Estudio de cohorte prospectivo sobre enfermedad renal crónica en niños (CKiD) (CKiD)

Desde su inicio en 2003, los objetivos científicos de CKiD han sido determinar los factores de riesgo para la disminución de la función renal y definir cómo la disminución progresiva de la función renal afecta los biomarcadores de los factores de riesgo de enfermedad cardiovascular; función neurocognitiva y comportamiento; y el retraso del crecimiento y su morbilidad asociada.

El estudio CKiD es un estudio de cohorte prospectivo multicéntrico de niños de 6 meses a 16 años con insuficiencia renal de leve a moderada.
La población de estudio está compuesta por tres cohortes.
Descripción
Criterios de inclusión:
- Edad entre 1 y 16 años (antes de los 17 años) para las Cohortes 1 y 2; edad entre 6 meses y 16 años (antes de los 17 años) para la cohorte 3
- GFR de Schwartz estimado (basado en SCr) entre 30 y 90 ml/min|1,73 m2 para la cohorte 1 O una TFG estimada entre 45 y 90 ml/min|1,73 m2 basado en la fórmula de Schwartz actualizada para la Cohorte 2
- Voluntad y capacidad para proporcionar consentimiento informado y asentimiento
- Para la Cohorte 3, se inscribirán niños con diagnóstico no glomerular y duración de la enfermedad renal de menos de 5 años.
Criterio de exclusión:
- Trasplante renal, de otro órgano sólido, de médula ósea o de células madre
- Tratamiento de diálisis en los últimos tres meses
- Diagnóstico de cáncer o diagnóstico/tratamiento de VIH en los últimos doce meses
- Embarazo actual o embarazo en los últimos doce meses
- Incapacidad para completar los principales procedimientos de recopilación de datos.
- Inscripción actual en un ensayo clínico aleatorizado en el que se desconoce el tratamiento específico
- No habla inglés o español con fluidez.
- Planes para mudarse fuera del área de cualquier sitio CKiD participante (las familias pueden ser transferidas a otro sitio CKiD si se mudan)
- Antecedentes de cardiopatía estructural.
- Síndromes genéticos que afectan al sistema nervioso central (p. ej., síndrome de Down)
- Antecedentes de retraso mental severo a profundo (es decir, cociente de inteligencia (CI) <40, deterioro significativo en la función de adaptación y/o incapacidad para ejecutar de forma independiente las habilidades de cuidado personal)
- Para la cohorte 3, no se reclutarán los niños que se espera que reciban terapia de reemplazo renal dentro de los 6 meses posteriores a la fecha de inscripción.
