Genotipo 1/2-MC4R y aumento de peso inducido por fármacos antipsicóticos pediátricos

Realizaremos un estudio abierto aleatorizado de 12 semanas, que comparará el tratamiento antipsicótico de atención habitual (CU) (aripiprazol, quetiapina, risperidona) con ziprasidona (ZIP) en niños y adolescentes de 13 a 18 años.
Los pacientes tendrán 10 días o menos de exposición a los antipsicóticos de por vida y tendrán una necesidad clínica de tratamiento antipsicótico para un trastorno psiquiátrico pediátrico con indicación de la FDA para el uso de antipsicóticos, es decir, manía bipolar, trastornos del espectro esquizofrénico e irritabilidad asociada con el trastorno autista.
Además, también incluiremos a jóvenes que cumplan con los criterios de diagnóstico de investigación para la desregulación grave del estado de ánimo (SMD).
La aleatorización se estratificará según el riesgo genético alto o bajo de aumento de peso inducido por antipsicóticos en función del genotipo MC4R y el resultado principal será el cambio de peso desde el inicio hasta el final entre el tratamiento con antipsicóticos ZIP y UC en cada uno de los dos grupos de genotipo.
Como se detalla a continuación, también se medirán y compararán otros parámetros metabólicos y de seguridad cardíaca entre tratamientos en cada uno de los grupos de genotipo.

Criterios de inclusión:
- edad 13-18 años
- Habla ingles
- Diagnósticos del DSM que tienen una indicación de la FDA para el uso de SGA para al menos un agente en el grupo de edad pediátrico o adulto respectivo. Específicamente, el diagnóstico primario según el DSM-IV de los trastornos del espectro de la esquizofrenia de aparición temprana; manía del trastorno bipolar I (y BP-NOS); irritabilidad asociada con el trastorno del espectro autista, así como una grave desregulación del estado de ánimo (SMD) según Leibenluft et al. (2011) con una puntuación de irritabilidad ABC de >/= 18 Las niñas sexualmente activas deben estar de acuerdo en usar dos formas efectivas de control de la natalidad o abstenerse
- El participante tiene un cuidador principal que ha conocido bien al niño durante al menos 6 meses antes de ingresar al estudio. El cuidador principal puede participar en las citas del estudio.
- Capacidad del niño para participar en todos los aspectos del protocolo según el juicio clínico del investigador.
Criterio de exclusión:
- Las principales enfermedades neurológicas o médicas que afectan el peso (p. ej., enfermedad tiroidea inestable), requieren un medicamento sistémico prohibido (p. ej., diabetes mellitus [insulina], insuficiencia renal crónica [esteroides]); Glucosa en ayunas > 125 mg/dL en 2 ocasiones durante el cribado
- Cualquier medicamento (aparte de los medicamentos psicotrópicos recetados actualmente) que alteraría significativamente el peso
- Antidepresivos no permitidos durante al menos 2 semanas en pacientes BP-I o BP-NOS
- Diagnóstico DSM-IV de anorexia o bulimia nerviosa
- Diagnóstico DSM-IV de trastorno por dependencia de sustancias (que no sea dependencia del tabaco) en el último mes
- El examen inicial de toxicología en orina y el examen de seguimiento indican el uso continuo de sustancias ilícitas
- Hipersensibilidad a los antipsicóticos ZIP o UC
- Embarazada, amamantando o no dispuesta a cumplir con los requisitos anticonceptivos
- Selección o QTc inicial > 450 ms
- CI < 55
- Riesgo significativo de peligrosidad para uno mismo o para los demás
- Abuso infantil en curso o no revelado anteriormente que requiere la intervención de un nuevo departamento de servicio social.
