschen mit Cochlea-Implantaten, Herzschrittmachern und implantierten Stimulatoren
- Menschen mit Vorgeschichte von Anfällen
- Menschen mit implantierter Baclofen-Pumpe

Nur robotisches Gangtraining
|
Fünfzehn Menschen mit Rückenmarksverletzungen erhalten täglich 20 Sitzungen mit robotergestütztem Gangtraining.
Während des assistierten Gehens erhalten sie während der Standphase des Gangs auch eine nicht-invasive transspinale Stimulation als Impulsfolge bei 30 Hz.
Vor und nach dem Training werden standardisierte klinische und neurophysiologische Tests durchgeführt, um die Erholung der sensomotorischen Funktion zu beurteilen.

Plastizität kortikaler und kortikospinaler neuronaler Schaltkreise

Neurophysiologische Tests zur Untersuchung der kortikalen und kortikospinalen Erregbarkeit werden vor und nach dem Eingriff gemessen.
Transkranielle Einzelpuls-Magnetstimulation (TMS) wird verwendet, um die Rekrutierungskurve von motorisch evozierten Potentialen zusammenzustellen, und Paarpuls-TMS wird verwendet, um Veränderungen in kortikalen hemmenden und unterstützenden neuronalen Schaltkreisen zu untersuchen.

Neurophysiologische Tests zur Untersuchung der spinalen Reflexerregbarkeit werden vor und nach jedem Eingriff durch Stimulation der N. tibialis posterior und suralis während des Lokomat-assistierten Steppings gemessen, wobei die Amplitudenmodulation des Soleus-H-Reflexes und des Tibialis-Anterior-Flexor-Reflexes dargestellt wird.
