Zu bewerten:
- die Inzidenz venöser thromboembolischer Ereignisse (VTE)
- die Inzidenz hämorrhagischer Komplikationen, in einer Population von Patienten mit Myelom, die mit IMiDs behandelt werden und eine 6-monatige Thromboseprophylaxe benötigen, unter Verwendung eines oralen Anti-Xa-Antikoagulans, Apixaban, in einem präventiven Schema, 2,5 mg x2/Tag

Beschreibung
Einschlusskriterien:
- Patienten (Männer/Frauen) über 18 Jahre
- Alle konsekutiven Patienten mit Myelom, in Erstlinienbehandlung oder im Rückfall, die behandelt werden – mit IMiDs (MPT, Melphalan – Prednison – Thalidomid; Lenalidomid – Dexamethason).
UND
- die eine Vorbeugung venöser thromboembolischer Ereignisse mit Aspirin oder niedermolekularem Heparin (LMWH) für eine Mindestdauer von 6 Monaten benötigen. Mindestens 2/3 der Patienten werden mit Lenalidomid-Dexamethason behandelt.
- Schriftliche Einverständniserklärung
- Patienten, die dem französischen Sozialversicherungssystem oder einem gleichwertigen System angeschlossen sind
Ausschlusskriterien:
- Patient, der eine kurative gerinnungshemmende Behandlung (Heparin, LMWH, Vitamin-K-Antagonisten, Dabigatran, Rivaroxaban, Apixaban) für eine assoziierte Erkrankung (mechanische Klappe, Vorhofflimmern oder venöse thromboembolische Erkrankung in den letzten 6 Monaten) benötigt.
- Patient, der postoperativ eine vorbeugende Behandlung mit einem Antikoagulans benötigt
- Patient, der eine Thrombozytenaggregationshemmung benötigt (Aspirin, Clopidogrel, Prasugrel, Ticagrelor oder duale Thrombozytenaggregationshemmung)
- Patient mit aktiver Blutung oder hohem Blutungsrisiko (Ulkuserkrankung, intrakranielle Blutung in den letzten 6 Monaten, unkontrollierter Bluthochdruck)
- Patienten, die sich innerhalb der letzten 30 Tage einem chirurgischen Eingriff unterzogen haben, der sie wahrscheinlich einem Blutungsrisiko aussetzt
- Aktive Lebererkrankung (Hepatitis, Zirrhose)
- Schwere Niereninsuffizienz (Kreatinin-Clearance nach der Cockcroft-Gleichung < 30 ml/min)
- Bekannte allergische Reaktion auf Apixaban
- Kontraindikation für die Verwendung einer gerinnungshemmenden Behandlung
Verbotene Begleitbehandlung
- Inhibitoren von CYP3A4 und P-gp: Azol-Antimykotika (Ketoconazol, Itraconazol, Voriconazol, Posaconazol), Inhibitoren der HIV-Protease (Ritonavir, Indinavir, Nelfinavir, Atazanavir, Saquinavir), spezifische Makrolid-Antibiotika (Clarithromycin, Telithromycin)
- andere antithrombotische Behandlung: Salicylatderivate (Aspirin, aspirinhaltige Produkte), Thrombozytenaggregationshemmer, Heparin (unfraktioniertes Heparin, niedermolekulares Heparin, Danaparoid-Natrium, Fondaparinux), Hirudine, orale Antikoagulanzien (Vitamin-K-Antagonisten, Rivaroxaban, Dabigatran)
- Patient mit AST- oder ALT-Frequenz > 3-fache Obergrenze des Normalwerts
- Patient mit Bilirubin-Rate > 1,5-facher Obergrenze des Normalwerts
- Patient mit Thrombozytenzahl < 75 G/l
- Patient mit Kreatinin-Clearance (Cockcroft) < 30 ml/min
- Zufallsbefund einer proximalen tiefen Venenthrombose im Screening-Ultraschall
- Patienten, die sich weigern oder nicht in der Lage sind, eine schriftliche Zustimmung zu Informationen zu geben
- Der Patient ist nach Meinung des Prüfarztes nicht in der Lage, die Protokollanforderungen zu erfüllen
- Lebenserwartung weniger als 6 Monate
- Inhaftierte Patienten
- Schwangerschaft oder Möglichkeit einer Schwangerschaft innerhalb von 6 Monaten
- Frauen im gebärfähigen Alter ohne zuverlässige Empfängnisverhütung
- Ökog > 2
