Beskrivelse
Inklusionskriterier:
- Klinisk diagnose af motorisk inkomplet rygmarvsskade (SCI).
- SCI er over thorax 12 hvirvel.
- Manglende permanente ankelledskontrakturer.
- SCI opstod 6 måneder før tilmelding til undersøgelsen.
Eksklusionskriterier:
- Supraspinal læsioner
- Neuropatier i det perifere nervesystem
- Degenerative neurologiske lidelser i rygsøjlen eller rygmarven
- Motor komplet SCI
- Tilstedeværelse af tryksår
- Urinvejsinfektion
- Neoplastiske eller vaskulære lidelser i rygsøjlen eller rygmarven
- Gravide kvinder eller kvinder, der har mistanke om, at de kan være eller kan blive gravide.
- Mennesker med cochleaimplantater, pacemaker og implanterede stimulatorer
- Mennesker med anfaldshistorie
- Mennesker med implanteret Baclofen-pumpe

SHAM_COMPARATOR: Robotisk gangtræning
Kun robotgangtræning
|
Femten personer med rygmarvsskade vil modtage 20 daglige sessioner med robot-gangtræning.
Under assisteret stepping vil de også modtage ikke-invasiv transspinal stimulation som et pulstog ved 30 Hz under standfasen af gangarten.
Før og efter træning vil standardiserede kliniske og neurofysiologiske tests blive brugt til at vurdere genopretning af sensorimotorisk funktion.

EKSPERIMENTEL: Robotisk gangtræning og lavfrekvent transspinal stimulation.
Robotisk gangtræning vil blive administreret sammen med ikke-invasiv transspinal stimulation over thoracolumbarområdet under assisteret stepping ved lav frekvens (0,3 Hz).

Femten personer med rygmarvsskade vil modtage 20 daglige sessioner med robot-gangtræning.
Under assisteret stepping vil de også modtage ikke-invasiv transspinal stimulation som en enkelt puls ved 0,3 Hz under standfasen af gangarten.
Før og efter træning vil standardiserede kliniske og neurofysiologiske tests blive brugt til at vurdere genopretning af sensorimotorisk funktion.

Plasticitet af kortikale og corticospinal neuronale kredsløb
Tidsramme: 3 år
|
Neurofysiologiske test, der undersøger kortikal og kortikospinal excitabilitet, vil blive målt før og efter interventionen.
Single-puls transcranial magnetic stimulation (TMS) vil blive brugt til at samle rekrutteringskurven af motorisk fremkaldte potentialer, og paired-pulse TMS vil blive brugt til at undersøge ændringer i kortikale hæmmende og faciliterende neuronale kredsløb.
