Beschreibung
Wichtige Einschlusskriterien:
- Lebenserwartung ≥ 12 Wochen.
- Messbare Krankheit gemäß RECIST v1.1.
- Histologisch oder zytologisch bestätigte lokal fortgeschrittene (nicht resezierbare) oder metastasierte UC des Nierenbeckens, Harnleiters, der Blase oder der Harnröhre.
- Archiviertes oder frisches Tumorgewebe, das muskelinvasives UC oder lokal fortgeschrittenes oder metastasiertes UC umfasst, sollte zur Einreichung beim Zentrallabor verfügbar sein.
- Negativer Schwangerschaftstest für Frauen im gebärfähigen Alter (WOCBP) (negativer Serumtest beim Screening und negativer Urin- oder Serumtest innerhalb von 72 Stunden vor der ersten Dosis).
- Kohorte 1: Zuvor unbehandelt: Anspruch auf eine platinbasierte Chemotherapie (entweder Cisplatin- oder Carboplatin-basierte Chemotherapie, basierend auf der Entscheidung des Prüfarztes).
Kohorte 1: Die Teilnehmer dürfen zuvor keine systemische Therapie gegen lokal fortgeschrittene oder metastasierte UC erhalten haben, mit den folgenden Ausnahmen:
- Vorherige lokale intravesikale Chemotherapie, lokale Operation, wenn keine vollständige Resektion erreicht werden kann, lokale Immuntherapie und Strahlentherapie sind zulässig, wenn sie mindestens 4 Wochen vor Beginn der Studienbehandlung abgeschlossen sind und alle akuten Toxizitäten abgeklungen sind.
- Vorherige neoadjuvante/adjuvante Chemotherapie oder auf Monomethylauristatin E (MMAE) basierende Therapie mit Rezidiv >12 Monate nach Abschluss der Therapie.
- Vorherige neoadjuvante/adjuvante Immun-Checkpoint-Inhibitor-Therapie mit Rezidiv >12 Monate nach Abschluss der Therapie.
- Kohorte 2: Zuvor behandelt: Die Teilnehmer müssen ≥ 1 vorherige systemische Behandlung für lokal fortgeschrittenes oder metastasiertes UC erhalten haben. Dies schließt eine neoadjuvante/adjuvante platinbasierte Chemotherapie ein, wenn innerhalb von 12 Monaten nach Abschluss der Therapie ein Rezidiv auftrat.
- Kohorte 2: Fortschreiten oder Wiederauftreten der UC während oder nach Erhalt der letzten Therapie.
Wichtige Ausschlusskriterien:
- Aktive Keratitis oder Hornhautgeschwüre.
- Während des Studiums ist eine Behandlung mit starken Inhibitoren oder starken Induktoren des menschlichen Cytochroms P450 3A (CYP3A) oder Inhibitoren des P-Glykoproteins (P-gp), einschließlich pflanzlicher oder lebensmittelbasierter Inhibitoren, erforderlich.
- Jeder Zustand, der eine aktuelle Behandlung mit hochdosierten Kortikosteroiden erfordert (> 10 mg täglich Prednison oder Äquivalent).
- Bekannte Überempfindlichkeit oder Allergie gegen einen der Inhaltsstoffe einer der Studieninterventionen oder gegen MMAE.
- Hat sich von einer kürzlich erfolgten größeren Operation (mit Ausnahme der Platzierung eines Gefäßzugangs) nicht ausreichend erholt.
- Erhalt eines Lebendimpfstoffs oder abgeschwächten Impfstoffs innerhalb von 30 Tagen nach der ersten Dosis.
- Kohorte 1: Bisher unbehandelt: Vorherige Behandlung mit einem Checkpoint-Inhibitor (CPI) wegen einer anderen bösartigen Erkrankung innerhalb der letzten 12 Monate.
- Kohorte 2: Zuvor behandelt: Hat zuvor mehr als eine platinbasierte Chemotherapie gegen lokal fortgeschrittenes oder metastasiertes UC erhalten. Dies schließt eine neoadjuvante/adjuvante platinbasierte Chemotherapie ein, wenn innerhalb von 12 Monaten nach Abschluss der Therapie ein Rezidiv auftrat.
- Kohorte 2: Vorherige Behandlung mit Enfortumab Vedotin oder einer anderen MMAE-basierten Therapie

Aktiver Komparator: Kohorte 1: Arm 3
Die Teilnehmer erhalten eine Platin-basierte Kombinationschemotherapie +/- Avelumab-Erhaltungstherapie

Die Teilnehmer erhalten Gemcitabin an den Tagen 1 und 8 jedes 21-Tage-Zyklus sowie Cisplatin oder Carboplatin an Tag 1 jedes 21-Tage-Zyklus.
Nach 4–6 Zyklen Gemcitabin + Cisplatin oder Carboplatin erhalten die Teilnehmer, sofern klinisch indiziert, an den Tagen 1 und 15 jedes 28-Tage-Zyklus eine Erhaltungstherapie mit Avelumab.
