Beschreibung
Einschlusskriterien:
1) Patienten im Alter von 18 bis 80 Jahren unabhängig vom Geschlecht. 2) Patienten mit ECOG (Eastern Cooperative Oncology Group) (ECOG) ≤ 2 und American Society of Anaesthesiologen (ASA) Score ≤ 2, die Kandidaten für eine kurative D2 -Gastrektomie sind.
3) Patienten ohne vorherige Magen -Darm -Operation, Chemotherapie oder Strahlentherapie.
4) Patienten mit normaler Leber-, Nieren-, Herz-, Lungen- und Knochenmarkfunktion (GPT × 109 /L, PLT>109 /L).
5) Patienten, die in der Lage sind, das Forschungsprotokoll zu verstehen und einzuhalten. 6) Patienten, die entweder persönlich oder durch einen gesetzlichen Vertreter eine schriftliche Einverständniserklärung abgeben können.
7) Patienten mit cT1NxM0-Magenkrebs oder nach nicht-kurativer ESD-Resektion, gemäß dem UICC TNM-Stufensystem, 8. Auflage.
Ausschlusskriterien:
1) Patienten mit einer Kontraindikation für eine Magenspiegelung. 2) Patienten mit unkontrollierbaren Krankheiten wie Gerinnungsstörungen, Epilepsie, Erkrankungen des Zentralnervensystems oder psychischen Störungen, Herz-Lungen-Insuffizienz, instabiler Angina pectoris, Myokardinfarkt, einem innerhalb von 6 Monaten aufgetretenen zerebrovaskulären Unfall und anderen chirurgischen Kontraindikationen.
3) Patienten, die aufgrund von Erkrankungen im Zusammenhang mit anderen Organen oder einer Operation nicht bereit sind, sich einer vollen Anästhesie oder einer chirurgischen Behandlung zu unterziehen.
4) Patienten mit Magenstumpfkrebs, rezidivierendem Magenkrebs, mehreren primären bösartigen Tumoren in der Bauch-Becken-Höhle oder einer Vorgeschichte anderer bösartiger Tumoren in den letzten 5 Jahren.
5) Schwangere oder stillende Frauen. 6) Teilnehmer, die an anderen klinischen Studien teilgenommen haben. 7) Patienten mit nicht bestimmbarem Tracer-Färbebereich oder Kontraindikationen für die Verwendung von Tracer.
8) Patienten, die die ESD -Therapie nicht erhalten oder nicht versagen. 9) Patienten, die die absolute Indikation von ESD erfüllen.

Aktiver Komparator: D2 -Gastrektomie
Ein präoperativer kontrastmittelverstärkter CT-Scan des Abdomens wird durchgeführt, um die Lage der Läsion, die Tumorgröße und die Lymphknotenmetastasierung (LNM) zu beurteilen.
Die präoperative endoskopische Farbstoffinjektion oder die intraoperative endoskopische Lokalisierung wird verwendet, um die Tumorstelle genau zu identifizieren und angemessene Resektionsränder sicherzustellen.
Das Ausmaß der Lymphknotendissektion (LND) richtet sich nach den japanischen Behandlungsrichtlinien für Magenkrebs 2023 (6. Auflage)[4].
Insbesondere umfasst die distale Gastrektomie D2 die Lymphknoten Nr. 1, 3, 4sb, 4d, 5, 6, 7, 8a, 9, 11p und 12a.
Die proximale Gastrektomie D2 umfasst die Knoten 1, 2, 3a, 4sa, 4sb, 7, 8a, 9, 11p und 12a, während die totale Gastrektomie D2 die Knoten 1, 2, 3, 4sa, 4sb, 4d, 5, 6, 7 umfasst. 8a, 9, 11p, 11d und 12a.

Vorbereitung: Der Patient wird unter Vollnarkose auf dem Rücken gelagert.
Eine Magensonde und ein Foley-Katheter werden eingeführt.
Es werden Antibiotika verabreicht und Folgestrümpfe angelegt.
Portplatzierung: Das Pneumoperitoneum wird über eine Veress-Nadel am Nabel angelegt.
In den oberen Quadranten sind Arbeitsöffnungen angebracht, mit einer fünften Öffnung für die Leberretraktion.
Abdomenuntersuchung: Der Bauch wird auf Metastasen untersucht und eine Peritonealzytologie durchgeführt.
Dissektion und Lymphknotenentfernung: Das Omentum minus wird in der Nähe der Leber durchtrennt und erreicht die Kardia und das Zwerchfell.
Das Lig. gastrocolicus wird entlang des Colon transversum durchtrennt.
Die Dissektion der Lymphknoten beginnt entlang der Milzarterie und setzt sich dann zur linken Magenarterie und den Zöliakieknoten fort.
Die linken Magengefäße werden mit Endoclips kontrolliert. Nach der Lymphknotendissektion wird eine distale subtotale Gastrektomie durchgeführt.
Die Rekonstruktion des Verdauungstrakts erfolgt typischerweise durch eine Mini-Laparotomie.

Experimental: Fluoreszenzgeführte laparoskopisch-endoskopische kooperative Sentinel-Lymphknoten-Navigationschirurgie-Strat
Die laparoskopisch-endoskopische Kooperationschirurgie (LECS) bietet einen gezielteren Ansatz durch die Integration der komplementären Stärken von Endoskopie und Laparoskopie.
LECS ermöglicht eine präzise Zielausrichtung, optimale Resektionsränder und gewebeschonende Exzision.
Folglich erhält LECS die Magenarchitektur und -funktion besser, was möglicherweise zu einer verbesserten postoperativen Genesung und Lebensqualität (QoL) führt.
Dennoch bleibt die derzeitige Evidenz, die LECS für SNNS unterstützt, begrenzt.

Patients without prior ESD received ESD with laparoscopic sentinel basin dissection (LSBD) for ESD resectable lesions, otherwise patients received laparoscopic-endoscopic cooperative regional gastrectomy (LRG) with LSBD. For patients with prior non-curative ESD, LSBD alone was performed if margins were negative, otherwise LRG with LSBD was conducted. Intraoperative frozen-section pathological examination of the horizontal resection margin of the ESD or full-thickness specimen. During laparoscopic sentinel node basin dissection (LSBD), indocyanine green (ICG) (2 mL, 2.5 mg/mL) was endoscopically injected into the submucosal layer at four quadrants around the marking points (0.5 mL per quadrant). Fifteen minutes after ICG injection, sentinel lymph node basins (SLBs) were defined as the area within a 2 cm margin of the detected fluorescence stained nodes, which were marked with laparoscopic clips. |
