Dieta Chetogenica Basso-Carboidrato Adiuvante per Migliorare la Neuromodulazione Guidata da Imaging nella Depressione Resistente al Trattamento

L'obiettivo di questo studio clinico è testare se la combinazione di una dieta chetogenica (KD) con una stimolazione intermitente theta burst accelerata e personalizzata (iTBS) produce una riduzione maggiore dei sintomi depressivi rispetto alla iTBS combinata con una dieta sana standard in adulti con depressione resistente al trattamento. Lo studio mira anche a determinare se i partecipanti possono seguire in modo fattibile una dieta chetogenica durante un ciclo di trattamento accelerato con iTBS e se la dieta produce cambiamenti misurabili nei livelli di chetoni.

Nello specifico, lo studio mira a determinare se l'intervento combinato:

  3. È fattibile e tollerabile durante il trattamento accelerato con iTBS

I partecipanti inizieranno una dieta chetogenica (KD) o una dieta allineata alla Guida Alimentare Canadese (CFGD) con un periodo di introduzione dietetica di 3 settimane, dopo il quale seguiranno un ciclo di iTBS accelerata e personalizzata continuando la dieta assegnata. Descrizione

Criteri di inclusione:

  • Età 18-65 anni di qualsiasi sesso, identità di genere, etnia e stato socioeconomico
  • Attualmente in un episodio depressivo maggiore come definito dai criteri DSM-5-TR e confermato da un medico dello studio
  • Presentazione di almeno una gravità moderata dei sintomi (MADRS ≥ 20)
  • Soddisfacimento dei criteri per la depressione resistente al trattamento (TRD), definita come mancata risposta ad almeno due adeguati tentativi di trattamento antidepressivo
  • Nessuna precedente esperienza di neuromodulazione (nessun precedente rTMS o terapia elettroconvulsiva)
  • In grado di fornire il consenso informato
  • Disponibilità per l'intervento di 15 settimane e disponibilità a seguire una dieta chetogenica o allineata alla Guida alimentare canadese

Sperimentale: Ketogenic Diet
Participants in this arm will follow a well-formulated ketogenic diet (low carbohydrate, moderate protein, high fat) for a 3-week dietary lead-in period prior to neuromodulation, and will continue the diet for a total of 12 weeks. The diet is designed to achieve and maintain nutritional ketosis (blood ketone levels of 0.5 to 3 mmol/L). Dietitian support will be provided through scheduled counseling and ongoing monitoring of finger-stick ketone and glucose testing.
Intermittent theta burst stimulation (iTBS), a form of repetitive transcranial magnetic stimulation (rTMS), is a non-invasive brain stimulation technique approved by the FDA and Health Canada for the treatment of TRD. In this study, participants will receive an accelerated course of imaging-guided, neuronavigated left dorsolateral prefrontal cortex (DLPFC) iTBS targeted based on functional connectivity with the subgenual anterior cingulate cortex (sgACC). Stimulation protocol consists of 1800 pulses per session, delivered at 110% of resting motor threshold, with 50-minute inter-session intervals, for 8 sessions per day over 5 consecutive days.

The diet will emphasize balanced intake of vegetables, fruits, whole grains, and protein foods, without specific macronutrient restrictions. Dietitian counseling will be approximately matched in frequency and duration to the ketogenic diet arm. Nutritional monitoring will include dietary logs and metabolic assessments without targeted induction of ketosis. Participants will perform finger-stick glucose testing.

Descrizione del piano IPD

I dati dei singoli partecipanti anonimizzati (IPD) e la relativa documentazione di supporto (inclusi il protocollo dello studio, il piano di analisi statistica e il dizionario dei dati) saranno condivisi per consentire ricerche secondarie. I dati saranno condivisi con ricercatori qualificati per analisi scientificamente valide, soggette all'approvazione e alla stipula di un accordo di utilizzo dei dati in conformità alle politiche del Sunnybrook Research Institute. Le richieste di accesso saranno esaminate per garantire il merito scientifico, la fattibilità e la protezione della riservatezza dei partecipanti.

Periodo di condivisione IPD

Il protocollo di studio e il modulo di consenso informato saranno pubblicati prima dell'inizio dello studio. I risultati della sperimentazione saranno diffusi il prima possibile dopo il completamento dello studio. I dati dei partecipanti non identificati (IPD) saranno disponibili a partire da 12 mesi dopo la pubblicazione dei risultati primari (o 12 mesi dopo il completamento dello studio se non avviene alcuna pubblicazione).

Criteri di accesso alla condivisione IPD

I dati condivisi includeranno IPD anonimizzati e documenti di supporto (protocollo di studio, piano di analisi statistica e dizionario dei dati).

L'accesso sarà limitato ai ricercatori con un'affiliazione istituzionale appropriata e un'approvazione etica documentata, che presentino una proposta metodologicamente valida. Le richieste saranno esaminate dal Principal Investigator dello studio (o dal comitato di accesso ai dati designato).

Gli utenti approvati otterranno l'accesso attraverso sistemi sicuri di condivisione dei dati approvati a livello istituzionale. Tutti gli utenti devono firmare un accordo di utilizzo dei dati che vieta la re-identificazione, la condivisione successiva e qualsiasi uso al di fuori della proposta approvata.
