Descripción
Criterios de inclusión:
Mujeres adultas (mayores o iguales a 18 años de edad) de bajo riesgo prenatal, saludables, que incluyen:
- Capaz de hablar y entender inglés
- Mujeres con gestación única
- Capaz de deambular sin restricciones de movilidad (es decir, sin dificultad para pasar de estar sentado a estar de pie)
- Obesidad inferior a clase III (IMC <40 kg/m2) al inicio de la atención prenatal
- Sin infecciones activas como VIH, Hepatitis B, Hepatitis C, brote de HSV (en profilaxis aceptable), GBS positivo es aceptable
- Sin condiciones médicas preexistentes tales como: enfermedad cardíaca, asma no controlada, diabetes de cualquier tipo, hipertensión crónica u otra condición que requiera observación continua y/o restricciones de actividad
Sin condiciones de embarazo de alto riesgo: incluyendo preeclampsia, hipertensión gestacional, gestación prematura, gestación múltiple, abuso de sustancias, desprendimiento de placenta u otro sangrado vaginal inexplicable, cesárea previa, sospecha de macrosomía fetal (> 4500 g) o restricciones de crecimiento intrauterino (< percentil 10) , u otra condición que requiera observación continua y/o restricciones de actividad
Criterios de inclusión laboral:
- Más de 37 semanas y menos de 42 semanas completas de gestación en presentación de vértice
- No hipertensos ni febriles (dos presiones arteriales de 140/90 con cuatro horas de diferencia; dos fiebres de más de 100,4 con una hora de diferencia)
- Tonos cardíacos fetales de categoría 1 (obtenidos en una tira de monitor fetal externo de admisión de 20 minutos)
- El saco amniótico puede estar intacto o roto. Si se rompe, el líquido amniótico debe ser claro.
Criterio de exclusión:
Las mujeres serán excluidas del estudio si no cumplen con los criterios de inclusión anteriores o serán excluidas en las siguientes circunstancias:
- Necesidad conocida de cesárea
- El participante puede ser excluido del estudio en cualquier momento a discreción de la partera (se documentará el motivo de la exclusión del estudio, pero permanecerá en el grupo de estudio previamente seleccionado según la intención de tratar).

Las mujeres en el grupo de parto en tierra trabajarán y darán a luz de acuerdo con los procedimientos estándar de atención.

Experimental: Parto en el agua
Las mujeres del grupo de parto en el agua darán a luz en el agua.
Durante la primera etapa del trabajo de parto, las mujeres pueden entrar o salir del agua en cualquier momento.

El grupo de parto en el agua usará una tina de agua para el trabajo de parto y el parto.

Median Time of Labor Duration
Periodo de tiempo: Time of labor onset to time of birth
|
The median time of labor duration (1st stage onset of strong regular contractions resulting in cervical change, 2nd stage pushing and birth of the neonate, 3rd stage delivery of the placenta) of women in the waterbirth cohort will be compared to the land birth cohort.

United States Birth Satisfaction Scale-Revised (US-BSS-R) Score
Periodo de tiempo: Up to 72 hours post delivery
|
The US-BSS-R is a 10 question scale using a 5 point Likert scale with answers ranging from "strongly agree" to "strongly disagree".
The overall US-BSS-R total birth satisfaction score ranging from 0-40 (least birth satisfaction - most birth satisfaction).
Women in the waterbirth cohort will be compared to those in the land birth cohort for overall US-BSS-R score.

Number of Reported Maternal Adverse Outcomes
