Genótipo 1/2-MC4R e ganho de peso induzido por drogas antipsicóticas pediátricas

Conduziremos um estudo aberto randomizado de 12 semanas, comparando o tratamento antipsicótico de cuidado usual (UC) (aripiprazol, quetiapina, risperidona) com ziprasidona (ZIP) em crianças e adolescentes de 13 a 18 anos de idade.
Os pacientes terão 10 dias ou menos de exposição antipsicótica ao longo da vida e terão necessidade clínica de tratamento antipsicótico para um transtorno psiquiátrico pediátrico com indicação do FDA para uso de antipsicóticos, ou seja, mania bipolar, transtornos do espectro da esquizofrenia e irritabilidade associada ao transtorno autista.
Além disso, também incluiremos jovens que preencham os critérios diagnósticos de pesquisa para desregulação grave do humor (SMD).
A randomização será estratificada por risco genético alto versus baixo para ganho de peso induzido por antipsicóticos com base no genótipo MC4R e o resultado primário será alteração de peso desde a linha de base até o ponto final entre o tratamento antipsicótico ZIP e UC em cada um dos dois grupos de genótipos.
Conforme detalhado abaixo, outros parâmetros metabólicos e de segurança cardíaca também serão medidos e comparados entre tratamentos em cada um dos grupos de genótipos.

Critério de inclusão:
- idade 13-18 anos
- fala inglês
- Diagnósticos DSM que têm uma indicação do FDA para uso de SGA para pelo menos um agente na respectiva faixa etária pediátrica ou adulta. Especificamente, diagnóstico primário do DSM-IV de transtornos do espectro da esquizofrenia de início precoce; mania de transtorno bipolar I (e BP-NOS); irritabilidade associada ao transtorno do espectro do autismo, bem como desregulação grave do humor (SMD) de acordo com Leibenluft et al. (2011) com um escore ABC de irritabilidade de >/=18 Meninas sexualmente ativas devem concordar em usar duas formas eficazes de controle de natalidade ou ser abstinentes
- O participante tem um cuidador principal que conhece bem a criança por pelo menos 6 meses antes da entrada no estudo O cuidador principal pode participar das consultas do estudo
- Capacidade da criança de participar de todos os aspectos do protocolo de acordo com o julgamento clínico do investigador.
Critério de exclusão:
- Principais doenças neurológicas ou médicas que afetam o peso (por exemplo, doença instável da tireoide), requerem uma medicação sistêmica proibida (por exemplo, diabetes mellitus [insulina], insuficiência renal crônica [esteróides); Glicemia em jejum > 125 mg/dL em 2 ocasiões durante a triagem
- Qualquer medicamento (além dos medicamentos psicotrópicos atualmente prescritos) que altere significativamente o peso
- Antidepressivos não permitidos por pelo menos 2 semanas em pacientes com BP-I ou BP-NOS
- Diagnóstico DSM-IV de anorexia ou bulimia nervosa
- Diagnóstico DSM-IV de transtorno de Dependência de Substância (exceto dependência de tabaco) no último mês
- A triagem inicial de toxicologia urinária e a triagem de acompanhamento indicam o uso contínuo de substâncias ilícitas
- Hipersensibilidade a antipsicóticos ZIP ou UC
- Grávida, amamentando ou não querendo cumprir os requisitos contraceptivos
- Triagem ou linha de base QTc > 450 mseg
- QI < 55
- Risco significativo de perigo para si ou para os outros
- Abuso infantil em andamento ou não revelado anteriormente, exigindo intervenção do novo departamento de serviço social.
