Beschreibung
Einschlusskriterien:
- Positive Blutkultur mit Gram-Färbung, die GNB zeigt
- Zum Zeitpunkt des Ergebnisses der Gram-Färbung im Krankenhaus
Ausschlusskriterien:
- Positive Blutkultur für GNB in derselben Einrichtung innerhalb der letzten 7 Tage (sofern zum Zeitpunkt des Gram-Färbungsergebnisses bekannt)
- Zum Zeitpunkt des Ergebnisses der Gram-Färbung verstorben
- Grampositive Bazillen, grampositive Kokken, gramnegative Kokken, Hefen, Pilze oder mehrere GNB-Morphologien, nachgewiesen in der Gram-Färbung einer Blutkultur
- Frühere Einschreibung in diese Studie

Kein Eingriff: Pflegestandard
Wenn eine Blutkultur, die GNB anbaut, in den Standardbehandlungsarm randomisiert wird, werden die positiven Blutkulturen mithilfe eines Standardbehandlungstests auf antimikrobielle Empfindlichkeit (AST) charakterisiert.

Aktiver Komparator: Aufdecken
Wenn eine Blutkultur mit wachsendem GNB randomisiert dem Reveal-Arm zugeteilt wird, wird die positive Blutkultur mithilfe von Reveal, einem schnellen AST und dem Standard-AST der Pflege charakterisiert.
|
Reveal ist eine schnelle AST-Methode, die die Sensortechnologie kleiner Moleküle nutzt, um das Wachstum von Bakterienpopulationen durch Messung flüchtiger Metaboliten zu erkennen, und AST-Ergebnisse in ca. 5 Stunden liefert.
Reveal™ ist für den klinischen Einsatz in der Europäischen Union (EU) und in Israel zugelassen und die Zulassung in Indien läuft derzeit. Es bietet minimale Hemmkonzentrationen (MHK) für 28 Antibiotika und 9 gramnegative Spezies, die zusammen etwa 90 % der Organismen ausmachen verursacht gramnegative Blutstrominfektionen (BSI).

Participant Clinical Outcomes, as Measured by Desirability of Outcome Ranking (DOOR)

|
The composite 3-category DOOR outcome will assess three deleterious events (unsuccessful discharge, lack of clinical response, and undesirable events) in addition to survival up to 30 days after Gram stain result. The primary DOOR outcome measure is defined using three ordered levels. From best to worst, they are:

|
New acquisition is defined as detection of MDRO/C. difficile in subjects who do not have preceding clinical or surveillance cultures with these organisms in the prior 3 months. MDRO will be identified on routine clinical or surveillance samples using local laboratory diagnostic procedures and include: Methicillin-resistant Staphylococcus aureus Vancomycin-resistant Enterococcus species 3rd generation cephalosporin-non-susceptible Enterobacterales Carbapenem-resistant Enterobacterales, as defined by the Center for Disease Control and Prevention: resistant to imipenem, meropenem, doripenem, or ertapenem OR documentation that the isolate produces a carbapenemase Pseudomonas aeruginosa resistant to carbapenems/multi-drug resistant (resistant to aminoglycosides, cephalosporins, fluoroquinolones, and carbapenems) Carbapenem-resistant Acinetobacter species Candida auris |
