Descrizione
Criterio di inclusione:
Donne adulte prenatali sane a basso rischio (di età superiore o uguale a 18 anni), tra cui:
- In grado di parlare e capire l'inglese
- Donne con gestazione singola
- In grado di deambulare senza limitazioni di mobilità (ovvero, nessuna difficoltà a passare da seduti a in piedi)
- Obesità inferiore alla classe III (BMI <40 kg/m²) all'inizio dell'assistenza prenatale
- Nessuna infezione attiva come HIV, epatite B, epatite C, focolaio di HSV (con profilassi accettabile), GBS positivo è accettabile
- Nessuna condizione medica preesistente come: malattie cardiache, asma non controllato, diabete di qualsiasi tipo, ipertensione cronica o altra condizione che richieda osservazione continua e/o limitazioni di attività
Nessuna condizione di gravidanza ad alto rischio: tra cui preeclampsia, ipertensione gestazionale, gestazione pretermine, gestazione multipla, abuso di sostanze, distacco della placenta o altro sanguinamento vaginale inspiegabile, precedente taglio cesareo, sospetta macrosomia fetale (>4500gm) o restrizione della crescita intrauterina (<10° percentile) , o altra condizione che richieda un'osservazione continua e/o limitazioni di attività
Criteri di inclusione lavorativa:
- Più di 37 settimane e meno di 42 settimane completate di gestazione nella presentazione del vertice
- Non iperteso o febbrile (due valori pressori 140/90 a distanza di quattro ore; due febbri superiori a 100,4 a distanza di un'ora)
- Toni cardiaci fetali di categoria 1 (ottenuti su una striscia del monitor fetale esterno di ammissione di 20 minuti)
- Il sacco amniotico può essere intatto o rotto. Se rotto, il liquido amniotico deve essere limpido.
Criteri di esclusione:
Le donne saranno escluse dallo studio se non soddisfano i criteri di inclusione di cui sopra o saranno ulteriormente escluse nelle seguenti circostanze:
- Necessità nota di taglio cesareo
- Il partecipante può essere escluso dallo studio in qualsiasi momento a discrezione dell'assistente al parto (il motivo dell'esclusione dallo studio sarà documentato ma rimarrà nel gruppo di studio precedentemente selezionato in base all'intenzione di trattare).

Le donne nel gruppo di nascita in terra lavoreranno e partoriranno secondo le procedure standard di cura.

Sperimentale: Nascita in acqua
Le donne del gruppo parto in acqua partoriranno in acqua.
Durante la prima fase del travaglio, le donne possono entrare o uscire dall'acqua in qualsiasi momento.

Il gruppo per il parto in acqua utilizzerà una vasca d'acqua per il travaglio e il parto.

Median Time of Labor Duration

|
The median time of labor duration (1st stage onset of strong regular contractions resulting in cervical change, 2nd stage pushing and birth of the neonate, 3rd stage delivery of the placenta) of women in the waterbirth cohort will be compared to the land birth cohort.

United States Birth Satisfaction Scale-Revised (US-BSS-R) Score
Lasso di tempo: Up to 72 hours post delivery
|
The US-BSS-R is a 10 question scale using a 5 point Likert scale with answers ranging from "strongly agree" to "strongly disagree".
The overall US-BSS-R total birth satisfaction score ranging from 0-40 (least birth satisfaction - most birth satisfaction).
Women in the waterbirth cohort will be compared to those in the land birth cohort for overall US-BSS-R score.
