Typ-2-Diabetes ist ein weit verbreitetes, komplexes Gesundheitsproblem, das zu vielen schwerwiegenden und kostspieligen gesundheitlichen Komplikationen führen kann, wenn es nicht richtig behandelt und kontrolliert wird. Bei einem von vier Veteranen wird Typ-2-Diabetes diagnostiziert. Gefährdete Veteranen, wie Angehörige von Minderheiten, einkommensschwache oder ländliche Veteranen, und Veteranen mit komorbiden psychischen Gesundheitsdiagnosen sind überproportional betroffen und haben mit geringerer Wahrscheinlichkeit ihren Diabetes unter Kontrolle. Das Ziel dieser Studie, die ein randomisiertes vergleichendes Wirksamkeitsdesign verwendet, ist es, die vergleichende Wirksamkeit eines interaktiven, maßgeschneiderten Selbstmanagement-SMS-Protokolls (DD-TXT) im Vergleich zu einer traditionellen Nur-Bildungs-Intervention (DSE) in einer Stichprobe von Veteranen zu testen mit unkontrolliertem Diabetes.
Die Prüfärzte werden aus Veteranen ab 18 Jahren rekrutieren, die an den Studienzentren aktiv betreut werden (wie durch den bevorstehenden geplanten Termin bestimmt) und unkontrollierte Glukosewerte (HbA1c >= 8,0 % für mindestens 50 % der letzten 6 Monate) haben. . Teilnehmer, die diese Bedingungen erfüllen und sich für die Studie anmelden, werden randomisiert einer von zwei Gruppen zugeteilt. Diejenigen, die in die Interventionsgruppe randomisiert wurden, erhalten 6 Monate lang Textnachrichten vom DD-TXT-Protokoll, und die in der Vergleichsgruppe erhalten 6 Monate lang Textnachrichten von DSE, einem SMS-Protokoll nur für die Aufklärung von Diabeteskompetenzen. Das primäre Ergebnis ist die HbA1c-Prozentzeit unter Kontrolle. Zu den sekundären Endpunkten gehören die selbstberichtete Einhaltung der Empfehlungen zur Diabetes-Selbstversorgung (SCI-R), die Diabetes-Selbstwirksamkeit, Diabetes-Distress, LDL und Blutdruckkontrolle. Die Forscher gehen von der Hypothese aus, dass DD-TXT zu besseren proximalen Gesundheitsergebnissen und Diabetes-Selbstmanagementverhalten im Vergleich zu einem reinen Schulungsprotokoll (DSE) führen wird.

Einschlusskriterien:
- Veteranen
- aktive VHA-Patienten mit Typ-2-Diabetes
- im vergangenen Jahr mindestens 4 ambulante VA-Begegnungen hatten
- einen zukünftigen Termin geplant haben und nicht ins Krankenhaus eingeliefert oder institutionalisiert sind oder an der Entwicklung von Aim 1 DD-TXT teilgenommen haben
- muss für die Teilnahme Zugriff auf das Handy oder Smartphone eines eigenen oder eines mit ihm zusammenlebenden Familienmitglieds haben. - Sie müssen bereit und in der Lage sein, Textnachrichten zu schreiben, dürfen keine Sehbehinderung haben, die sie daran hindern würde, Textnachrichten zu lesen oder darauf zu antworten, und müssen kognitiv in der Lage sein, der Teilnahme zuzustimmen.
- muss auch HbA1c-Labordaten (2+ Werte) für die 12 Monate vor der RCT-Rekrutierung haben.
- muss eine unzureichende glykämische Kontrolle für mindestens 50 % der 6 Monate vor der Einschreibung haben.

Experimental: DSMS+ (DSMS Plus)
Die Teilnehmer dieser Gruppe wurden für eine interaktive, patientenzentrierte und anpassbare Diabetes-Selbstverwaltungsunterstützung SMS-Intervention (DSMS+) angemeldet.
Dieser Arm erhielt den gleichen täglichen Bildungsinhalt wie der DSMS-Arm, der durch Folgendes verstärkt wurde: reguläre zweiwege-Check-in-Nachrichten über Diabetes-Selbstmanagementverhalten, eine Einladung, Nachrichten zu empfangen, die von anderen Veteranen, die mit Diabetes auf Anforderungen leben, geschrieben wurden, und ein Menü mit optionalem und anpassungsfähigen Modulen (z. B. Medikamentenanträge). unterstützte ihre Diabetes-Selbstmanagementziele.
Die Patienten konnten das Timing und die Häufigkeit ihrer Nachrichten anpassen.

The investigators examined the changes in HbA1c percent time in control.
The cutoff for being in control was a HbA1c of 8.0 percent.
Using HbA1c measures pulled from the electronic medical record data, we estimated the percentage of time HbA1c<8.0% in the 6 month window prior to enrollment and during the 6 month trial.
We then calculated the change in HbA1c percent time in control for each participant.

The score was calculated by averaging the 12 items relevant to type 2 diabetes, each item scored 1(Never) to 5(Always), and converting to a 0-100 scale.
A higher score was better.
