Tämän kliinisen tutkimuksen tavoitteena on testata, tuottaako ketogeenisen ruokavalion (KD) yhdistäminen henkilökohtaiseen, nopeutettuun intermitenttiseen thetasalvostimulaatioon (iTBS) suurempia masennusoireiden vähennyksiä kuin iTBS yhdistettynä vakioon terveelliseen ruokavalioon hoidolle vastustuskykyistä masennusta sairastavilla aikuisilla. Tutkimuksen tavoitteena on myös selvittää, voivatko osallistujat käytännössä noudattaa ketogeenista ruokavaliota nopeutetun iTBS-hoidon aikana ja tuottaako ruokavalio mitattavia muutoksia ketonitasoissa.

Erityisesti tutkimuksen tavoitteena on selvittää, tuottaako yhdistetty interventio:

  1. Masennusoireiden vähenemistä
  2. Kierroksessa olevien ketonitasojen nousua
  3. Onko se käytännöllinen ja siedettävä nopeutetun iTBS-hoidon aikana

Osallistujat aloittavat joko KD:n tai Kanadan elintarvikeoppaan mukaisen ruokavalion (CFGD) 3 viikon ruokavaliojohdatuksen jälkeen, minkä jälkeen he käyvät läpi henkilökohtaisen, nopeutetun iTBS-kurssin jatkaen määrättyä ruokavaliota. Ennen ja jälkeen iTBS-hoitokurssin osallistujat suorittavat kliinisiä arviointeja, antavat verinäytteitä metabolisten testien suorittamiseksi ja käyvät läpi MRI-skannauksia aivoyhteyksien arvioimiseksi. Ketonitasoja mitataan päivittäin koko 15 viikon ruokavaliointervention ajan. Osallistumiskriteerit:

  • Ikä 18–65 vuotta, mikä tahansa sukupuoli, sukupuoli-identiteetti, etninen tausta ja sosioekonominen asema
  • Käynnissä oleva masennusjakso DSM-5-TR-kriteerien mukaan, jonka tutkimuslääkäri on vahvistanut

Kokeellinen: Ketogenic Diet
Participants in this arm will follow a well-formulated ketogenic diet (low carbohydrate, moderate protein, high fat) for a 3-week dietary lead-in period prior to neuromodulation, and will continue the diet for a total of 12 weeks. The diet is designed to achieve and maintain nutritional ketosis (blood ketone levels of 0.5 to 3 mmol/L). Dietitian support will be provided through scheduled counseling and ongoing monitoring of finger-stick ketone and glucose testing.
Intermittent theta burst stimulation (iTBS), a form of repetitive transcranial magnetic stimulation (rTMS), is a non-invasive brain stimulation technique approved by the FDA and Health Canada for the treatment of TRD. In this study, participants will receive an accelerated course of imaging-guided, neuronavigated left dorsolateral prefrontal cortex (DLPFC) iTBS targeted based on functional connectivity with the subgenual anterior cingulate cortex (sgACC). Stimulation protocol consists of 1800 pulses per session, delivered at 110% of resting motor threshold, with 50-minute inter-session intervals, for 8 sessions per day over 5 consecutive days.

Participants in this arm will follow a Canadian Food Guide-aligned diet for a 3-week dietary lead-in period prior to neuromodulation and will continue the diet for a total of 12 weeks. The diet will emphasize balanced intake of vegetables, fruits, whole grains, and protein foods, without specific macronutrient restrictions. Dietitian counseling will be approximately matched in frequency and duration to the ketogenic diet arm. Nutritional monitoring will include dietary logs and metabolic assessments without targeted induction of ketosis. Participants will perform finger-stick glucose testing.

IPD-suunnitelman kuvaus

Tunnistetiedoton yksittäisten osallistujien data (IPD) ja asiaankuuluva tukidokumentaatio (mukaan lukien tutkimussuunnitelma, tilastollinen analyysisuunnitelma ja tietosanakirja) jaetaan mahdollistaakseen toissijaisen tutkimuksen. Data jaetaan päteville tutkijoille tieteellisesti perusteltuja analyysejä varten, mikäli data-käyttösopimus hyväksytään ja allekirjoitetaan Sunnybrook Research Instituten käytäntöjen mukaisesti. Pääsyä koskevia pyyntöjä tarkistetaan varmistaakseen tieteellisen arvon, toteuttamiskelpoisuuden ja osallistujien luottamuksellisuuden suojelun.

IPD-jaon aikakehys

Tutkimussuunnitelma ja tietoiseen suostumukseen perustuva lomake julkaistaan ennen tutkimuksen aloittamista. Tutkimustulokset levitetään mahdollisimman pian tutkimuksen päätyttyä. Anonymisoitu IPD-data tulee saataville 12 kuukautta ensisijaisten tulosten julkaisun jälkeen (tai 12 kuukautta tutkimuksen päätyttyä, jos julkaisua ei tapahdu).

IPD-jaon käyttöoikeuskriteerit

Jaetut tiedot sisältävät anonymisoidut IPD-tiedot ja tukiasiakirjat (tutkimusprotokolla, tilastollinen analyysisuunnitelma ja tietosanakirja).

Pääsy rajoitetaan tutkijoille, joilla on asianmukainen institutionaalinen yhteydenpito ja dokumentoitu eettinen hyväksyntä, ja jotka esittävät metodologisesti kestävän ehdotuksen. Pyynnöt arvioi tutkimuksen pääinvestigaattori (tai nimetty tietojen käyttökomitea).

Hyväksytyt käyttäjät saavat pääsyn turvallisten, institutionaalisesti hyväksyttyjen tietojen jakamisjärjestelmien kautta. Kaikkien käyttäjien on allekirjoitettava tietojen käyttösopimus, joka kieltää uudelleen tunnistamisen, edelleen jakamisen ja minkä tahansa käytön hyväksytyn ehdotuksen ulkopuolella.
