Prise de poids induite par le génotype 1/2-MC4R et les antipsychotiques pédiatriques

Nous allons mener une étude ouverte randomisée de 12 semaines, comparant un traitement antipsychotique de soins habituels (CU) (aripiprazole, quétiapine, rispéridone) à la ziprasidone (ZIP) chez des enfants et des adolescents âgés de 13 à 18 ans.
Les patients auront 10 jours ou moins d'exposition aux antipsychotiques à vie et auront un besoin clinique de traitement antipsychotique pour un trouble psychiatrique pédiatrique avec indication de la FDA pour l'utilisation d'antipsychotiques, c'est-à-dire la manie bipolaire, les troubles du spectre de la schizophrénie et l'irritabilité associée au trouble autistique.
En outre, nous inclurons également des jeunes répondant aux critères de diagnostic de la recherche pour la dysrégulation sévère de l'humeur (DMS).
La randomisation sera stratifiée en fonction du risque génétique élevé ou faible de prise de poids induite par les antipsychotiques en fonction du génotype MC4R et le résultat principal sera le changement de poids entre la ligne de base et le point final entre le traitement antipsychotique ZIP et UC dans chacun des deux groupes de génotype.
Comme détaillé ci-dessous, d'autres paramètres de sécurité métaboliques et cardiaques seront également mesurés et comparés entre les traitements dans chacun des groupes de génotypes.

Critère d'intégration:
- âge 13-18 ans
- anglophone
- Diagnostics DSM qui ont une indication FDA pour l'utilisation de SGA pour au moins un agent dans le groupe d'âge pédiatrique ou adulte respectif. Plus précisément, le diagnostic primaire du DSM-IV des troubles précoces du spectre de la schizophrénie ; manie de trouble bipolaire I (et BP-NOS); l'irritabilité associée aux troubles du spectre autistique, ainsi qu'une dérégulation sévère de l'humeur (SMD) selon Leibenluft et al. (2011) avec un score d'irritabilité ABC >/=18 Les filles sexuellement actives doivent accepter d'utiliser deux formes efficaces de contraception ou être abstinentes
- Le participant a un gardien principal qui connaît bien l'enfant depuis au moins 6 mois avant l'entrée à l'étude Le gardien principal est en mesure de participer aux rendez-vous de l'étude
- Capacité de l'enfant à participer à tous les aspects du protocole selon le jugement clinique de l'investigateur.
Critère d'exclusion:
- Les maladies neurologiques ou médicales majeures qui affectent le poids (par exemple, une maladie thyroïdienne instable), nécessitent un médicament systémique interdit (par exemple, le diabète sucré [insuline], l'insuffisance rénale chronique [stéroïdes); Glycémie à jeun > 125 mg/dL à 2 reprises lors du dépistage
- Tout médicament (autre que les médicaments psychotropes actuellement prescrits) qui modifierait considérablement le poids
- Antidépresseurs non autorisés pendant au moins 2 semaines chez les patients BP-I ou BP-NOS
- Diagnostic DSM-IV de l'anorexie ou de la boulimie nerveuse
- Diagnostic DSM-IV de trouble de dépendance à une substance (autre que la dépendance au tabac) au cours du mois précédent
- Le dépistage toxicologique initial de l'urine et le dépistage de suivi indiquent l'utilisation continue de substances illicites
- Hypersensibilité aux antipsychotiques ZIP ou UC
- Enceinte, allaitante ou refusant de se conformer aux exigences en matière de contraception
- Dépistage ou QTc de base > 450 msec
- QI < 55
- Risque important de dangerosité pour soi ou pour autrui
- Abus d'enfants en cours ou non divulgué auparavant nécessitant l'intervention d'un nouveau département des services sociaux.
