Valutazione dell'uso di un anticoagulante orale diretto anti-Xa, Apixaban, nella prevenzione della malattia tromboembolica venosa in pazienti trattati con IMiD durante il mieloma: uno studio pilota
Valutare:
- l'incidenza di eventi tromboembolici venosi (TEV)
- l'incidenza di complicanze emorragiche, In una popolazione di pazienti con mieloma che sono trattati con IMiD e richiedono tromboprofilassi per 6 mesi, utilizzando un anticoagulante orale anti-Xa, Apixaban, in uno schema preventivo, 2,5 mg x2/die

Descrizione
Criterio di inclusione:
- Pazienti (uomini/donne) di età superiore a 18 anni
- Tutti i pazienti consecutivi, con mieloma, in trattamento di prima linea o in recidiva, trattati - Con IMiD (MPT, Melfalan -Prednisone - Talidomide; Lenalidomide - Desametasone).
E
- che richiedono la prevenzione degli eventi tromboembolici venosi con Aspirina o Eparina a basso peso molecolare (LMWH) per una durata minima di 6 mesi Almeno 2/3 dei pazienti saranno trattati con Lenalidomide-Desametasone.
- Consenso informato scritto
- Pazienti affiliati al sistema di sicurezza sociale francese o equivalente
Criteri di esclusione:
- Paziente che necessita di trattamento anticoagulante curativo (eparina, EBPM, antagonisti della vitamina K, Dabigatran, Rivaroxaban, Apixaban) per un disturbo associato (valvola meccanica, fibrillazione atriale o malattia tromboembolica venosa nei 6 mesi precedenti).
- Paziente che necessita di un trattamento preventivo con un anticoagulante in un contesto post-operatorio
- Paziente che necessita di trattamento antipiastrinico (Aspirina, Clopidogrel, Prasugrel, Ticagrelor o doppia terapia antipiastrinica)
- Paziente con sanguinamento attivo o ad alto rischio di sanguinamento (malattia ulcerosa, sanguinamento intracranico nei 6 mesi precedenti, ipertensione incontrollata)
- Paziente che ha subito un intervento chirurgico negli ultimi 30 giorni suscettibile di esporlo a rischio emorragico
- Malattia epatica attiva (epatite, cirrosi)
- Grave insufficienza renale (clearance della creatinina usando l'equazione di Cockcroft < 30 ml/mn)
- Reazione allergica nota ad Apixaban
- Controindicazione all'uso di un trattamento anticoagulante
Trattamento concomitante vietato
- inibitori del CYP3A4 e della P-gp: agenti antimicotici azolici (ketoconazolo, itraconazolo, voriconazolo, posaconazolo), inibitori della proteasi dell'HIV (ritonavir, indinavir, nelfinavir, atazanavir, saquinavir), antibiotici macrolidi specifici (claritromicina, telitromicina)
- altri trattamenti antitrombotici: derivati salicilati (aspirina, prodotti contenenti aspirina), terapia antipiastrinica, eparina (eparina non frazionata, eparina a basso peso molecolare, danaparoide sodique, fondaparinux), irudine, anticoagulanti orali (antagonisti della vitamina K, rivaroxaban, dabigatran)
- Pazienti con frequenza di AST o ALT > 3 volte il limite superiore della norma
- Paziente con tasso di bilirubina > 1,5 volte il limite superiore della norma
- Paziente con tasso di piastrine < 75 G/l
- Paziente con clearance della creatinina (Cockcroft) < 30 ml/mn
- Riscontro incidentale di una trombosi venosa profonda prossimale all'ecografia di screening
- Pazienti che rifiutano o non sono in grado di fornire un consenso scritto alle informazioni
- Paziente incapace di rispettare i requisiti del protocollo, secondo l'opinione dello sperimentatore
- Aspettativa di vita inferiore a 6 mesi
- Pazienti incarcerati
- Gravidanza o possibilità di gravidanza entro 6 mesi
- Donne in età fertile senza contraccezione affidabile
- Ecog > 2
